正念訓練對疼痛體驗和疼痛反應(yīng)的影響

【摘要】:自1982年卡巴金創(chuàng)立正念減壓療法以來,已有大量研究在實踐和理論方面證明正念可以有效降低慢性疼痛患者的疼痛。然而,對于“為什么有效”,這一潛在的機制問題目前仍缺少研究。此外,隨著疼痛理論的發(fā)展,研究者已在理論上將疼痛體驗和疼痛反應(yīng)加以區(qū)分,但是,在以往的實證研究中甚少將兩者區(qū)分,且缺少在一個研究中將疼痛反應(yīng)三方面,即災難化認知、疼痛相關(guān)的焦慮情緒、逃避行為,以及疼痛體驗兩方面,即感覺維度疼痛、情感維度痛苦進行全面的考察。 本研究旨在探討正念對疼痛體驗和疼痛反應(yīng)的影響,并初步探索正念干預疼痛的潛在作用機制。 通過兩學期的校園講座招募被試,其中52人完成隨機對照的實驗設(shè)計。被試填寫正念五因素量表(FFMQ)問卷和簡式簡明心境量表(POMS-SF),然后,完成固定時間的冰水冷壓實驗,報告疼痛消除愿望,以及疼痛程度和內(nèi)心痛苦程度,最后,填寫應(yīng)對策略問卷(CSQ),完成實驗后分組,正念組參與8周的正念訓練。訓練結(jié)束后,兩組被試再次進行相同的實驗。出于倫理考慮,在實驗結(jié)束后對控制組被試再進行8周正念訓練。 研究結(jié)果顯示:正念量表中不評判、不反應(yīng)維度與災難化認知、焦慮情緒呈顯著負相關(guān),而與疼痛體驗有負相關(guān)趨勢但不顯著;八周的正念干預顯著降低了參與者的災難化認知以及疼痛相關(guān)的焦慮情緒,而疼痛體驗降低不顯著;災難化認知、疼痛相關(guān)的焦慮情緒以及回避行為均對參與者的內(nèi)心痛苦有顯著的預測作用。 據(jù)此,研究結(jié)論為:相較于疼痛體驗,正念與疼痛反應(yīng)的關(guān)系更加密切;相較于疼痛體驗,正念對疼痛反應(yīng)的干預效果更加明顯;通過參與者一系列的疼痛反應(yīng),可以更好的判斷其內(nèi)心的痛苦程度。綜上,正念對疼痛反應(yīng)的直接影響更明顯,這可能意味著正念干預疼痛的潛在機制是通過改變疼痛反應(yīng)模式進而降低參與者的疼痛體驗,特別是內(nèi)心痛苦體驗的。
[Abstract]:Since Kabakin founded mindfulness decompression therapy in 1982, a large number of studies have proved in practice and theory that mindfulness can effectively reduce pain in patients with chronic pain.However, there is still no research on why this potential mechanism is effective.In addition, with the development of pain theory, researchers have made a theoretical distinction between pain experience and pain response. However, in previous empirical studies, the two are rarely distinguished, and there is a lack of three aspects of pain response in one study.That is, disaster cognition, pain related anxiety, escape behavior, and pain experience, that is, sensory dimension pain, emotional dimension pain for a comprehensive study.The purpose of this study was to explore the effects of mindfulness on pain experience and pain response, and to explore the underlying mechanism of mindfulness intervention in pain.Two-term campus lectures were conducted to recruit participants, 52 of whom completed a randomized controlled experimental design.The subjects filled out the FFMQ questionnaire and the brief mood scale POMS-SFN. Then, they completed the ice water cold pressing experiment for a fixed time to report their desire for pain relief, the degree of pain and the degree of inner pain. Finally, they completed the cold pressing experiment of ice and water for a fixed period of time to report their desire for pain relief, the degree of pain and the degree of inner pain.Fill out the coping strategy questionnaire and divide into groups after the experiment. The mindfulness group participated in 8 weeks of mindfulness training.After the training, the two groups conducted the same experiment again.For ethical reasons, the participants in the control group were trained for 8 weeks after the experiment.The results showed that the dimensionality of non-response was negatively correlated with cognition of disaster, anxiety was negatively correlated with pain experience, but it was not significant in mindfulness scale.Eight weeks of mindfulness significantly reduced the participants' perception of disaster and pain related anxiety, while the pain experience was not significantly reduced;Pain-related anxiety and avoidance behavior were significant predictors of participants' inner pain.Accordingly, the study concluded that mindfulness is more closely related to pain response than pain experience; mindfulness intervention in pain response is more effective than pain experience; through a series of pain reactions,You can better judge the degree of pain in your heart.In conclusion, the direct effect of mindfulness on pain response is more obvious, which may mean that the underlying mechanism of mindfulness intervention in pain is to reduce the pain experience of participants by changing the pain response pattern, especially the pain experience in the heart.
