【摘要】：心力衰竭是一組具有相似臨床表現(xiàn)的綜合征,是各種心血管疾病的終點事件。隨著社會的發(fā)展、人口老齡化的加快,冠心病、高血壓、糖尿病等發(fā)病率不斷增加,其發(fā)病率、再入院率也在不斷升高,嚴重威脅了患者的生命健康,影響了患者的生存質(zhì)量,因此引起了醫(yī)學界的廣泛關注與重視�，F(xiàn)代醫(yī)學對心力衰竭病理生理的認識,在神經(jīng)內(nèi)分泌系統(tǒng)、血流動力學,分子生物學等方面均不斷取得新的突破,但也的確存在一些弊端。祖國醫(yī)學對心力衰竭病因病機、證候表現(xiàn)的認識源遠流長,并不斷形成了自己獨特的理論體系,積累了豐富的臨床經(jīng)驗,在心力衰竭的治療上有著一定的優(yōu)勢。張艷教授從事心血管臨床、科研、教學工作30余年,尤其對心力衰竭的病因病機的認識及治療上積累了豐富的經(jīng)驗。吾師博覽古籍,對中醫(yī)學理論深刻理解的基礎上,結合自己長期的臨床經(jīng)驗提出,慢性心衰的病因病機可以用“虛、瘀、毒”來概括,“虛、瘀、毒”貫穿慢性心衰的發(fā)病、病理變化、以及并發(fā)癥全過程當中。因此在治療上根據(jù)心衰不同時期的主要矛盾及“毒”邪的主要表現(xiàn)形式,通過辨別“毒”邪的性質(zhì)而分別施以化瘀解毒,清熱解毒,利水解毒等不同解毒之法,構建了以“益氣扶正,活血化瘀,除毒防變”為主的治療原則,吾師潛心研究自擬強心通脈湯作為基礎方,隨證加減,中西醫(yī)互補,臨床上多獲良效。
[Abstract]:Heart failure is a group of syndromes with similar clinical manifestations, which is the endpoint of various cardiovascular diseases. With the development of society and the acceleration of population aging, the incidence of coronary heart disease, hypertension and diabetes is increasing. The incidence rate and readmission rate are also increasing, which seriously threatens the life and health of patients and affects the quality of life of patients. As a result, it has aroused widespread concern and attention in the medical field. In modern medicine, new breakthroughs have been made in neuroendocrine system, hemodynamics, molecular biology and so on, but there are some drawbacks in the understanding of pathophysiology of heart failure. Chinese medicine has a long history of understanding the etiology and pathogenesis of heart failure and syndromes, and has formed its own unique theoretical system, accumulated rich clinical experience, and has certain advantages in the treatment of heart failure. Professor Zhang Yan has been engaged in cardiovascular clinical, scientific research and teaching for more than 30 years. He has accumulated rich experience in the understanding and treatment of the etiology and pathogenesis of heart failure. On the basis of our profound understanding of the theory of traditional Chinese medicine and our own long-term clinical experience, my teacher reviewed the ancient books and put forward that the etiology and pathogenesis of chronic heart failure can be summed up by "deficiency, blood stasis, and poison", and that "deficiency, stasis, poison" runs through the pathogenesis of chronic heart failure. Therefore, according to the principal contradiction in different periods of heart failure and the main manifestations of "toxin" evil, different detoxification methods such as removing blood stasis, clearing away heat and detoxifying poison are applied respectively by distinguishing the nature of "toxin" evil, such as removing toxin from blood stasis, clearing away heat and detoxifying toxin, and promoting water and detoxification. Based on the principle of "invigorating Qi, promoting blood circulation, removing blood stasis, removing toxin and preventing change", my teacher has studied the prescription of QXT as a basic prescription, which is added and subtracted with syndrome, complementary of traditional Chinese and western medicine, and has more good effects in clinic.
