【摘要】：目的:通過觀察溫胃方治療前后脾胃虛寒型慢性非萎縮性胃炎患者的臨床癥狀、中醫(yī)癥候等變化情況,探討溫胃方治療脾胃虛寒型慢性非萎縮性胃炎的認識及臨床應用價值,評價其療效性及安全性。方法:采取隨機對照的研究方法,收集符合納入標準的脾胃虛寒型慢性非萎縮性胃炎患者72例,按隨機數字表法分為治療組與對照組各36例,治療組予中藥"溫胃方"治療,對照組予西藥"復方鋁酸鉍顆粒"治療,兩組均以6周為一療程,對治療前后兩組的臨床綜合療效、中醫(yī)癥候、臨床癥狀、胃鏡檢查及病理檢查改善情況進行療效評定,以及血常規(guī)、肝腎功能、二便及心電圖等安全指標,并進行統(tǒng)計分析,評價溫胃方的療效性及安全性。結果:1.兩組共納入72例受試者,治療組與對照組各36例,治療組完成33例,對照組完成34例,脫落率6.94%。2.總有效率:治療組治療脾胃虛寒型慢性非萎縮性胃炎治愈11例,顯效12例,有效5例,無效5例,總有效率84.8%;對照組治愈5例,顯效8例,有效12例,無效9例,總有效率73.5%,差異具有統(tǒng)計學意義(P0.05)。3.中醫(yī)癥候改善情況:治療組在胃脘或脘腹脹滿、四肢不溫、胃脘疼痛、飲食減少、疲乏無力、大便稀溏等癥候的改善優(yōu)于對照組,其P分別為0.01、0.01、0.05、0.05、0.05、0.05。4.胃鏡改善情況:治療組臨床治愈7例,顯效6例,有效15例,無效5例,總有效率84.8%;對照組治愈2例,顯效2例,有效23例,無效7例,總有效率79.4%,差異具有統(tǒng)計學意義(P0.05)。5.病理改善情況:治療組臨床治愈5例,顯效8例,有效14例,無效6例,總有效率81.8%;對照組治愈3例,顯效1例,有效20例,無效10例,總有效率70.6%,差異具有統(tǒng)計學意義(P0.05)。6.安全性:治療后兩組患者血常規(guī)、肝腎功能、二便及心電圖均未見明顯異常變化。結論:溫胃方在改善脾胃虛寒型慢性非萎縮性胃炎患者的臨床癥狀、中醫(yī)癥候及胃鏡和病理檢查,具有良好的療效性和安全性。
[Abstract]:Objective: to observe the changes of clinical symptoms and symptoms of chronic non-atrophic gastritis of deficiency of spleen and stomach before and after treatment with Wenwei recipe, and to explore the recognition and clinical application value of Wenwei prescription in treating chronic non-atrophic gastritis of deficiency cold type of spleen and stomach.To evaluate its efficacy and safety.Methods: a randomized controlled study was conducted to collect 72 patients with chronic non-atrophic gastritis of spleen and stomach deficiency cold type. According to the method of random digital table, the patients were divided into treatment group (n = 36) and control group (n = 36). The treatment group was treated with traditional Chinese medicine "Wenwei prescription".The control group was treated with "compound bismuth aluminate granules", and the two groups were treated with 6 weeks as a course of treatment. The curative effects of the two groups were evaluated before and after treatment, including clinical symptoms, symptoms, gastroscopy and pathological examination.The safety indexes such as blood routine, liver and kidney function, stool and electrocardiogram were analyzed to evaluate the efficacy and safety of Wenwei prescription.The result is 1: 1.There were 72 subjects in the two groups, 36 cases in the treatment group and 36 cases in the control group, 33 cases in the treatment group and 34 cases in the control group, the shedding rate was 6.94.2.Total effective rate: 11 cases of chronic non-atrophic gastritis with deficiency of spleen and stomach were cured in the treatment group, 12 cases were effective, 5 cases were effective, 5 cases were ineffective, and the total effective rate was 84.8 cases in the treatment group, while in the control group, 5 cases were cured, 8 cases were markedly effective, 12 cases were effective, 9 cases were ineffective.The total effective rate was 73.5, and the difference was statistically significant (P 0.05).The improvement of TCM symptoms in the treatment group was better than that in the control group (P = 0.010.01), and the improvement of the treatment group was better than that of the control group (P < 0.01), and the improvement of the symptoms of the treatment group was better than that of the control group (P = 0.011, 0.01), the limbs were not warm, the stomachache, diet reduction, fatigue and loose stools were better than those in the control group (P < 0.05).In the treatment group, 7 cases were cured, 6 cases were effective, 15 cases were effective, 5 cases were ineffective, the total effective rate was 84.8 cases, while in the control group, 2 cases were cured, 2 cases were markedly effective, 23 cases were effective, 7 cases were ineffective, the total effective rate was 79.4%, the difference was statistically significant.Pathological improvement: in the treatment group, 5 cases were clinically cured, 8 cases were effective, 14 cases were effective, 6 cases were ineffective, the total effective rate was 81.8 cases, while in the control group, 3 cases were cured, 1 case was effective, 20 cases were effective, 10 cases were ineffective, the total effective rate was 70.6%, the difference was statistically significant.Safety: there were no obvious abnormal changes in blood routine, liver and kidney function, stool and electrocardiogram in the two groups after treatment.Conclusion: Wenwei prescription has good efficacy and safety in improving the clinical symptoms, symptoms of TCM, gastroscopy and pathological examination in patients with chronic non-atrophic gastritis of deficiency of spleen and stomach.
