【摘要】：目的通過觀察針刀配合中藥熏蒸法對膝關(guān)節(jié)骨性關(guān)節(jié)炎導(dǎo)致的膝關(guān)節(jié)屈曲攣縮的臨床治療效果,探討針刀對KOA膝關(guān)節(jié)屈曲攣縮的膝關(guān)節(jié)屈曲角度與KOA癥狀、體征的影響,探討針刀治療KOA膝關(guān)節(jié)屈曲攣縮的作用機(jī)制及其對改善KOA癥狀、體征的影響。方法將符合納入標(biāo)準(zhǔn)的黑龍江中醫(yī)藥大學(xué)第一附屬醫(yī)院骨三科臨床60例住院患者隨機(jī)劃分為觀察組和對照組,觀察組以針刀松解配合中藥熏蒸法進(jìn)行治療,對照組采用推拿手法配合中藥熏蒸組進(jìn)行對比治療,兩周為一個治療周期。治療前后對兩組患者膝關(guān)節(jié)伸直位X線側(cè)位片屈曲角度、VAS疼痛表和Lequene-Mery膝關(guān)節(jié)骨性關(guān)節(jié)炎嚴(yán)重程度評分表進(jìn)行觀察。結(jié)果運(yùn)用針刀配合中藥熏蒸治療KOA膝關(guān)節(jié)屈曲攣縮,患者的臨床癥狀、體征具有不同的改善程度,與治療前比較存在差異,具有統(tǒng)計學(xué)意義(P0.05)；膝關(guān)節(jié)伸直位X線側(cè)位片屈曲角度改善明顯,差異顯著,具有統(tǒng)計學(xué)意義(P0.01)。結(jié)論經(jīng)針刀配合中藥熏蒸治療后患者膝關(guān)節(jié)屈曲角度、疼痛、腫脹、晨僵、行走能力明顯改善,針刀配合中藥熏蒸對改善KOA膝關(guān)節(jié)屈曲攣縮方面具有較好的針對性及較高的臨床應(yīng)用價值。
[Abstract]:Objective To observe the therapeutic effect of needle knife combined with traditional Chinese medicine fumigation on knee joint flexion contracture caused by osteoarthritis of knee joint, and to explore the effect of acupotomy on flexion angle of knee joint and KOA symptoms and signs of KOA knee joint contracture, and explore the mechanism of acupotomy for flexion contracture of KOA knee and to improve the symptoms of KOA. Methods 60 patients who were hospitalized in the First Affiliated Hospital of Heilongjiang University Of Chinese Medicine were randomly divided into the observation group and the control group. The observation group was treated with Acupotomy and traditional Chinese medicine fumigation. The control group was treated with massage and Chinese medicine fumigation group, and two weeks was one of the two groups. Treatment cycle. Before and after treatment, the lateral flexion angle of knee joint position of two groups of patients, VAS pain meter and Lequene-Mery knee osteoarthritis severity score were observed. Results the Acupotomy combined with Chinese medicine fumigation treatment of KOA knee flexion contracture, the clinical symptoms and signs of the patients had different improvement degree, and the treatment and treatment were different. There was a significant difference before treatment (P0.05); the flexion angle of the lateral X-ray position of the knee joint was obviously improved, the difference was significant (P0.01). Conclusion the knee flexion angle, pain, swelling, morning stiffness, walking ability of the patients after acupuncture combined with Chinese medicine fumigation were obviously improved, and the Acupotomy combined with Chinese medicine fumigation. Improving the flexion contracture of KOA knee joint has better pertinence and higher clinical application value.
