【摘要】：近40年來(lái)的改革開(kāi)放使得我國(guó)經(jīng)濟(jì)、社會(huì)、民生等各個(gè)領(lǐng)域都取得了長(zhǎng)足進(jìn)步,成效顯著,醫(yī)療保險(xiǎn)制度也在日趨完善,全民醫(yī)保也基本得到實(shí)現(xiàn)。然而,在追求經(jīng)濟(jì)快速發(fā)展的同時(shí),農(nóng)村居民福利水平并沒(méi)有與城鎮(zhèn)居民同時(shí)得到增長(zhǎng)提升,反而使城鄉(xiāng)居民之間的醫(yī)療保險(xiǎn)水平差距在不斷拉大。因此,當(dāng)前要將重點(diǎn)放在健全和完善農(nóng)村醫(yī)療保險(xiǎn)制度和推進(jìn)城鄉(xiāng)基本醫(yī)療保險(xiǎn)制度一體化發(fā)展上,徹底改變這一差異性的醫(yī)療保險(xiǎn)制度現(xiàn)狀。統(tǒng)籌城鄉(xiāng)醫(yī)療保險(xiǎn)一體化的過(guò)程關(guān)系到國(guó)計(jì)民生,是我國(guó)國(guó)民經(jīng)濟(jì)平穩(wěn)發(fā)展和社會(huì)穩(wěn)定的重要保障。因此需要從理論和實(shí)踐的雙重角度來(lái)重點(diǎn)分析城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)制度一體化的構(gòu)建問(wèn)題。本文選取恩施市為研究對(duì)象,對(duì)其城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)制度進(jìn)行比較分析后,指出城鎮(zhèn)居民醫(yī)療保險(xiǎn)制度與新型農(nóng)村合作醫(yī)療保險(xiǎn)并軌運(yùn)行存在的弊端,歸納出整合城鄉(xiāng)醫(yī)療保險(xiǎn)制度的試點(diǎn)地區(qū)所取得的成功經(jīng)驗(yàn),并結(jié)合恩施市自身發(fā)展實(shí)際,總結(jié)出以其為代表的國(guó)內(nèi)欠發(fā)達(dá)地區(qū)構(gòu)建城鄉(xiāng)醫(yī)療保險(xiǎn)一體化的對(duì)策與建議。文章可分為五個(gè)部分,第一部分介紹了本文的研究對(duì)象和研究方法,并具體闡述國(guó)內(nèi)外學(xué)者針對(duì)城鄉(xiāng)醫(yī)療保險(xiǎn)相關(guān)問(wèn)題的研究現(xiàn)狀并進(jìn)行簡(jiǎn)要述評(píng)。第二部分詳細(xì)介紹了城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)一體化構(gòu)建的相關(guān)理論基礎(chǔ)。第三部分分析了恩施市城鄉(xiāng)居民醫(yī)療保險(xiǎn)現(xiàn)狀,并對(duì)恩施市城鄉(xiāng)兩種醫(yī)療保險(xiǎn)制度所引發(fā)的問(wèn)題進(jìn)行重點(diǎn)介紹。第四部分提出了恩施市實(shí)現(xiàn)城鄉(xiāng)醫(yī)療保險(xiǎn)一體化的可行因素,重點(diǎn)介紹國(guó)內(nèi)兩大試點(diǎn)地區(qū)重慶和鄂州的醫(yī)療保險(xiǎn)模式,歸納出寶貴經(jīng)驗(yàn)供恩施市借鑒。第五部分提出了恩施市實(shí)現(xiàn)城鄉(xiāng)醫(yī)療保險(xiǎn)一體化的障礙因素。第六部分從上述分析中歸納總結(jié)出了恩施市在構(gòu)建城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)制度一體化問(wèn)題上的對(duì)策和建議。
[Abstract]:The reform and opening up in the past 40 years has made great progress in various fields such as economy, society, people's livelihood and so on. The medical insurance system is becoming more and more perfect, and the universal medical insurance system has been basically realized. However, in the pursuit of rapid economic development, the welfare level of rural residents has not been increased at the same time as urban residents, on the contrary, the gap between the level of medical insurance between urban and rural residents is widening. Therefore, the emphasis should be placed on perfecting and perfecting the rural medical insurance system and promoting the integration of urban and rural basic medical insurance system, so as to completely change the status quo of this differential medical insurance system. The process of the integration of urban and rural medical insurance is related to the national economy and people's livelihood, and is an important guarantee for the steady development of our national economy and social stability. Therefore, it is necessary to analyze the integration of urban and rural residents' basic medical insurance system from the perspective of theory and practice. This paper selects Enshi city as the research object, compares and analyzes the basic medical insurance system of urban and rural residents, and points out the drawbacks of the parallel operation of the urban residents medical insurance system and the new rural cooperative medical insurance. This paper sums up the successful experience of integrating the urban and rural medical insurance system in the pilot areas, and combined with Enshi's own development practice, summarizes the countermeasures and suggestions for constructing the integration of urban and rural medical insurance in the underdeveloped areas represented by the urban and rural medical insurance system. The article can be divided into five parts. The first part introduces the research object and research methods of this paper, and describes the current situation of the domestic and foreign scholars on urban and rural medical insurance related issues and a brief review. The second part introduces the basic medical insurance integration of urban and rural residents in detail. The third part analyzes the current situation of medical insurance for urban and rural residents in Enshi, and focuses on the problems caused by the two medical insurance systems in Enshi. The fourth part puts forward the feasible factors to realize the integration of urban and rural medical insurance in Enshi city, focuses on introducing the medical insurance modes of Chongqing and Ezhou in two pilot areas of China, and sums up the valuable experience for Enshi city to draw lessons from. The fifth part puts forward the obstacle factors to realize the integration of urban and rural medical insurance in Enshi city. The sixth part summarizes the countermeasures and suggestions of Enshi City in constructing the integration of urban and rural residents' basic medical insurance system from the above analysis.
