【摘要】：目的探討不同的鼻飼營養(yǎng)途徑對出血壞死性胰腺炎患者疾病發(fā)展?fàn)顩r的影響。方法選擇162例連續(xù)禁食、胃腸減壓48h以上,且腸蠕動恢復(fù)、腹痛已緩解的出血壞死性胰腺炎患者按入院時間先后分成兩組,對照組行鼻胃管鼻飼(70例),觀察組鼻空腸管鼻飼(92例)。記錄兩組患者的并發(fā)癥發(fā)生情況、淀粉酶恢復(fù)正常時間、白蛋白恢復(fù)正常時間、C反應(yīng)蛋白恢復(fù)正常時間以及住院時間。結(jié)果觀察組患者在出現(xiàn)感染、消化道出血、腹瀉、腹腔膿腫、多臟器功能不全及死亡例數(shù)均顯著少于對照組,差異有統(tǒng)計學(xué)意義(P0.05)。觀察組患者的淀粉酶恢復(fù)正常時間、白蛋白恢復(fù)正常時間、C反應(yīng)蛋白恢復(fù)正常時間、住院時間均顯著短于對照組,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論對出血壞死性胰腺炎患者護(hù)理中使用螺旋空腸管進(jìn)行腸內(nèi)營養(yǎng)優(yōu)勢明顯,并發(fā)癥顯著減少,恢復(fù)時間短,護(hù)理難度低,值得臨床推廣。
[Abstract]:Objective to investigate the effect of nasal feeding in different ways to the development status of patients with hemorrhage necrotizing pancreatitis. Methods 162 cases of continuous fasting, gastrointestinal decompression of more than 48h, and the recovery of intestinal peristalsis, abdominal pain relief has been bleeding in patients with necrotizing pancreatitis according to the admission time were divided into two groups, the control group underwent nasal feeding (70 cases). The nasal jejunal tube feeding in observation group (92 cases). Recorded in two groups of patients with complications, amylase recovery time, recovery time of albumin, C reaction protein recovery time and hospitalization time. Results in the observation group were infection, gastrointestinal bleeding, diarrhea, abdominal abscess, multiple organ dysfunction and the number of deaths were significantly less than the control group, the difference was statistically significant (P0.05). The observation group of patients with amylase recovery time, recovery time of albumin, C reaction protein recovery time to live Hospital time was significantly shorter than that in the control group, the difference was statistically significant (P0.05). Conclusion the use of spiral jejunal tube in nursing of patients with hemorrhagic necrotizing pancreatitis has obvious advantages of enteral nutrition, with significant reduction in complications, short recovery time and low nursing difficulty, which is worthy of clinical promotion.
