【摘要】：目的:探討胃癌術(shù)前CT強化率、灌注參數(shù)與血清腫瘤標(biāo)志物含量、腫瘤病灶增殖分子表達的關(guān)系。方法:收集榆林市第二醫(yī)院收治的胃癌患者68例作為觀察組,根據(jù)腫瘤分期分為早中期胃癌組41例、晚期胃癌組27例;取同期在本院就診被證實為胃良性病變的患者50例作為胃良性病變組。采用CT掃描檢測三組患者的CT強化率、灌注參數(shù),采用酶聯(lián)免疫吸附法(ELISA)檢測血清腫瘤標(biāo)志物含量,采用RT-PCR法檢測病灶組織增殖基因mRNA表達量。結(jié)果:晚期胃癌組的CER、AF、BV、CL水平高于早中期胃癌組、胃良性病變組(P0.05);晚期胃癌組的血清CA72-4、CA19-9、CA125、CEA含量高于早中期胃癌組、胃良性病變組(P0.05);晚期胃癌組的組織CADM1、miRNA-34a、Cystatin M mRNA表達量低于早中期胃癌組、胃良性病變組,Survivin、I2PP2A mRNA表達量高于早中期胃癌組、胃良性病變組(P0.05)。經(jīng)Pearson檢驗發(fā)現(xiàn),胃癌患者的CT強化率、灌注參數(shù)與血清腫瘤標(biāo)志物含量、腫瘤病灶增殖分子表達量存在直接相關(guān)關(guān)系。結(jié)論:胃癌術(shù)前CT強化率、灌注參數(shù)與腫瘤惡性程度直接相關(guān),可作為遠期腫瘤診斷及惡性程度判斷的可靠手段。
[Abstract]:Objective: to investigate the relationship between CT enhancement rate, perfusion parameters and serum tumor markers and the expression of proliferative molecules in gastric cancer before operation. Methods: 68 patients with gastric cancer treated in the second Hospital of Yulin City were collected as observation group. According to tumor staging, 41 cases of early and middle stage gastric cancer group and 27 cases of advanced gastric cancer group were divided into three groups, 50 cases of gastric benign lesions were confirmed as gastric benign lesions in our hospital at the same time. Ct scanning was used to detect the enhancement rate of CT in the three groups. The serum tumor markers were detected by Elisa and the expression of proliferative gene mRNA was detected by RT-PCR method. Results: the level of RT-PCR in advanced gastric cancer group was higher than that in early and middle stage gastric cancer group. The level of serum CA72-4, CA19-9, CA125A, CADM1-miRNA-34aCystatin M mRNA in advanced gastric cancer group was higher than that in early and middle stage gastric cancer group, and the expression of survivin I2PP2A mRNA in gastric benign lesion group was higher than that in early metaphase gastric cancer group, while the expression of CADM1-miRNA-34aCystatin M mRNA in advanced gastric cancer group was lower than that in early and middle stage gastric cancer group, and the expression of CA72-4, CA19-9 and CA125A in advanced gastric cancer group was higher than that in early and middle stage gastric cancer group. Pearson test showed that there was a direct correlation between CT enhancement rate, perfusion parameters and serum tumor markers and the expression of proliferative molecules in gastric cancer. Conclusion: the CT enhancement rate of gastric cancer before operation is significantly higher than that of control group. The perfusion parameters are directly related to the malignant degree of the tumor and can be used as a reliable means for the long term diagnosis and the judgement of the malignant degree.
