【摘要】：目的:通過對(duì)化瘀滌痰湯治療痛風(fēng)性關(guān)節(jié)炎慢性期(痰瘀痹阻型)的臨床研究,從而觀察并評(píng)價(jià)其臨床療效及安全性,為臨床治療痛風(fēng)性關(guān)節(jié)炎提供更有效的中藥新藥。方法:通過對(duì)痛風(fēng)性關(guān)節(jié)炎慢性期(痰瘀痹阻型)的準(zhǔn)確中西醫(yī)診斷,需嚴(yán)格篩選出符合西醫(yī)痛風(fēng)性關(guān)節(jié)炎慢性期且中醫(yī)辯證屬痰瘀痹阻型診斷標(biāo)準(zhǔn)的病例方可納入實(shí)驗(yàn)。將符合納入標(biāo)準(zhǔn)的60例患者隨機(jī)分成2組:治療組、對(duì)照組,兩組均為30例。治療組給予中藥化瘀滌痰湯口服,對(duì)照組給予西藥別嘌醇口服,療程為1個(gè)月。觀察治療前后血尿酸水平、癥狀積分、安全性等指標(biāo)的變化。將所得數(shù)據(jù)應(yīng)用統(tǒng)計(jì)學(xué)方法計(jì)算得出結(jié)論�？茖W(xué)評(píng)價(jià)化瘀滌痰湯治療痛風(fēng)性關(guān)節(jié)炎慢性期(痰瘀痹阻型)的臨床療效及安全性。結(jié)果:總有效率方面治療組為93.33%,對(duì)照組為83.33%,治療組總體療效強(qiáng)于對(duì)照組(p0.05);治療前兩組患者血尿酸水平、癥狀積分無明顯差異(P0.05),治療前后兩組患者組內(nèi)比較血尿酸水平、癥狀積分均有下降,治療后組間比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),說明治療組對(duì)血尿酸水平、癥狀積分的改善情況要優(yōu)于對(duì)照組;治療組和對(duì)照組治療前和治療后單一癥狀積分比較:兩組治療前后組內(nèi)比較單一癥狀積分均有下降,治療后組間比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05),治療組對(duì)單一癥狀改善更明顯。治療組與對(duì)照組在治療后安全性方面比較,有顯著性差異(P0.05),治療組在安全性方面要優(yōu)于對(duì)照組。結(jié)論:化瘀滌痰湯對(duì)治療痛風(fēng)性關(guān)節(jié)炎具有較好的臨床療效,較服用別嘌醇在各方面的癥狀改善都存在優(yōu)勢(shì),且臨床應(yīng)用中未見明顯不良反應(yīng)及副作用。也充分體現(xiàn)了中醫(yī)辨證用藥的優(yōu)勢(shì),此方藥值得在臨床上廣泛推廣應(yīng)用。
[Abstract]:Objective: to observe and evaluate the clinical efficacy and safety of Huayu Ditan decoction (Huayu Ditan decoction) in treating chronic stage of gouty arthritis (phlegm stasis obstruction type), and to provide a more effective new Chinese medicine for the treatment of gouty arthritis. Methods: through the accurate diagnosis of chronic stage of gouty arthritis (phlegm and stasis obstruction type), it is necessary to screen out the cases that accord with the diagnostic criteria of phlegm and stasis obstruction type in traditional Chinese medicine. 60 patients who met the inclusion criteria were randomly divided into two groups: treatment group, control group, 30 cases in both groups. The treatment group was treated with traditional Chinese medicine Huayu Ditan decoction and the control group with allopurinol for one month. The changes of serum uric acid level, symptom score and safety were observed before and after treatment. The data are calculated by statistical method. To evaluate the clinical efficacy and safety of Huayu Ditan decoction (Huayu Ditan decoction) in treating chronic stage of gouty arthritis. Results: the total effective rate was 93.33 in the treatment group and 83.33 in the control group. The overall curative effect in the treatment group was stronger than that in the control group (p0.05), and there was no significant difference in serum uric acid level and symptom score between the two groups before and after treatment (P0.05). After treatment, the difference was statistically significant (P0.05), which indicated that the improvement of the treatment group on serum uric acid level and symptom score was better than that of the control group. Treatment group and control group before and after treatment of single symptom score comparison: the two groups before and after treatment compared with the single symptom score decreased, the difference between the treatment group was statistically significant (P0.05), the treatment group on the single symptoms more obvious improvement. There was significant difference between the treatment group and the control group in the safety after treatment (P0.05), the treatment group was better than the control group in terms of safety. Conclusion: Huayu Ditan decoction has good clinical effect on treating gouty arthritis, and has advantages over allopurinol in improving symptoms, and there is no obvious adverse reaction and side effect in clinical application. It also fully reflects the advantages of traditional Chinese medicine based on syndrome differentiation, which is worth popularizing in clinic.
