【摘要】：目的以磁共振成像為參照,探討高頻超聲對血友病性關(guān)節(jié)病的特征。方法收集44名血友病患者中70例膝關(guān)節(jié)行超聲檢查,其中11名小于18歲的血友病患者中16例膝關(guān)節(jié)完成磁共振成像(MRI)檢查,同時對膝關(guān)節(jié)按照2.1版血友病關(guān)節(jié)健康評估表(Hemophilia Joint Health Score,HJHS)評價其健康狀況,分析超聲評分與年齡及出血次數(shù)的相關(guān)性,并以磁共振成像為參照,分析關(guān)節(jié)超聲特點(diǎn)及其與、MRI及健康狀況之間的相關(guān)性。結(jié)果血友病性關(guān)節(jié)病變嚴(yán)重程度與血友病的嚴(yán)重程度無明顯相關(guān)性(F=0.013,P=0.987),隨著年齡增加、出血次數(shù)增多,關(guān)節(jié)病變越重;對于關(guān)節(jié)病變早期的滑膜增厚及軟骨改變的檢出,超聲及MRI檢查無明顯差異(P0.05);超聲檢查與MRI存在明顯相關(guān)相關(guān)性(r=0.702,P0.05),超聲檢查與HJHS2.1之間存在明顯相關(guān)性(r=0.659,P0.05)MRI評分與HJHS2.1之間存在明顯的相關(guān)性(r=0.640,P0.05)。結(jié)論隨著年齡及平均出血次數(shù)的增加均能使關(guān)節(jié)病變越重,高頻超聲對于血友病性關(guān)節(jié)病的早期診斷有重要意義,且因其經(jīng)濟(jì)、有效、方便、安全等特征,對臨床具有較好的實(shí)用性。
[Abstract]:Objective to study the characteristics of hemophilia arthropathy by high frequency ultrasound (HFUS) compared with magnetic resonance imaging (MRI) methods 70 of 44 hemophilia patients were examined by ultrasonography. Sixteen of 11 hemophilia patients under the age of 18 completed MRI MRI, and their health status was evaluated according to Hemophilia Joint Health ScoreHJHS2. The correlation of ultrasound score with age and bleeding frequency was analyzed, and magnetic resonance imaging (MRI) was used as a reference. Results there was no significant correlation between the severity of hemophilia and the severity of hemophilia. For early synovial thickening and cartilage change detection, There was no significant difference between ultrasound and MRI examination (P 0.05); there was a significant correlation between ultrasound examination and MRI. There was a significant correlation between ultrasound examination and HJHS2.1. There was a significant correlation between ultrasound examination and HJHS2.1. There was a significant correlation between MRI score and HJHS2.1. Conclusion with age and average bleeding, there is a significant correlation between MRI score and HJHS2.1. An increase in the number of times can make the joint disease worse. High frequency ultrasound is of great significance for the early diagnosis of hemophilia arthrosis, and has good practicability for clinical practice because of its economic, effective, convenient and safe characteristics.
