【摘要】：目的通過對嬰幼兒腹瀉病例糞便標(biāo)本進(jìn)行致病性大腸埃希菌的分離鑒定及毒力基因檢測,從分子生物學(xué)方面研究致病性大腸埃希菌引起嬰幼兒腹瀉的分布,以降低嬰幼兒致病性大腸埃希菌的感染率。方法收集江西省兒童醫(yī)院2011-2012年嬰幼兒腹瀉病例357份糞便標(biāo)本,按照致病性大腸埃希菌分離鑒定程序用EC肉湯增菌過夜,增菌后用麥康凱瓊脂培養(yǎng)基分離培養(yǎng),可疑菌用API 20E生化鑒定出大腸埃希菌,然后用聚合酶鏈反應(yīng)(PCR)檢測致病性大腸埃希菌相應(yīng)的毒力基因。結(jié)果 357份腹瀉患者糞便標(biāo)本中,檢出37份攜帶毒力基因的致病性大腸埃希菌,檢出率為10.36%,攜帶毒力基因的腸聚集性大腸埃希菌(EAggEC)、腸道致病性大腸埃希菌(EPEC)、腸道侵入性大腸埃希菌(EIEC)、腸道出血性大腸埃希菌(EHEC)檢出率分別為8.4%、0.56%、1.12%、0.28%,產(chǎn)腸毒素大腸埃希菌(ETEC)未檢出。結(jié)論嬰幼兒腹瀉病例中致病性大腸埃希菌主要由EaggEC引起,其次為EIEC,進(jìn)一步證實(shí)對致瀉大腸埃希菌進(jìn)行PCR毒力基因檢測具有重要臨床意義。
[Abstract]:Objective To study the distribution of pathogenic Escherichia coli causing infantile diarrhea from molecular biology through isolation and identification of pathogenic Escherichia coli from feces of infantile diarrhea cases and detection of virulent genes in order to reduce the infection rate of pathogenic Escherichia coli in infants and young children. 357 stool specimens of infantile diarrhea cases were cultured in McConkey agar medium after the addition of pathogenic Escherichia coli by EC broth. The suspicious bacteria were identified by API 20E biochemistry. The virulence genes of pathogenic Escherichia coli were detected by polymerase chain reaction (PCR). Results Among the 357 stool specimens of diarrhea patients, 37 pathogenic Escherichia coli carrying virulent genes were detected, the detection rate was 10.36%. The detection rates of enteric aggregated Escherichia coli (EAggEC), enteropathogenic Escherichia coli (EPEC), enteric invasive Escherichia coli (EIEC) and enterohemorrhagic Escherichia coli (EHEC) carrying virulent genes were respectively. Enterotoxigenic Escherichia coli (ETEC) was not detected in 8.4%, 0.56%, 1.12%, 0.28%. Conclusion The pathogenic Escherichia coli in infantile diarrhea cases was mainly caused by EaggEC, followed by EIEC. It was further confirmed that PCR virulence gene detection of diarrhogenic Escherichia coli had important clinical significance.
