【摘要】：目的研究上海市靜安區(qū)人類免疫缺陷病毒(human immunodeficiency virus,HIV)感染者中診斷延遲的比例及危險因素。方法收集靜安區(qū)自首次報告(1995年)至2013年管理的HIV感染者和艾滋病病人,研究被動檢測的病例所占比例及人群特征,分別計算"延遲報告"(late presentation,LP)和"伴進展性HIV感染的報告"(presentation with advanced HIV disease,PAHD)的比例并進行危險因素分析。結果 630例感染者和病人納入研究。227例(36.0%)為被動檢測,年齡≥50歲(OR=2.767,95%CI:1.661~4.612)是被動檢測的危險因素,而同性傳播(OR=0.321,95%CI:0.220~0.469)是被動檢測的保護因素。588例(93.3%)病人的診斷延遲情況明確:其中LP為353例(60.0%),PAHD為212例(36.1%)。被動檢測是LP(OR=1.659,95%CI:1.127~2.442)和PAHD(OR=2.035,95%CI:1.392~2.974)的危險因素,而同性傳播(OR=0.630,95%CI:0.424~0.938)是PAHD的保護因素。結論靜安區(qū)HIV感染者和艾滋病病人診斷延遲情況較嚴重,被動檢測確診的病例更容易出現(xiàn)診斷延遲。目前亟需創(chuàng)新的防控策略和措施來加強HIV感染者的早期檢測。
[Abstract]:Objective to study the proportion and risk factors of delayed diagnosis of HIV infected with human immunodeficiency virus (immunodeficiency) in Jing'an District of Shanghai. Methods the proportion and population characteristics of HIV infected persons and AIDS patients who were managed from the first report (1995) to 2013 in Jing'an District were studied.
