【摘要】：目的：本研究擬通過探討慢性濕疹的發(fā)病原因和發(fā)病機理及導師治療慢性濕疹的臨床經(jīng)驗,歸納導師論治慢性濕疹的臨床思路,總結(jié)其滋陰除濕法論治慢性濕疹的學術(shù)思想。并進一步提煉和升華,豐富中醫(yī)辨證論治體系,為廣大患者服務。方法：1.理論研究：結(jié)合古今文獻對慢性濕疹的研究,整理、歸納周勇教授滋陰除濕法治療慢性濕疹的臨床經(jīng)驗,總結(jié)、研究其學術(shù)思想。2.臨床研究：本研究通過觀察、評價周勇教授采用滋陰除濕湯治療血虛風燥型慢性濕疹的臨床療效,研究其滋陰除濕法論治慢性濕疹的臨床經(jīng)驗。將96例血虛風燥型慢性濕疹患者隨機分為2組,治療組48例,對照組48例。治療組口服中藥滋陰除濕湯,對照組口服氯雷他定口腔崩解片(商品名：雷寧,四川新斯頓制藥有限公司生產(chǎn),10mg/片,批號：國藥準字200800527)口服,兩組療程均為2周。觀察兩組患者在治療前后的癥狀、體征、T淋巴細胞亞群CD4+、CD8+、CD4+、CDg+水平等指標的變化。同時觀察豚鼠模型治療前后耳片重量、病理變化、IL-2、IL-4水平的變化。結(jié)果：1.理論研究：周勇教授對慢性濕疹病因病機的認識有獨到的見解,治療手段有自身的特色：①主張對慢性濕疹的病因病機、治則治法的探討應借鑒古代醫(yī)家對濕瘡病病因病機的認識,②認為陰血虧虛是慢性濕疹發(fā)病的基礎,濕邪是慢性濕疹發(fā)病的條件,風邪對慢性濕疹發(fā)病有重要影響,瘀血是慢性濕疹的病理產(chǎn)物,以上因素相互膠著,相互影響。2.臨床研究：周勇教授根據(jù)自身對慢性濕疹病因病機的認識,治療手段上,采用滋陰除濕法治療,同時兼顧祛風、活血,標本兼治。頗具特色。①.有效率的比較：治療組的總有效率為89.58%；而對照組的總有效率為68.75%。通過比較,治療組的療效明顯優(yōu)于對照組。②.治療前后CD4+水平的比較：治療前兩組CD4+水平明顯低于健康體檢組CD4+水平(P0.01),治療后兩組CD4+水平明顯升高,與健康體檢組相比無明顯差異(P0.05),但治療后治療組與對照組相比,CD4+水平差異有顯著性意義(P0.05)。③.治療前后CD8+水平的比較：治療前兩組CD8+水平均高于健康體檢組CD8+水平(P0.01),治療后兩組CD8+水平降低,與健康體檢組CD8+水平相比無明顯差異(P0.05),而治療后治療組與對照組相比CD8+水平無明顯差異(P0.05)。④.治療前后CD4+/CD8+水平的比較：治療前兩組CD4+/CD8+水平均明顯低于健康體檢組(P0.01),治療后治療組CD4+/CD8+水平與健康體檢組相比無顯著性差異(P0.05),而治療后對照組CD4+/CD8+水平較治療前有所升高(P0.01),但與健康體檢組相比,仍有明顯的差異(P0.01)。⑤.治療前后豚鼠模型IL-2、IL-4水平比較：治療前后與生理鹽水組比較,中藥組IL-2明顯升高,IL-4明顯降低,有極顯著性差異(P0.01),與氯雷他定組比較,差異有顯著性(P0.05)(見表11),皮膚病理、IL-2、IL-4水平均有變化。⑥.治療前后豚鼠模型病理切片比較：模型組：棘層明顯增厚,角化過度及角化不全,真皮淺層毛細血管壁增厚,膠原纖維變粗,呈慢性濕疹病理改變。生理鹽水組：表皮角化過度,棘層肥厚,變化與模型組相似。氯雷他定組：正常少量角化過度,角化不全,棘層變薄。中藥組：表皮基本恢復正常。結(jié)論：周勇教授滋陰除濕法論治慢性濕疹的臨床經(jīng)驗和學術(shù)思想符合臨床實際,切中病機,具有良好的滋陰養(yǎng)血、祛風除濕、活血化瘀作用,療效確切,具有較高的臨床價值。其滋陰除濕法法論治慢性濕疹的臨床經(jīng)驗和學術(shù)思想,對于豐富和提高中醫(yī)辨證論治體系,豐富中醫(yī)治療手段,拓寬中醫(yī)皮膚病治療范圍,提高臨床療效,減輕患者痛苦均具有重要的理論和現(xiàn)實意義。
[Abstract]:Objective: To explore the etiology and pathogenesis of chronic eczema and the clinical experience of the tutor in the treatment of chronic eczema, summarize the clinical mentality of the tutor in the treatment of chronic eczema, and summarize the academic thought of the theory of nourishing yin and removing dampness in the treatment of chronic eczema. Methods: 1. Theoretical research: Combining ancient and modern literature on chronic eczema research, collate, summarize Professor Zhou Yong's clinical experience in the treatment of chronic eczema by Nourishing Yin and removing dampness, summarize, study its academic thinking. 2. Clinical research: This study through observation, evaluate Professor Zhou Yong's clinical treatment of chronic eczema by Nourishing Yin and removing dampness Decoction for blood deficiency and wind dryness. Ninety-six patients with chronic eczema of blood deficiency and wind dryness type were randomly divided into two groups: the treatment group (48 cases) and the control group (48 cases). The symptoms, signs, T lymphocyte subsets CD4 +, CD8 +, CD4 +, CDg + levels of the two groups were observed before and after treatment. Meanwhile, the weight of the guinea pig ear slices, pathological changes, IL-2, IL-4 levels were observed before and after treatment. Results: 1. Theoretical research: Professor Zhou Yong There are unique opinions on the etiology and pathogenesis of chronic eczema, and the treatment methods have their own characteristics: 1. Advocating the etiology and pathogenesis of chronic eczema, the discussion of treatment principles and methods should draw lessons from the ancient physicians'understanding of the etiology and pathogenesis of eczema; 2. Considering that deficiency of yin and blood is the basis of chronic eczema, eczema is the condition of chronic eczema, wind evil. Clinical research: Professor Zhou Yong, according to his own understanding of the etiology and pathogenesis of chronic eczema, uses the method of nourishing Yin and removing dampness to treat chronic eczema, and at the same time gives consideration to dispelling wind, activating blood circulation and treating both the root and the root. The total effective rate was 89.58% in the treatment group and 68.75% in the control group. By comparison, the curative effect of the treatment group was obviously better than that of the control group. There was no significant difference between the two groups (P 0.05), but there was significant difference between the treatment group and the control group in the level of CD4 + (P 0.05). The level of CD4 + / CD8 + in the treatment group was significantly lower than that in the health check-up group (P 0.01). There was no significant difference in the level of CD4 + / CD8 + between the treatment group and the control group (P 0.05). There was no significant difference in the level of CD4 + / CD8 + between the treatment group and the health check-up group (P 0.05). Compared with the normal saline group, the levels of IL-2 and IL-4 in the traditional Chinese medicine group were significantly higher and IL-4 was significantly lower (P 0.01). Compared with the loratadine group, the differences were significant (P 0.01). Significant (P 0.05) (see table 11), skin pathology, IL-2, IL-4 levels were changed. _. Before and after treatment, pathological sections of guinea pig model: model group: the spinous layer was significantly thickened, hyperkeratosis and incomplete keratosis, superficial dermal capillary wall thickened, collagen fibers thickened, showing chronic eczema pathological changes. Normal saline group: epidermal hyperkeratosis, spines. Conclusion: Professor Zhou Yong's clinical experience and academic thought in treating chronic eczema by Nourishing Yin and removing dampness accord with the clinical reality, cutting the pathogenesis, nourishing yin and nourishing blood, dispelling wind and removing dampness. Its theory of nourishing yin and eliminating dampness has important theoretical and academic value in enriching and improving the system of syndrome differentiation and treatment of TCM, enriching the means of TCM treatment, broadening the scope of TCM skin disease treatment, improving the clinical efficacy and alleviating the pain of patients. Practical significance.
