【摘要】：醫(yī)療保險(xiǎn)制度是我國(guó)社會(huì)保障事業(yè)的重點(diǎn)，醫(yī)療資源的合理配置是人民健康得以保障的前提，關(guān)系到最廣大人民的切身利益。太原市基本醫(yī)療保險(xiǎn)制度起步較晚，于2003年開(kāi)始新型農(nóng)村合作醫(yī)療制度試點(diǎn)工作，由于太原市長(zhǎng)期實(shí)行城鄉(xiāng)二元經(jīng)濟(jì)結(jié)構(gòu)和社會(huì)結(jié)構(gòu)，城鄉(xiāng)基本醫(yī)療保險(xiǎn)制度存在差異性。與城鎮(zhèn)相比，，太原市農(nóng)村社會(huì)保障體系保障項(xiàng)目少且覆蓋的人數(shù)比重小，雖然近年來(lái)新型農(nóng)村合作醫(yī)療保險(xiǎn)制度覆蓋率和參合率都有很大提升,但是補(bǔ)償保障水平與社會(huì)滿(mǎn)意度依然很低。由此可見(jiàn)，太原市目前的新型農(nóng)村合作醫(yī)療保險(xiǎn)制度存在缺陷,只能在一定程度上緩解農(nóng)民因病致貧、因病返貧的負(fù)擔(dān),由于保障水平偏低,不能從根本上解決農(nóng)民因病致貧、因病返貧的問(wèn)題。 本文在充分分析太原市新型農(nóng)村合作醫(yī)療制度的基礎(chǔ)上，采用文獻(xiàn)綜述法、實(shí)地調(diào)研法、比較研究方法以及定性分析與定量研究相結(jié)合的方式，首先分析了國(guó)內(nèi)外對(duì)新型農(nóng)村合作醫(yī)療制度的研究以及對(duì)本文所需概念和理論支撐進(jìn)行深入分析，包括社會(huì)保障制度、醫(yī)療保險(xiǎn)制度和醫(yī)療保險(xiǎn)城鄉(xiāng)統(tǒng)籌概念的界定以及福利經(jīng)濟(jì)學(xué)理論、醫(yī)療保險(xiǎn)特殊性理論、二元經(jīng)濟(jì)結(jié)構(gòu)理論、城鄉(xiāng)一體化理論；其次，深入研究了太原市農(nóng)村合作醫(yī)療保險(xiǎn)制度的發(fā)展歷程，包括傳統(tǒng)農(nóng)村合作醫(yī)療制度的發(fā)展歷程以及新型農(nóng)村合作醫(yī)療制度的發(fā)展歷程；再次，通過(guò)對(duì)基金籌集、醫(yī)療服務(wù)資源及補(bǔ)償制度等方面的研究，深入分析了太原市新型農(nóng)村合作醫(yī)療制度的供需情況和運(yùn)行現(xiàn)狀，從中發(fā)現(xiàn)了現(xiàn)階段太原市新型農(nóng)村合作醫(yī)療制度存在農(nóng)村內(nèi)部醫(yī)療供需失衡以及運(yùn)行中的問(wèn)題及原因最終提出了完善監(jiān)督管理機(jī)制、提高醫(yī)療資源服務(wù)水平、健全基金籌集和補(bǔ)償制度、走城鄉(xiāng)一體化道路，從而為太原市新型農(nóng)村合作醫(yī)療制度的可持續(xù)發(fā)展提出了建議和對(duì)策。
[Abstract]:The medical insurance system is the focal point of the social security in our country. The rational allocation of the medical resources is the premise of the people's health protection and relates to the vital interests of the vast majority of the people. The basic medical insurance system of Taiyuan started late, and the pilot work of the new rural cooperative medical insurance system began in 2003. Due to the long-term dual economic structure and social structure of urban and rural areas, there are differences between urban and rural basic medical insurance system. Compared with the cities and towns, the rural social security system in Taiyuan has fewer and smaller proportion of the number of people covered, although the coverage and participation rate of the new rural cooperative medical insurance system have greatly improved in recent years. But the level of compensation and social satisfaction is still very low. Thus, it can be seen that the current new rural cooperative medical insurance system in Taiyuan has defects, which can only alleviate the burden of peasants' poverty caused by illness and return to poverty due to illness to a certain extent. Due to the low level of security, it cannot fundamentally solve the problem of peasants' poverty caused by illness. The problem of returning to poverty as a result of illness. Based on the full analysis of the new rural cooperative medical system in Taiyuan, this paper adopts the literature review method, the field investigation method, the comparative research method and the combination of qualitative analysis and quantitative research. First of all, it analyzes the research on the new rural cooperative medical system at home and abroad, as well as the concept and theoretical support needed in this paper, including the social security system. The definition of medical insurance system and the concept of urban and rural planning of medical insurance, as well as the theory of welfare economics, the theory of particularity of medical insurance, the theory of dual economic structure, the theory of urban-rural integration; secondly, The development of rural cooperative medical insurance system in Taiyuan, including the development of the traditional rural cooperative medical system and the development of the new rural cooperative medical system. Thirdly, through raising funds, the paper studies the development process of rural cooperative medical insurance system in Taiyuan, including the development process of traditional rural cooperative medical system and the new rural cooperative medical system. Based on the research of medical service resources and compensation system, this paper deeply analyzes the supply and demand of the new rural cooperative medical system in Taiyuan and its current operating situation. It is found that there is imbalance between the supply and demand of medical service in the new rural cooperative medical system in Taiyuan at the present stage, and the problems and reasons in operation finally put forward to improve the supervision and management mechanism and improve the service level of medical resources. Perfecting the system of fund raising and compensation, taking the road of urban-rural integration, and putting forward suggestions and countermeasures for the sustainable development of the new rural cooperative medical system in Taiyuan.
