預(yù)存式自體輸血對(duì)擇期手術(shù)患者術(shù)后淋巴細(xì)胞亞群的影響

【摘要】:目的評(píng)價(jià)預(yù)存式自體輸血對(duì)擇期手術(shù)患者術(shù)后免疫功能的影響。方法選擇本院2015年11月-2016年8月符合預(yù)存式自體輸血指征的擇期手術(shù)患者51名,分為自體血組(n=20):術(shù)前采集自體血、術(shù)中回輸;異體血組(n=15):術(shù)前未預(yù)存自體血,術(shù)中輸異體血。未輸血組(n=16):術(shù)前未預(yù)存自體血,術(shù)中未輸注異體血。分析比較3組患者術(shù)前、術(shù)后1 d和5 d CD3+、CD4+、CD8+、CD4+/CD8+和NK細(xì)胞的變化。結(jié)果術(shù)后1 d,CD3+(%)、CD4+(%)及NK細(xì)胞數(shù)(×109),自體血組較異體血組與未輸血組分別為59.81±5.82 vs 50.04±4.37 vs 56.28±5.56,31.06±5.77 vs 22.88±3.80 vs 29.00±2.77,0.17±0.06 vs 0.12±0.05 vs 0.17±0.05,自體血組較異體血組明顯升高(P0.05);術(shù)后5 d CD3+(%)、CD4+(%)、CD4/CD8、NK細(xì)胞比例(%)及絕對(duì)計(jì)數(shù)(×109),自體血組、異體血組與未輸血組分別為69.56±6.02 vs 59.81±5.38 vs 67.93±7.89,42.91±6.08 vs 31.14±3.99 vs 43.40±3.31,1.73±0.36 vs 1.23±0.37 vs 1.77±0.46,12.53±4.33 vs 7.84±3.41 vs 12.11±3.72,0.21±0.11 vs 0.10±0.06 vs 0.18±0.06),自體血組較異體血組明顯升高(P0.05)。結(jié)論預(yù)存式自體輸血較異體輸血對(duì)于患者術(shù)后免疫功能影響較小,對(duì)細(xì)胞免疫功能有正向調(diào)節(jié)作用。
[Abstract]:Objective to evaluate the effect of preexisting autologous blood transfusion on postoperative immune function in patients undergoing elective operation. Methods from November 2015 to August 2016, 51 patients were selected and divided into autologous blood group (n = 20) and autologous blood group (n = 20). Autologous blood was collected before operation and reinfused during operation. Allogeneic blood group (nm15): no autologous blood was stored before operation and allogeneic blood was infused during operation. No autologous blood was prestored before operation and allogeneic blood was not infused during operation in no transfusion group (n = 16). The changes of CD3, CD4, CD8, CD4 / CD8 and NK cells before and after operation were analyzed and compared in three groups. Results 1 day after operation, CD3 (%), CD4 (%) and the number of NK cells (脳 10 ~ 9) were found. The vs of allogeneic blood group and non-transfusion group were 69.56 鹵6.02 vs 59.81 鹵5.38 vs 67.93 鹵7.89 vs 42.91 鹵6.08 vs 31.14 鹵3.99 vs 43.40 鹵3.31 vs 1.73 鹵0.36 vs 1.23 鹵0.37 vs 1.77 鹵0.4612.53 鹵4.33 vs 7.84 鹵3.33 respectively. 41 vs 12.11 鹵3.72 vs 0.21 鹵0.11 vs 0.10 鹵0.06 vs 0.18 鹵0.06). Autologous blood group was significantly higher than that of allogeneic blood group (P0.05). Conclusion Preexisting autologous blood transfusion has less effect on postoperative immune function than allogeneic blood transfusion, and has a positive effect on cellular immune function.
