【摘要】：目的探討血培養(yǎng)與PCT聯(lián)合檢測(cè)在膿毒血癥早期診斷中的價(jià)值。方法采用回顧性研究,從2015年3月到2016年3月,選擇我院45例膿毒血癥患者作為觀察組,選取一般感染者45例作為對(duì)照組。采集所有研究對(duì)象靜脈血液進(jìn)行培養(yǎng)和PCT、CRP含量測(cè)定。結(jié)果觀察組APACHE-II評(píng)分、CRP、PCT和血培養(yǎng)陽性率顯著高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。但兩組平均動(dòng)脈壓,WBC計(jì)數(shù)異常差異無統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組中膿毒血癥患者26例,膿毒血癥合并MODS13例,膿毒性休克6例。三組PCT水平比較有統(tǒng)計(jì)學(xué)差異(P0.05)。根據(jù)住院后第7d患者結(jié)果分為死亡組11例,好轉(zhuǎn)組34例,死亡組住院第1d血漿PCT值顯著高于好轉(zhuǎn)組,隨后好轉(zhuǎn)組患者血漿PCT顯著降低,死亡組血漿PCT顯著升高,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論血漿PCT的值與膿毒血癥患者的病情呈正相關(guān),血培養(yǎng)和PCT聯(lián)合檢測(cè)對(duì)于膿毒血癥的早期診斷和預(yù)后判斷具有重要臨床價(jià)值。
[Abstract]:Objective to evaluate the value of combined detection of blood culture and PCT in the early diagnosis of sepsis. Methods from March 2015 to March 2016, 45 patients with sepsis in our hospital were selected as observation group and 45 patients with general infection as control group. Venous blood was collected from all subjects for culture and PCT,CRP content determination. Results the positive rate of APACHE-II score, CRP,PCT and blood culture in the observation group was significantly higher than that in the control group (P 0.05). However, there was no significant difference in mean arterial pressure and WBC count between the two groups (P 0.05). In the observation group, there were 26 patients with sepsis, 13 patients with sepsis complicated with MODS and 6 patients with septic shock. There was significant difference in PCT level among the three groups (P 0.05). According to the results of the 7th day after hospitalization, the patients were divided into death group (n = 11) and improvement group (n = 34). The plasma PCT level in the death group was significantly higher than that in the improvement group on the first day of hospitalization, and the plasma PCT in the improvement group was significantly lower than that in the improvement group, and the plasma PCT in the death group was significantly increased (P 0.05). Conclusion the value of plasma PCT is positively correlated with the condition of sepsis patients. The combined detection of blood culture and PCT has important clinical value in the early diagnosis and prognosis of sepsis.
