田玉美教授治療肝病的臨床經(jīng)驗(yàn)研究

【摘要】：目的探討田玉美教授對(duì)于肝病的認(rèn)識(shí),梳理其辨證論治的思路,總結(jié)其學(xué)術(shù)思想,為編撰完整的田老學(xué)術(shù)思想與臨床經(jīng)驗(yàn)著作奠定基礎(chǔ)。使廣大醫(yī)學(xué)工作者在治療肝病的臨床實(shí)踐中更好的學(xué)習(xí)傳承及運(yùn)用名醫(yī)經(jīng)驗(yàn)。方法利用Excel錄入收集的病案數(shù)據(jù),包括姓名、性別、主訴、癥狀、病因、病機(jī)、證型、治則治法、方藥組成等,建立名醫(yī)經(jīng)驗(yàn)數(shù)據(jù)庫,將數(shù)據(jù)庫導(dǎo)入SPSS軟件,對(duì)其進(jìn)行歸納、分析、總結(jié),從而挖掘田教授臨床診療用藥經(jīng)驗(yàn)。結(jié)果1.肝病常見癥狀有:乏力、脅痛、納呆、腹脹、口苦、小便黃、大便溏、寐差、眼干、頭昏、苔黃、苔膩、苔暗紅、舌裂紋、舌紫色、舌淡紅、舌齒痕、舌淡胖、脈弦滑或細(xì)數(shù)、口干、眼干、目黃、身黃等。2.肝病常見病機(jī):濕熱毒邪侵襲、肝郁脾虛、濕熱蘊(yùn)結(jié)、氣滯血瘀、濕熱中阻、肝胃不和、肝腎陰虛等。其中,濕熱毒邪為主因、濕熱蘊(yùn)結(jié)或阻滯中焦,氣滯血瘀為病理因素。3.肝病常見證型:肝膽濕熱證、肝郁脾虛證、熱毒內(nèi)結(jié)證、肝郁氣滯證、肝腎陰虛證、肝胃不和證、濕濁內(nèi)蘊(yùn)證、氣滯血瘀證等。4.田玉美教授治療肝病常用治則治法:清熱解毒、健脾利濕、疏肝解郁、滋養(yǎng)肝腎、活血化瘀、調(diào)肝運(yùn)脾、疏肝健脾、清熱燥濕、清熱養(yǎng)陰、清熱利濕、清肝瀉火、利膽退黃、健脾和胃、化瘀通絡(luò)等。5.田玉美教授治療肝病常用方劑:茵陳蒿湯、柴胡舒肝散、逍遙散、四逆散、一貫煎、龍膽瀉肝湯、四君子湯、滋水清肝飲、茵陳五苓散。6.田玉美教授治療肝病常用藥對(duì):柴胡、丹參、郁金;蒼術(shù)與厚樸;黃芩與郁金。結(jié)論1.濕、熱、毒、瘀、郁等為肝病主要致病因素。2.病位主要在肝脾,多累及膽、胃、腎。3.肝病病性復(fù)雜多變,常常虛實(shí)夾雜。4.田老治療肝病常以祛邪與補(bǔ)虛為原則,祛邪主在清熱解毒利濕退黃,補(bǔ)虛主在健脾補(bǔ)氣養(yǎng)陰。
[Abstract]:Objective to probe into Professor Tian Yumei's understanding of liver disease, sort out his thinking of syndrome differentiation and treatment, summarize his academic thought, and lay a foundation for compiling the complete works of Tian Lao's academic thought and clinical experience. So that the majority of medical workers in the treatment of liver disease in clinical practice better learning inheritance and use of famous medical experience. Methods the data of medical records recorded by Excel, including name, sex, main complaint, symptom, etiology, pathogenesis, syndrome type, treatment principle, prescription and drug composition, were used to establish the database of famous doctors' experiences, and the database was imported into SPSS software and summarized. Analysis, summary, thus excavating professor Tian clinical diagnosis and treatment drug experience. Result 1. Common symptoms of liver disease include: fatigue, hypochondria, anorexia, abdominal distension, bitter mouth, yellow urine, loose stool, poor sleep, dry eyes, dizziness, yellow fur, greasy moss, dark red fur, crack tongue, purple tongue, pale red tongue, tongue teeth mark, pale fat tongue, smooth veins or thin count. Dry mouth, dry eyes, yellow body, etc. Common pathogenesis of liver disease: damp-heat toxin evil invasion, liver stagnation and spleen deficiency, damp-heat accumulation knot, Qi stagnation and blood stasis, damp-heat resistance, liver and stomach disharmony, liver and kidney yin deficiency. Among them, dampness and heat toxin evil is the main cause, damp-heat accumulation knot or block middle coke, Qi stagnation and blood stasis are pathological factors. Common syndrome types of liver disease: liver dampness and heat syndrome, liver stagnation and spleen deficiency syndrome, heat toxin internal knot syndrome, liver stagnation qi stagnation syndrome, liver and kidney yin deficiency syndrome, liver and stomach disharmony syndrome, dampness and turbid internal accumulation syndrome, qi stagnation and blood stasis syndrome, etc. 4. Professor Tian Yumei's common principles for treating liver diseases are as follows: clearing heat and detoxifying toxin, invigorating spleen and dampness, soothing liver and relieving depression, nourishing liver and kidney, activating blood circulation and removing blood stasis, regulating liver and spleen, soothing liver and spleen, clearing heat and dampness, clearing heat and nourishing yin, clearing away heat and dampness, clearing liver and reducing fire, promoting gallbladder and retreating yellow. Invigorating spleen and stomach, removing blood stasis and unblocking collaterals, etc. Professor Tian Yumei treatment of liver diseases commonly used prescriptions: Yinchen Artemisia decoction, Chaihu Shugan Powder, Xiaoyao San, Sini San, consistent decoction, Longdan Xiegan decoction, Sijunzi decoction, Zishui Qinggan drink, Yinchen Wuling Powder. 6. Professor Tian Yumei treatment of liver diseases: Bupleurum, Salvia miltiorrhiza, Tujin; Atractylodes and Magnolia; Scutellaria baicalensis and Tujin. Conclusion 1. Dampness, heat, toxin, blood stasis and depression are the main pathogenic factors of liver disease. 2. The disease is mainly located in the liver and spleen, involving gallbladder, stomach, kidney. 3. Liver disease is complex and changeable, often with a mixture of deficiency and substance. 4. Tian Lao's treatment of liver disease is often based on the principle of dispelling evil and tonifying deficiency. The main dispelling evil is clearing heat and detoxifying dampness and removing yellow and tonifying deficiency and nourishing spleen and qi and nourishing yin.
