【摘要】：目的:本研究主要目的是觀察祛濕通絡(luò)針刺法治療脾虛濕困型緊張型頭痛的臨床療效。方法:按照脾虛濕困型緊張型頭痛的診斷標準選擇60例門診的患者,應(yīng)用隨機數(shù)字表法分為觀察組和對照組,每組各30例。觀察組采用祛濕通絡(luò)針刺法,取穴為太陽、合谷、外關(guān)、百會、足三里、中脘、天樞、豐隆、陰陵泉,操作手法為常規(guī)針刺手法;對照組采用常規(guī)針刺法,取穴:太陽、頭維、百會、風池、頸夾脊、神門、足三里、太沖,操作手法為常規(guī)針刺手法�；颊呙咳找淮吾槾�,留針45分鐘,15天為一個療程,連續(xù)3個療程,每個療程間隔一天。對于本觀察的療效,采用的觀察指標為頭痛指數(shù)評分法和中醫(yī)證候評分法。通過統(tǒng)計數(shù)據(jù)的變化,觀察祛濕通絡(luò)針刺法治療脾虛濕困型緊張型頭痛的療效。結(jié)果:1.頭痛指數(shù)比較:觀察組治療前、后頭痛指數(shù)相比較,P0.01,有極顯著差異,具有統(tǒng)計學(xué)意義,說明治療組經(jīng)治療后,頭痛得到顯著緩解,治療有效;治療后,觀察組和對照組間比較,P0.05,有顯著差異,具有統(tǒng)計學(xué)意義,說明在治療后,觀察組在改善頭痛癥狀方面明顯優(yōu)于對照組。2.中醫(yī)證候評分比較:觀察組治療前、后中醫(yī)證候相比較,P0.01,有極顯著差異,具有統(tǒng)計學(xué)意義,說明觀察組經(jīng)治療后,患者全身癥狀有顯著改變;治療后,觀察組和對照組間比較,P0.01,有極顯著差異,具有統(tǒng)計學(xué)意義,說明在治療后,觀察組在緩解患者全身癥狀方面明顯優(yōu)于對照組。3.總有效率:祛濕通絡(luò)針刺法組總顯效率93.3%,常規(guī)針刺組總顯效率83.33%,祛濕通絡(luò)針刺組總有效率高于常規(guī)針刺組。結(jié)論:1.祛濕通絡(luò)針刺法能明顯減輕脾虛濕困型緊張型頭痛患者的頭痛程度、發(fā)病持續(xù)時間、發(fā)病頻率且效果優(yōu)于常規(guī)針刺組。2.祛濕通絡(luò)針刺法能明顯改善脾虛濕困型緊張型頭痛患者的全身癥狀及體征,且效果優(yōu)于常規(guī)針刺組。
[Abstract]:Objective: to observe the clinical effect of removing dampness and dredging collaterals acupuncture in the treatment of spleen asthenia dampness and tension type headache. Methods: according to the diagnostic criteria of spleen deficiency and dampness trapped tension type headache, 60 outpatients were selected and randomly divided into observation group and control group with 30 cases in each group. The observation group used the acupuncture method of removing dampness and unblocking collaterals, taking points as Sun, Hegu, Waiguan, Baihui, Zusanli, Zhongwan, Tianshu, Fenglong, Yin Lingquan, the operation method was routine acupuncture; The control group was treated with routine acupuncture. The acupoints were collected: sun, head Wei, Baihui, Fengchi, neck jiaji, Shenmen, Zusanli, Taichung. The patients were treated with acupuncture once a day for 45 minutes, 15 days as a course of treatment, 3 consecutive courses of treatment with one day interval. For the curative effect of this observation, the observation indexes are headache index scoring method and TCM syndrome scoring method. Through the change of statistical data, we observed the curative effect of the acupuncture method of removing dampness and dredging collaterals in the treatment of spleen deficiency and dampness trap type headache. The result is 1: 1. Comparison of headache index: before and after treatment, the headache index of the observation group was significantly different (P0.01), which showed that the headache of the treatment group was significantly relieved after treatment, and the treatment was effective. After treatment, there was significant difference between the observation group and the control group (P 0.05), which showed that the observation group was better than the control group in improving headache symptoms after treatment. Comparison of TCM syndromes score: before and after treatment in the observation group, P0.01, there was a significant difference, with statistical significance, indicating that the observation group after treatment, patients with systemic symptoms have a significant change; After treatment, there was a significant difference between the observation group and the control group (P 0.01), which showed that the observation group was better than the control group in relieving the systemic symptoms of the patients after treatment. The total effective rate: the total effective rate of the acupuncture group was 93.3%, the total effective rate of the routine acupuncture group was 83.33 and the total effective rate of the acupuncture group was higher than that of the routine acupuncture group. Conclusion: 1. The acupuncture method of removing dampness and dredging collaterals can obviously reduce the headache degree, duration, frequency and effect of routine acupuncture group. 2. The acupuncture method of removing dampness and dredging collaterals can obviously improve the whole body symptoms and signs of the patients with spleen deficiency and dampness trap type of tension headache, and the effect is better than that of the routine acupuncture group.
