【摘要】：目的:通過(guò)收集患者的基本信息、四診資料、CAG結(jié)果、血脂水平等相關(guān)資料,分析冠心病不穩(wěn)定型心絞痛中醫(yī)證型與CAG結(jié)果及血脂水平等因素的相關(guān)關(guān)系,為中醫(yī)防治本病提供依據(jù)。方法:收集2016年2月—2016年6月于中國(guó)醫(yī)科大學(xué)附屬盛京醫(yī)院心內(nèi)科住院病例122例,行CAG檢查前進(jìn)行中醫(yī)辨證分型,通過(guò)采取空腹前臂靜脈血漿,測(cè)定TG、TC、HDL、LDL的數(shù)值,根據(jù)冠狀動(dòng)脈病變情況計(jì)算Gensini評(píng)分,利用統(tǒng)計(jì)軟件分析中醫(yī)證型與各項(xiàng)指標(biāo)的相關(guān)關(guān)系。結(jié)果:1、120例患者中女性平均年齡大于男性平均年齡;2、各危險(xiǎn)因素的分布頻次依次為高血壓吸煙飲酒脂質(zhì)代謝異常糖尿病或糖耐量異常;3、實(shí)證組Gensini評(píng)分平均值虛證組Gensini評(píng)分平均值,心血瘀阻證患者的Gensini評(píng)分平均值明顯大于其它證型組患者的Gensini評(píng)分平均值,P0.05,有統(tǒng)計(jì)學(xué)意義;4、痰阻心脈證患者的血脂水平明顯高于其它證型的患者,P0.05,有統(tǒng)計(jì)學(xué)意義。結(jié)論:1、不穩(wěn)定型心絞痛多發(fā)于中老年人,男性患者數(shù)量多于女性患者數(shù)量,且隨著年齡的增長(zhǎng),女性患者數(shù)量逐漸增加;2、不穩(wěn)定型心絞痛發(fā)病的危險(xiǎn)因素從高到低依次為,高血壓、吸煙、飲酒、脂質(zhì)代謝異常、糖尿病或糖耐量異常;3、不穩(wěn)定型心絞痛實(shí)證組患者的冠脈病變程度較虛證組患者的冠脈病變程度重,心血瘀阻證患者的冠脈病變程度明顯大于其他五組患者;4、不穩(wěn)定型心絞痛患者中痰阻心脈證患者的血脂水平最高。
[Abstract]:Objective: to analyze the correlation between TCM syndrome type of unstable angina pectoris of coronary heart disease (CHD), CAG result and blood lipid level by collecting the basic information of the patients, the data of four diagnoses and the results of CAG, the level of blood lipid and so on. To provide the basis for TCM prevention and treatment of this disease. Methods: from February 2016 to June 2016, 122 cases of cardiology hospitalized in Shengjing Hospital affiliated to China Medical University were collected. Before CAG examination, TCM syndrome differentiation was carried out, and fasting forearm venous plasma was taken to determine the value of TG,TC,HDL,LDL. The Gensini score was calculated according to coronary artery disease, and the correlation between TCM syndrome types and various indexes was analyzed by statistical software. Results the average age of female was larger than that of male, and the distribution frequency of each risk factor was hypertension, smoking, drinking, abnormal lipid metabolism, diabetes mellitus or abnormal glucose tolerance. The average Gensini score of positive group was the average of Gensini score in deficiency syndrome group. The average value of Gensini score in patients with heart blood stasis syndrome was significantly higher than that in other syndrome groups (P 0.05, P 0.05). The blood lipid level of patients with phlegm blocking heart and pulse syndrome was significantly higher than that of patients with other syndrome types (P 0.05). Conclusion the number of male patients with unstable angina pectoris is more than that of female patients, and the number of female patients increases gradually with the increase of age. The risk factors of unstable angina pectoris are from high to low. Hypertension, smoking, drinking, abnormal lipid metabolism, diabetes mellitus or impaired glucose tolerance. The severity of coronary artery disease in patients with unstable angina pectoris was higher than that in patients with deficiency syndrome. The degree of coronary artery lesion in the patients with heart blood stasis syndrome was significantly higher than that in the other five groups, and the blood lipid level was the highest in the patients with unstable angina pectoris.
