肝細(xì)胞癌多種治療方式的聯(lián)合與對(duì)比研究

研究背景目前,肝細(xì)胞癌(hepatocellular carcinoma,HCC)是全球范圍內(nèi)第5大常見(jiàn)癌癥,也是造成癌癥相關(guān)死亡的第3大病因。在我國(guó),HCC的發(fā)病率和病死率分別位列第4位和第2位。HCC的發(fā)生給患者帶來(lái)了極大的經(jīng)濟(jì)和精神負(fù)擔(dān),但是臨床上HCC的預(yù)后仍不盡如人意。究其原因,一方面,雖然目前HCC的治療在外科切除、肝移植、局部治療(射頻消融(radiofrequency ablation,RFA)、微波消融(microwave ablation,MWA)等方面有很大的發(fā)展,但由于治療方式選擇和策略應(yīng)用缺乏準(zhǔn)確性、個(gè)體化以及HCC本身侵襲性較強(qiáng),常合并有肝硬化,即使在根治治療后,患者的術(shù)后5年復(fù)發(fā)率仍高達(dá)70%,生存率低于20%;另一方面,由于早期HCC無(wú)明顯臨床癥狀,導(dǎo)致相當(dāng)部分的HCC患者在確診時(shí)已處于疾病晚期,失去了根治機(jī)會(huì),僅可接受肝動(dòng)脈化療栓塞(transarterial chemoembolization,TACE)、分子靶向藥物治療等姑息治療方法,療效甚微。因此,針對(duì)HCC患者,如何選擇最佳的治療方式和應(yīng)用策略,探究更加精準(zhǔn)有效的治療靶點(diǎn),有待于進(jìn)一步研究。外...
