【摘要】:目的以重癥監(jiān)護(hù)方向護(hù)理專(zhuān)業(yè)學(xué)位碩士研究生(Master of Nursing Specialist,MNS)應(yīng)當(dāng)具備的核心能力為框架,探討其專(zhuān)科化的培養(yǎng)體系,為各大護(hù)理院校培養(yǎng)研究生水平的高層次、高質(zhì)量重癥監(jiān)護(hù)專(zhuān)科護(hù)士提供理論指導(dǎo),從而促進(jìn)我國(guó)高等護(hù)理教育的發(fā)展和護(hù)理隊(duì)伍的專(zhuān)科化進(jìn)程。方法1.采用文獻(xiàn)回顧法、半結(jié)構(gòu)式專(zhuān)家訪談法初步擬定函詢問(wèn)卷,進(jìn)行預(yù)實(shí)驗(yàn),修改完善問(wèn)卷,形成正式函詢問(wèn)卷,采用德?tīng)柗?Delphi)函詢法對(duì)護(hù)理管理、護(hù)理教育、重癥監(jiān)護(hù)和重癥醫(yī)學(xué)領(lǐng)域的資深專(zhuān)家進(jìn)行兩輪問(wèn)卷調(diào)查,請(qǐng)專(zhuān)家來(lái)評(píng)價(jià)該培養(yǎng)指標(biāo)體系的合理性和重要性。2.采用Excel 2010和SPSS 21.0統(tǒng)計(jì)學(xué)軟件進(jìn)行整理分析。用均數(shù)、標(biāo)準(zhǔn)差和百分比表示專(zhuān)家基本情況,用問(wèn)卷有效回收率表示專(zhuān)家積極系數(shù),用專(zhuān)家的學(xué)術(shù)水平以及專(zhuān)家對(duì)指標(biāo)的熟悉程度和判斷依據(jù)表示專(zhuān)家權(quán)威程度,用重要性賦值均數(shù)(`X)、認(rèn)可率(Cj)、滿分頻率(Kj)、變異系數(shù)(CV)和指標(biāo)的權(quán)重值表示各指標(biāo)的重要性及專(zhuān)家意見(jiàn)集中程度,用Kendall’s W和諧系數(shù)表示專(zhuān)家協(xié)調(diào)程度。3.條目納入標(biāo)準(zhǔn):同時(shí)滿足`X3.5,CV0.25,Cj≥50%,Kj≥20%,否則修改或刪除,最終形成一套系統(tǒng)的符合重癥監(jiān)護(hù)方向護(hù)理專(zhuān)業(yè)學(xué)位碩士研究生的培養(yǎng)體系。結(jié)果1.選取來(lái)自北京市、上海市、廣東省、江蘇省、山東省、遼寧省、河南省7個(gè)省市,涵蓋護(hù)理管理、護(hù)理教育、重癥監(jiān)護(hù)和重癥醫(yī)學(xué)領(lǐng)域的27名專(zhuān)家。年齡平均48.22±9.42(28~65)歲;工作年限平均23.15±10.15(11~49)年。2.經(jīng)過(guò)兩輪函詢后,專(zhuān)家積極系數(shù)分別為:87.9%、93.1%;權(quán)威程度分別為:0.853±0.121,0.877±0.107;Kendall’s W和諧系數(shù)分別為0.264和0.337。3.根據(jù)指標(biāo)納入和排除標(biāo)準(zhǔn),本研究中“組織和系統(tǒng)的領(lǐng)導(dǎo)能力”中“采用定量和定性數(shù)據(jù)評(píng)估循證護(hù)理實(shí)踐效果的能力”CV=0.25,Kj=3.7%,故將其刪除;在臨床實(shí)踐安排中“護(hù)理科研實(shí)踐3個(gè)月”CV=0.33,故將其刪除;將“組織和系統(tǒng)的領(lǐng)導(dǎo)能力”中“通過(guò)跨學(xué)科合作評(píng)估、預(yù)防、降低重癥護(hù)理風(fēng)險(xiǎn)的能力”改為“具有降低重癥護(hù)理風(fēng)險(xiǎn)的能力”;將“科研能力”下設(shè)的各項(xiàng)指標(biāo)整合為“科研實(shí)踐操作能力”,并增加“科研思維能力”和“科研成果應(yīng)用能力”;在課程設(shè)置中,將“護(hù)理理論”改為“高級(jí)護(hù)理理論實(shí)踐”,將“護(hù)理倫理學(xué)”融合到“醫(yī)學(xué)相關(guān)法律法規(guī)”中,并添加“護(hù)理信息學(xué)”等。初步確立了培養(yǎng)目標(biāo)、入學(xué)條件、課程和學(xué)分設(shè)置、臨床實(shí)踐安排、培養(yǎng)方式和質(zhì)量評(píng)價(jià)6個(gè)要素及其延伸的29個(gè)一級(jí)指標(biāo)和78個(gè)二級(jí)指標(biāo)。結(jié)論函詢專(zhuān)家積極程度、權(quán)威程度和意見(jiàn)協(xié)調(diào)程度較好,最終得出的指標(biāo)體系有較高的科學(xué)性、合理性。該培養(yǎng)體系的探討可為我國(guó)各護(hù)理院校MNS培養(yǎng)大綱的制定提供參考和借鑒。但隨著我國(guó)護(hù)理學(xué)科的不斷發(fā)展以及重癥監(jiān)護(hù)專(zhuān)科護(hù)士角色職能的不斷轉(zhuǎn)變,該培養(yǎng)體系的探討還將處于不斷調(diào)整之中。
[Abstract]:Objective to explore the specialized training system of Master of Nursing Specialist-NNS (MNS) in the framework of the core competence of Master of Nursing Specialisten (MNS) in the direction of intensive care, so as to cultivate the high level of graduate students in all nursing colleges and universities. High quality intensive care specialist nurses provide theoretical guidance to promote the development of higher nursing education and the specialized process of nursing staff in China. Method 1. Literature review method, semi-structured expert interview method were used to preliminarily draw up the questionnaire, carry out preliminary experiment, modify and perfect the questionnaire, form the formal questionnaire, and adopt Delphi method to deal with nursing management and nursing education. Two rounds of questionnaires were conducted by senior experts in the field of intensive care and intensive medicine, and experts were asked to evaluate the reasonableness and importance of the training index system. Excel 2010 and SPSS21.0 statistical software were used to organize and analyze. The average, standard deviation and percentage are used to express the basic situation of the expert, the positive coefficient of the expert is expressed by the effective recovery rate of the questionnaire, and the degree of authority of the expert is expressed by the academic level of the expert and the degree of familiarity and judgment of the expert to the index. The importance of each index and the concentration degree of expert opinion are expressed by the mean value of importance assignment (`X), the acceptance rate (CJ), the full score frequency (KJ), the coefficient of variation (CV) and the weight value of the index.
