【摘要】：目的:探討圍手術期腸外營養(yǎng)對胃腸道惡性腫瘤患者術后營養(yǎng)狀況的干預效果。方法:胃腸道惡性腫瘤患者84例,按隨機數(shù)字表法隨機分為實驗組與對照組,每組各42例。實驗組術前7 d開始給予腸外營養(yǎng),對照組常規(guī)給予糖電解質輸液;兩組患者均于術后第1天開始給予等氮等熱量的腸外營養(yǎng)治療,觀察兩組患者術前1 d、術后7 d各項指標的變化。結果:實驗組患者術前1 d的體重指數(shù)(body mass index,BMI)值、血清前白蛋白(prealbumin,PA)和轉鐵蛋白(transferrin,TF)值均高于對照組,差異有統(tǒng)計學意義(P0.05)。術后7 d,實驗組患者的BMI、肱三頭肌皮褶厚度(triceps skinfold thickness,TSF)值、血白蛋白(albumin,ALB)、血紅蛋白(hemoglobin,HGB)、PA和TF值均高于對照組,實驗組肺部感染發(fā)生率低于對照組,差異均有統(tǒng)計學意義(P0.05)。結論:存在營養(yǎng)風險的胃腸道惡性腫瘤患者術前給予腸外營養(yǎng)治療可有效改善患者術后的營養(yǎng)狀況,降低術后肺部感染發(fā)生率。
[Abstract]:Objective: to investigate the effect of perioperative parenteral nutrition on postoperative nutritional status of patients with gastrointestinal malignancy. Methods: 84 patients with gastrointestinal malignancy were randomly divided into experimental group and control group according to random digital table. The experimental group received parenteral nutrition 7 days before operation, the control group received routine infusion of sugar and electrolyte, and the two groups received iso-nitrogen and equal caloric parenteral nutrition on the first day after operation. Results: the body mass index (BMI), serum albumin prealbumin (PAA) and transferrin transferrin (TFF) of the patients in the experimental group were higher than those in the control group 1 day before operation and 7 days after operation. On the 7th day after operation, the BMIs, triceps skinfold thicknessen, albumin albumin, hemoglobulin HGBP and TF of the patients in the experimental group were higher than those in the control group, and the incidence of pulmonary infection in the experimental group was lower than that in the control group. Conclusion: parenteral nutrition therapy can effectively improve the postoperative nutritional status and reduce the incidence of postoperative pulmonary infection in patients with gastrointestinal cancer with nutritional risk.
