磁共振擴散加權成像表觀擴散系數與腫塊型浸潤性乳腺癌分子亞型的相關性

【摘要】:目的探討磁共振擴散加權成像表觀擴散系數(ADC)與腫塊型浸潤性乳腺癌5種分子亞型及其生物學預后因子相關性。方法回顧分析64例經手術或活檢病理證實為浸潤性乳腺癌患者(69個腫塊型病灶)的磁共振影像學資料,比較不同分子亞型間平均ADC值及其與乳腺癌生物學預后因子雌激素受體、孕激素受體、人類表皮生長因子受體-2、Ki-67的表達的關系。結果浸潤性乳腺癌5種分子亞型間ADC值無顯著差異(P0.05);雌激素受體、孕激素受體、人類表皮生長因子受體-2陽性、陰性組ADC值差異均無統(tǒng)計學意義(P0.05);但HER2陽性組平均ADC值高于陰性組;Ki-67陽性、陰性組ADC值差異有統(tǒng)計學意義(P=0.023)。陰性組ADC值明顯高于陽性組,ADC值與Ki-67的表達呈負相關(r=-0.249)。結論 ADC不能鑒別腫塊型浸潤性乳腺癌5種分子亞型,但與浸潤性乳腺癌預后因子Ki-67的表達存在相關性,對指導治療和判斷預后有一定意義。
[Abstract]:Objective to investigate the correlation between Mr diffusion weighted apparent diffusion coefficient (ADCC) and five molecular subtypes of invasive breast cancer and their biological prognostic factors. Methods the MRI data of 64 patients with invasive breast cancer (69 mass lesions) confirmed by operation or biopsy were retrospectively analyzed. The mean ADC values among different molecular subtypes were compared and compared with estrogen receptor, a prognostic factor of breast cancer. Expression of progesterone receptor, human epidermal growth factor receptor-2 and Ki-67. Results there was no significant difference in ADC values among the five molecular subtypes of invasive breast cancer (P 0.05), estrogen receptor, progesterone receptor and human epidermal growth factor receptor -2 were positive. There was no significant difference in ADC value between the negative group and the negative group, but the average ADC value in the HER2 positive group was higher than that in the negative group, and the ADC value in the negative group was significantly higher than that in the negative group. The ADC value of negative group was significantly higher than that of positive group. There was a negative correlation between ADC value and Ki-67 expression. Conclusion ADC can not differentiate 5 molecular subtypes of invasive breast cancer, but it is related to the expression of prognostic factor Ki-67 in invasive breast cancer.
