【摘要】：目的:采用隨機(jī)對(duì)照臨床試驗(yàn)方法,給予自擬痛風(fēng)方治療濕熱蘊(yùn)結(jié)型急性痛風(fēng)性關(guān)節(jié)炎,觀察患者治療前、后各指標(biāo),并對(duì)實(shí)驗(yàn)結(jié)果進(jìn)行分析。方法:本次實(shí)驗(yàn)患者采集于遼寧中醫(yī)藥大學(xué)附屬二院骨科門(mén)診及病房,共50例,依據(jù)急性痛風(fēng)性關(guān)節(jié)炎診斷標(biāo)準(zhǔn)確診,按照隨機(jī)對(duì)照原則,將病人分為治療組和對(duì)照組,每組25例,兩組均飲食攝入干預(yù),治療組即結(jié)合組,給予自擬痛風(fēng)方配合非布司他片治療,對(duì)照組即西藥組,給予口服非布司他片治療,治療一個(gè)療程后,將VAS評(píng)分、癥狀體征和血尿酸濃度作為主要觀察指標(biāo),并在療程結(jié)束后隨訪2周,然后將觀察結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)處理,評(píng)價(jià)該方治療濕熱蘊(yùn)結(jié)型痛風(fēng)性關(guān)節(jié)炎的療效。結(jié)果:1.治療后,兩組VAS評(píng)分、關(guān)節(jié)腫脹、活動(dòng)度評(píng)分和血尿酸濃度均下降。2.治療后兩組止痛效果比較,結(jié)合組止痛效果優(yōu)于西藥組。3.治療后VAS評(píng)分、關(guān)節(jié)腫脹、活動(dòng)度評(píng)分比較,結(jié)合組優(yōu)于西藥組。4.治療后兩組降血尿酸濃度比較,降血尿酸方面兩組功能相近。5.隨訪于治療一個(gè)療程后2周,結(jié)合組優(yōu)于西藥組。結(jié)論:本次實(shí)驗(yàn)根據(jù)臨床癥狀分析,結(jié)果顯示自擬痛風(fēng)方對(duì)治療濕熱蘊(yùn)結(jié)型急性痛風(fēng)性關(guān)節(jié)炎有一定的療效,該方能夠有效緩解急性期的癥狀、減輕疼痛,降低血尿酸數(shù)值,并可減少緩解期的復(fù)發(fā),實(shí)驗(yàn)表明其副作用亦較小,為治療痛風(fēng)性關(guān)節(jié)炎提供了新方法。
[Abstract]:Objective: to treat acute gouty arthritis of dampness and heat accumulation type by random controlled clinical trial, observe the indexes before and after treatment, and analyze the experimental results. Methods: 50 patients were collected from orthopedic outpatient department and ward of the second affiliated Hospital of Liaoning University of traditional Chinese Medicine. According to the diagnostic criteria of acute gouty arthritis, the patients were divided into treatment group and control group according to the principle of random control. There were 25 cases in each group, both groups were treated with dietary intervention. The treatment group, that is, the combination group, was treated with self-made gout prescription combined with non-Budesta tablet, while the control group, the western medicine group, was treated with oral Festa tablet. After one course of treatment, the VAS score was evaluated. The symptoms and signs and serum uric acid concentration were taken as the main observation indexes and followed up for 2 weeks after the end of the course of treatment. Then the observation results were statistically processed to evaluate the efficacy of the prescription in treating dampness and heat accumulation type gouty arthritis. The result is 1: 1. After treatment, the VAS score, joint swelling, activity score and uric acid concentration decreased by 0. 2. 2 in both groups. After treatment, the analgesic effect of the combined group was better than that of the western medicine group. 3. After treatment, VAS score, joint swelling, activity score comparison, combined group is better than western medicine group. 4. After treatment, the blood uric acid concentration in the two groups was similar to that in the two groups, and the function of the two groups was similar to that of the control group. Two weeks after a course of treatment, the combination group was superior to the western medicine group. Conclusion: according to the analysis of clinical symptoms, the results show that the self-made gout prescription has certain curative effect on acute gouty arthritis of damp-heat accumulation type, and it can effectively relieve the symptoms of acute stage, relieve pain and reduce the value of blood uric acid. The results showed that the side effects were small, which provided a new method for the treatment of gouty arthritis.
