【摘要】：目的:探討和分析化瘀祛濕方治療膝骨關(guān)節(jié)炎早期(Early Knee Osteoarthritis,KOA)的臨床療效研究。方法:按照病例納入標(biāo)準(zhǔn)選擇廣西中醫(yī)藥大學(xué)第一附屬醫(yī)院骨三科門診就診的膝骨關(guān)節(jié)炎早期患者60例,隨機(jī)分為治療組30例和對(duì)照組30例,治療組:對(duì)照組=1:1。治療組予以化瘀祛濕方口服,對(duì)照組予以雙醋瑞因膠囊口服,整個(gè)療程為12周。采用國(guó)際公認(rèn)的Lequesne指數(shù)和視覺(jué)模擬評(píng)分法(VAS)進(jìn)行評(píng)估,觀察服藥前后臨床療效。結(jié)果:Lequesne指數(shù),兩組治療前后自身前后對(duì)照,治療組:2周后:P=0.9860.05,無(wú)統(tǒng)計(jì)學(xué)意義;4周后:P=0.0230.05,有統(tǒng)計(jì)學(xué)意義;12周后:P=0.0000.01,有顯著統(tǒng)計(jì)學(xué)意義。對(duì)照組:2周后:P=0.2120.05,無(wú)統(tǒng)計(jì)學(xué)意義;4周后:P=0.0130.05,有統(tǒng)計(jì)學(xué)意義;12周后:P=0.0000.01,有顯著統(tǒng)計(jì)學(xué)意義。組間比較:4周后:P=0.9240.05,無(wú)統(tǒng)計(jì)學(xué)意義;12周后:P=0.1280.05,無(wú)統(tǒng)計(jì)學(xué)意義。治療組總有效數(shù)為28例,無(wú)效2例,總有效率為93.33%;對(duì)照組總有效數(shù)為27例,無(wú)效3例,總有效率為90.00%,兩組在改善膝關(guān)節(jié)功能方面無(wú)顯著差異(P0.05)。VAS評(píng)分,兩組治療前后自身前后對(duì)照,治療組:2周后:P=0.0790.05,無(wú)統(tǒng)計(jì)學(xué)意義;4周后:P=0.0350.05,有統(tǒng)計(jì)學(xué)意義;12周后:P=0.0000.01,有顯著統(tǒng)計(jì)學(xué)意義。對(duì)照組:2周后:P=0.120.05,無(wú)統(tǒng)計(jì)學(xué)意義;4周后:P=0.0290.05,有統(tǒng)計(jì)學(xué)意義;12周后:P=0.0000.01,有顯著統(tǒng)計(jì)學(xué)意義。組間比較:4周后:P=0.4340.05,無(wú)統(tǒng)計(jì)學(xué)意義;12周后:P=0.5690.05,無(wú)統(tǒng)計(jì)學(xué)意義。治療組總有效數(shù)為27例,無(wú)效3例,總有效率為90.00%;對(duì)照組總有效數(shù)為26例,無(wú)效4例,總有效率為86.67%,兩組在改善膝關(guān)節(jié)疼痛上無(wú)顯著差異(P0.05)。結(jié)論:化瘀祛濕方治療早期膝骨關(guān)節(jié)炎所出現(xiàn)的癥狀上,可以改善患者膝關(guān)節(jié)的功能,減輕疼痛,與雙醋瑞因膠囊治療療效無(wú)顯著差異。
[Abstract]:Objective: to investigate and analyze the clinical curative effect of Huayu Qisheng prescription (Huayu Qisheng Fang) on early (Early Knee Osteoarthritis,KOA of knee osteoarthritis. Methods: according to the criteria of case inclusion, 60 patients with early knee osteoarthritis were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group was treated with Huayu Qishi prescription and the control group was treated with diacetate Ruiyin capsule for 12 weeks. The internationally recognized Lequesne index and visual analogue score (VAS) were used to evaluate the clinical efficacy before and after treatment. Results: Lequesne index before and after treatment was significantly higher in the treatment group than that in the control group. In the treatment group, there was no significant difference between the two groups after 2 weeks (P < 0.05), after 4 weeks (P < 0.05), and after 12 weeks (P = 0.0000.01). Control group: after 2 weeks: Pao 0.2120.05, no statistical significance; 4 weeks later: Pu 0.0130.05, with statistical significance; 12 weeks after: Pu 0.0000.01, there was significant statistical significance. Comparison between groups: after 4 weeks, there was no statistical significance in Pao 0.9240.05, and after 12 weeks, there was no statistical significance in PX 0.1280.05. The total effective number of 28 cases, ineffective 2 cases, total effective rate 93.33 in the treatment group, 27 cases in the control group, 3 cases in the control group and 90.00000 in the total effective rate, there was no significant difference in improving the knee joint function between the two groups (P0.05). VAS score). The total effective number was 27 cases, ineffective 3 cases, total effective rate 90.005 in the treatment group, 26 cases in the control group and 4 cases in the control group, the total effective rate was 86.677.There was no significant difference between the two groups in improving the knee joint pain (P0.05). Conclusion: Huayu Qishi prescription can improve the function of knee joint and relieve pain in the treatment of early knee osteoarthritis.
