【摘要】：吳銀根教授認為肺系頑疾特點是病情反復,不易控制,往往已經(jīng)多種治療,正虛邪實夾雜,亦虛亦實;邪實不宜單純套用西醫(yī)抗感染而大肆用苦寒清熱、清化痰熱方藥,避免"閉門留寇"和損傷氣陽,致病情加重,變證叢生;肺系頑疾與氣陽虛弱、宗氣不足、抗御外邪能力低下、免疫調(diào)節(jié)能力下降密切關(guān)聯(lián),應(yīng)"扶正以祛邪"和"祛邪以安正"靈活運用,補虛瀉實貫穿始終,至于孰主孰次,依病情、病變階段不同而異。臨證注重溫法,將《金匱要略》"病痰飲者,當以溫藥和之"具體化與系統(tǒng)化,包括溫肺化飲、溫肺益氣、溫脾健運、溫腎行水化痰,溫化痰濁等,代表方劑分別為苓甘五味姜辛湯、補肺湯、苓桂術(shù)甘湯合六君子湯、痰飲丸、二陳湯合三子養(yǎng)親湯;吳老特別強調(diào)"肺為氣之主,腎為氣之根",指出肺系頑疾往往腎虛,腎具"水火"屬性,有陰虛陽虛,注重補腎,喜用仙靈脾、巴戟天溫腎補氣,桑椹、女貞子滋腎填精。附泛細支氣管炎、特發(fā)性肺間質(zhì)纖維化、左肺上葉鱗癌術(shù)后、慢性咳嗽、支氣管哮喘、慢性低熱驗案。
[Abstract]:Professor Wu Yingen believes that lung disease is characterized by repeated disease, not easy to control, often has many kinds of treatment, the mixture of positive and evil, but also vacuity and solid; It is unsuitable to simply apply western medicine to resist infection and use bitter cold to clear away heat and eliminate phlegm and heat so as to avoid "leaving the aggressors behind closed doors" and damaging Qi Yang, thus aggravating the illness and changing the syndrome. The persistent disease of lung system is closely related to the weakness of Qi Yang, deficiency of qi, low ability of resisting external evil and decreasing ability of immune regulation. It should be used flexibly to "strengthen and dispel evil" and "remove evil to an Zheng". The stage of disease varies according to the state of the disease. Clinical syndromes pay attention to the method of warming, "the synopsis of the Golden Chamber" disease phlegm drink, when warming medicine and its "specific and systematic, including warming the lung to drink, warming the lung to replenish qi, warming the spleen to transport, warm the kidney to water phlegm, warm phlegm turbid, etc. The representative prescriptions are Linggan Wuwei Jiangxin decoction, Bufei decoction, Lingguizhugan decoction and Liujunzi decoction, phlegm and drink pills, Erchen decoction and Sanzi Yangfu decoction; Wu Lao especially stressed that "the lung is the master of Qi and the kidney is the root of Qi", pointing out that the lung system is often characterized by kidney deficiency, kidney with "water and fire" property, yin deficiency and yang deficiency, attention to tonifying the kidney, predilection for the use of Xianling spleen, Morinda officinalis warming the kidney and invigorating qi, mulberry fruit and Ligustrum lucidum to nourish the kidney. With panbronchiolitis, idiopathic pulmonary interstitial fibrosis, left upper lobe squamous cell carcinoma, chronic cough, bronchial asthma, chronic hypothermia.
