新常態(tài)下山西省新醫(yī)改財(cái)政投入績效研究

【摘要】:提高政府財(cái)政投入新醫(yī)改績效,是維護(hù)醫(yī)療衛(wèi)生事業(yè)健康有效運(yùn)行的關(guān)鍵。十八大提出要實(shí)現(xiàn)基本公共服務(wù)的均等化,十八屆三中和四中全會(huì)又分別提出繼續(xù)深化醫(yī)療衛(wèi)生體制改革和推動(dòng)醫(yī)療衛(wèi)生工作的法治化。要解決居民“看病貴、看病難”難題,關(guān)鍵在于政府如何提高財(cái)政醫(yī)療衛(wèi)生投入績效。從2009年新醫(yī)改推進(jìn)以來,各級(jí)地方政府擔(dān)負(fù)主要的財(cái)政保障職責(zé),因此從省級(jí)政府角度研究新醫(yī)改財(cái)政投入績效,不僅在理論方面有其指導(dǎo)意義,更在于對(duì)進(jìn)一步深化新醫(yī)改也有實(shí)質(zhì)性現(xiàn)實(shí)意義。本文首先梳理了新醫(yī)改相關(guān)政策及其價(jià)值導(dǎo)向和實(shí)現(xiàn)目標(biāo),并從公共產(chǎn)品、市場失靈、政府干預(yù)等角度分析了政府主導(dǎo)醫(yī)療衛(wèi)生投入的理論依據(jù)。文章接下來分析,不論是山西全省還是各地級(jí)市經(jīng)濟(jì)發(fā)展都由超高速迅速滑落為低速增長,甚至出現(xiàn)負(fù)增長,山西經(jīng)濟(jì)步入了調(diào)整經(jīng)濟(jì)發(fā)展方式的新常態(tài)。經(jīng)濟(jì)發(fā)展的放緩直接影響到公共財(cái)政收入的增長,而公共財(cái)政收入萎縮伴隨的卻是公共財(cái)政支出剛性增長的矛盾性困境。2015年山西省GDP增長為3.10%,公共財(cái)政收入增長為-9.77%和新醫(yī)改財(cái)政投入增長為-19.09%,既是明顯的印證。因此轉(zhuǎn)變山西省新醫(yī)改財(cái)政投入結(jié)構(gòu),由原先注重“量”的增長到注重“質(zhì)”的改進(jìn),是山西省新醫(yī)改財(cái)政投入績效提高的關(guān)鍵所在。本文運(yùn)用DEA方法對(duì)山西省新醫(yī)改財(cái)政投入績效作實(shí)證對(duì)比分析,推導(dǎo)出不論是從全省時(shí)間序列縱向分析,還是從省內(nèi)地區(qū)間或省際間橫向?qū)Ρ确治?影響山西省新醫(yī)改財(cái)政投入綜合效率的因素,主要是財(cái)政資源在醫(yī)療衛(wèi)生領(lǐng)域配置效率低下,即純技術(shù)效率是影響綜合績效的關(guān)鍵因素的結(jié)論。因此,提出了改進(jìn)山西省新醫(yī)改財(cái)政投入績效的落腳點(diǎn)應(yīng)為改善財(cái)政資源在醫(yī)療衛(wèi)生領(lǐng)域的配置結(jié)構(gòu),并據(jù)此結(jié)論,論文從保障新醫(yī)改財(cái)政投入適度增長、優(yōu)化新醫(yī)改財(cái)政投入配置效率以及提高新醫(yī)改財(cái)政投入的生產(chǎn)效率三個(gè)方面提出優(yōu)化建議。
[Abstract]:Improving the performance of the new medical reform is the key to maintain the healthy and effective operation of the medical and health care. The 18th National Congress proposed to realize the equalization of basic public services, and the third and fourth Plenary sessions of the 18th CPC Central Committee respectively proposed to continue to deepen the reform of the medical and health system and to promote the rule of law in medical and health work. In order to solve the problem of "expensive and difficult to see a doctor", the key lies in how the government improves the performance of financial health care investment. Since the promotion of the new medical reform in 2009, local governments at all levels have assumed the main financial support responsibilities. Therefore, it is not only of guiding significance in theory to study the financial investment performance of the new health care reform from the perspective of provincial governments, It is also of practical significance to further deepen the new health care reform. This paper first combs the new medical reform related policies and the value orientation and the realization goal, and analyzes the government leading medical and health investment theory basis from the public goods, the market failure, the government intervention and so on. The next analysis shows that the economic development of Shanxi Province or all prefectural cities has declined from super high speed to low speed and even negative growth. Shanxi economy has entered the new normal of adjusting the mode of economic development. The slowdown in economic development has a direct impact on the growth of public revenue. In 2015, the GDP growth of Shanxi Province was 3.10%, the public finance revenue growth was -9.77%, and the financial input of the new medical reform was -19.09%, which was the obvious proof. Therefore, the key to improve the financial investment performance of Shanxi new medical reform is to change the financial input structure of new medical reform in Shanxi province from the growth of "quantity" to the improvement of "quality". This paper uses DEA method to make empirical comparative analysis on the financial investment performance of Shanxi Province's new medical reform, and deduces the vertical analysis of the province's time series, as well as the horizontal comparative analysis between regions within and between provinces. The main factors affecting the comprehensive efficiency of financial input in Shanxi Province are the low efficiency of allocation of financial resources in the field of medical and health care, that is, the conclusion that pure technical efficiency is the key factor affecting the comprehensive performance. Therefore, the paper puts forward that the aim of improving the financial investment performance of new medical reform in Shanxi Province should be to improve the allocation structure of financial resources in the field of medical and health care, and based on the conclusion, the paper tries to ensure the moderate increase of financial investment in the new medical reform. To optimize the allocation efficiency of new medical reform financial input and to improve the production efficiency of new medical reform financial input, the paper puts forward three optimization suggestions.
