內(nèi)蒙古醫(yī)療衛(wèi)生財(cái)政支出效率研究

【摘要】:醫(yī)療衛(wèi)生服務(wù)是公共服務(wù)的一個(gè)重要組成部分,也是關(guān)系民生的一個(gè)重大問題,政府醫(yī)療衛(wèi)生支出是醫(yī)療衛(wèi)生事業(yè)公共性的體現(xiàn)。國(guó)家“十三五”規(guī)劃綱要中提出要建立健全基本醫(yī)療衛(wèi)生制度,使每個(gè)人都能公平享有基本的醫(yī)療衛(wèi)生服務(wù),將基本公共衛(wèi)生服務(wù)作為公共產(chǎn)品向全社會(huì)的成員提供。為了貫徹實(shí)施國(guó)家政策,在提供公共醫(yī)療衛(wèi)生服務(wù)、不斷深化醫(yī)療衛(wèi)生體制改革的過程中,中央和地方各級(jí)政府的醫(yī)療衛(wèi)生財(cái)政支出持續(xù)增長(zhǎng)。但是,擴(kuò)大醫(yī)療衛(wèi)生財(cái)政支出的規(guī)模并不一定能夠?qū)崿F(xiàn)醫(yī)療衛(wèi)生財(cái)政支出效率的提升。如何提高醫(yī)療衛(wèi)生財(cái)政支出效率,是解決不斷增長(zhǎng)的醫(yī)療衛(wèi)生支出需求和有限的財(cái)政收入矛盾的關(guān)鍵所在。內(nèi)蒙古地處我國(guó)西部地區(qū),經(jīng)濟(jì)發(fā)展較落后,財(cái)政收入來源有限,限制其醫(yī)療衛(wèi)生財(cái)政支出水平,醫(yī)療衛(wèi)生資源需求與財(cái)政投入不相匹配,“看病難、看病貴”等現(xiàn)象仍然存在。因此,研究?jī)?nèi)蒙古醫(yī)療衛(wèi)生財(cái)政支出效率,對(duì)加強(qiáng)內(nèi)蒙古醫(yī)療衛(wèi)生領(lǐng)域財(cái)政資金的管理和提高醫(yī)療衛(wèi)生服務(wù)水平具有重要意義。本文以提高內(nèi)蒙古醫(yī)療衛(wèi)生財(cái)政支出效率為出發(fā)點(diǎn),以公共產(chǎn)品理論為基礎(chǔ),從投入、產(chǎn)出和效果等方面構(gòu)建效率指標(biāo)體系,運(yùn)用對(duì)比分析方法,對(duì)內(nèi)蒙古醫(yī)療衛(wèi)生財(cái)政支出進(jìn)行綜合分析,并結(jié)合數(shù)據(jù)包絡(luò)分析法測(cè)算了內(nèi)蒙古各盟市的醫(yī)療衛(wèi)生財(cái)政支出的綜合技術(shù)效率、純技術(shù)效率和規(guī)模效率。研究發(fā)現(xiàn),近年來內(nèi)蒙古醫(yī)療衛(wèi)生財(cái)政支出效率從微觀角度來看,投入和產(chǎn)出狀態(tài)較好,但各盟市的支出效率差異較大,醫(yī)療衛(wèi)生支出管理有待加強(qiáng)。對(duì)此,本文立足于內(nèi)蒙古的實(shí)際情況,提出合理調(diào)整投入規(guī)模、優(yōu)化產(chǎn)出配置的規(guī)劃能力,加強(qiáng)對(duì)醫(yī)療衛(wèi)生財(cái)政資金的監(jiān)管,進(jìn)一步提高內(nèi)蒙古的醫(yī)療衛(wèi)生財(cái)政支出效率。
[Abstract]:Medical and health services are an important component of public services, and also a major issue related to the people's livelihood. Government medical and health expenditure is the embodiment of the public nature of medical and health undertakings. In the outline of the National 13th Five-Year Plan, it is proposed to establish and improve the basic medical and health care system so that everyone can enjoy basic medical and health services equitably. To provide basic public health services as public goods to members of the whole society. In order to implement national policies, in the process of providing public health services and deepening the reform of the medical and health system, However, expanding the scale of medical and health expenditure does not necessarily improve the efficiency of medical and health expenditure. How to improve the efficiency of medical and health financial expenditure, It is the key to solve the contradiction between the increasing demand for medical and health expenditure and the limited fiscal revenue. Inner Mongolia is located in the western part of China, where the economic development is relatively backward, the source of financial revenue is limited, and the level of medical and health financial expenditure is restricted. The need for medical and health resources does not match with the financial input, and the phenomenon of "difficult to see a doctor, expensive to see a doctor" still exists. Therefore, the study on the efficiency of medical and health financial expenditure in Inner Mongolia, It is of great significance to strengthen the management of medical and health financial funds in Inner Mongolia and to improve the level of medical and health services. This paper takes improving the efficiency of medical and health expenditure in Inner Mongolia as the starting point, taking the theory of public goods as the basis, and taking the investment as the basis. The efficiency index system is constructed in terms of output and effect, and the comprehensive analysis of medical and health expenditure in Inner Mongolia is carried out by using the method of comparative analysis. Combined with data envelopment analysis, the comprehensive technical efficiency, pure technical efficiency and scale efficiency of medical and health financial expenditure in Inner Mongolia are calculated. The state of input and output is good, but the expenditure efficiency of each league city is quite different, and the management of medical and health expenditure needs to be strengthened. In view of this, based on the actual situation of Inner Mongolia, this paper puts forward the planning ability of adjusting the scale of input reasonably and optimizing the allocation of output. Strengthen the supervision of medical and health financial funds and further improve the efficiency of medical and health expenditure in Inner Mongolia.
