【摘要】：目的:探討?zhàn)B心通脈膏以“益氣和血、調(diào)補(bǔ)陰陽(yáng)”法治療胸痹心痛(穩(wěn)定型心絞痛)的臨床療效,并分析其臨床應(yīng)用的作用機(jī)制。方法:病例均來(lái)源于2015年1月-2016年1月期間遼寧中醫(yī)藥大學(xué)附屬第二醫(yī)院心一科門(mén)診患者,通過(guò)對(duì)44例符合納入標(biāo)準(zhǔn)的胸痹心痛(穩(wěn)定型心絞痛)患者進(jìn)行前瞻性觀察,對(duì)其流行病學(xué)資料、用藥前后的中醫(yī)癥狀積分的變化、西雅圖心絞痛量表等進(jìn)行統(tǒng)計(jì)分析,來(lái)評(píng)估養(yǎng)心通脈膏以“益氣和血、調(diào)補(bǔ)陰陽(yáng)”法治療胸痹心痛(穩(wěn)定型心絞痛)的臨床療效,并探討胸痹心痛(穩(wěn)定型心絞痛)的發(fā)病特點(diǎn)及機(jī)制。統(tǒng)計(jì)方法應(yīng)用SPSS19.0統(tǒng)計(jì)軟件,計(jì)數(shù)資料采用X2檢驗(yàn),計(jì)量資料采用T檢驗(yàn),等級(jí)資料采用秩和檢驗(yàn)。結(jié)果:1養(yǎng)心通脈膏服用4周后,44例胸痹心痛(穩(wěn)定型心絞痛)患者的中醫(yī)癥狀均有所改善,其中胸悶、痛,心悸氣短,倦怠乏力,畏寒肢冷,自汗及心煩、不寐多夢(mèng)等癥狀改善較明顯,差異有統(tǒng)計(jì)學(xué)意義(P0.05);而腰膝酸軟、頭暈耳鳴諸癥治療后較治療前比,雖有改善,但差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。2服用養(yǎng)心通脈膏4周后,44例胸痹心痛(穩(wěn)定型心絞痛)患者的SAQ量表評(píng)分顯著高于治療前,軀體活動(dòng)受限程度、心絞痛穩(wěn)定狀態(tài)、心絞痛發(fā)作情況、治療滿(mǎn)意程度、疾病認(rèn)知程度等各個(gè)維度的積分用藥后較用藥前比均有增加,且P0.05或0.01,差異具有統(tǒng)計(jì)學(xué)意義。結(jié)論:依托中醫(yī)“治未病”的基礎(chǔ)理論,以“衡”為期,自擬具有“益氣和血、調(diào)補(bǔ)陰陽(yáng)”之功效的養(yǎng)心通脈膏,可以改善胸痹心痛(穩(wěn)定型心絞痛)患者的中醫(yī)癥狀,提高胸痹心痛(穩(wěn)定型心絞痛)患者的生活質(zhì)量,從而改善胸痹心痛(穩(wěn)定型心絞痛)患者的預(yù)后。
[Abstract]:Objective: to explore the clinical curative effect of Yangxin Tongmai ointment on chest arthralgia (stable angina pectoris) treated by "benefiting Qi and Blood, regulating Yin and Yang", and analyzing the mechanism of its clinical application. Methods: from January 2015 to January 2016, 44 patients with chest arthralgia (stable angina pectoris) were prospectively observed in the outpatient department of the second affiliated Hospital of Liaoning University of traditional Chinese Medicine. The epidemiological data, changes of TCM symptom score before and after medication, Seattle angina scale and so on were statistically analyzed to evaluate Yangxin Tongmai paste to "benefit qi and blood," The clinical effect of tonifying yin and yang on chest arthralgia (stable angina pectoris) and the pathogenesis of chest arthralgia (stable angina pectoris) were discussed. SPSS19.0 software was used for statistics, X2 test was used for counting data, T test was used for measuring data, and rank sum test was used for grade data. Results: 1After taking Yangxin Tongmai ointment for 4 weeks, the TCM symptoms of 44 patients with chest arthralgia (stable angina pectoris) were improved, including chest tightness, pain, palpitation short breath, burnout and fatigue, fear of cold limbs, self-sweating and annoyance. Insomnia and dreams and other symptoms were significantly improved, the difference was statistically significant (P0.05); But the waist and knee soft, dizziness and earsong symptoms after treatment than before treatment, although improved, but there was no significant difference (P0.05) .2After 4 weeks of Yangxin Tongmai ointment, there was no significant difference (P0.05). The scores of SAQ scale in 44 patients with chest arthralgia (stable angina pectoris) were significantly higher than those before treatment, the degree of physical activity restriction, the stable state of angina pectoris, the attack of angina pectoris, and the satisfactory degree of treatment. The scores of each dimension, such as cognitive degree of disease, were significantly higher than those before treatment, and the difference was statistically significant (P0.05 or 0.01). Conclusion: relying on the basic theory of "treating no disease" in traditional Chinese medicine and taking "Heng" for a period of time, the Yangxin Tongmai paste, which has the effect of "invigorating qi and blood, regulating yin and yang", can improve the TCM symptoms of patients with chest arthralgia (stable angina pectoris). To improve the quality of life of patients with chest arthralgia (stable angina pectoris) so as to improve the prognosis of patients with chest arthralgia (stable angina pectoris).
