【摘要】：吳鞠通(1758--1836),是清代杰出的溫病學(xué)家,生于江蘇淮陰,名瑭,字佩珩,號(hào)鞠通。因父病不起,遂購(gòu)方書,潛心醫(yī)藥。吳氏一無(wú)家傳,二無(wú)師授,伏案苦讀,勤于臨證,敢于創(chuàng)新,并結(jié)合自己多年的臨證經(jīng)驗(yàn),創(chuàng)立三焦辨證理論體系,著成《溫病條辨》一書。晚年著有《吳鞠通醫(yī)案》、《醫(yī)醫(yī)病書》,記載了豐富的臨床實(shí)踐及經(jīng)驗(yàn)總結(jié)。《溫病條辨》一書為理論指導(dǎo)臨床實(shí)踐,《吳鞠通醫(yī)案》則以實(shí)踐去印證理論,相輔相成,互為補(bǔ)充�！夺t(yī)醫(yī)病書》述其未完,除論述相關(guān)外感疾病,另外也闡述了許多內(nèi)傷雜病的證治大法,及用藥之道。本文主要圍繞吳鞠通流傳于世的三本著作進(jìn)行研究,歸納、整理了吳氏在三焦辨證理論指導(dǎo)下的用藥特色,總結(jié)在治療外感病及內(nèi)傷雜病方面的用藥經(jīng)驗(yàn),為基礎(chǔ)研究和指導(dǎo)臨床實(shí)踐提供可靠的理論依據(jù)。研究?jī)?nèi)容主要包括五大方面:1、劑量靈活,在治上焦如羽、治中焦如衡、治下焦如權(quán)的治療大法指導(dǎo)下,用藥輕則幾分,重則數(shù)斤,靈活多變。另有特殊情況,需特別對(duì)待。2、劑型多變,因丸劑、散劑、膏劑、丹劑各有千秋,吳氏因人因病施藥,重在取得療效。3、巧用引藥,藥物素來(lái)有歸經(jīng)理論,吳氏在此基礎(chǔ)上有所發(fā)揮,他尤其重視天人相應(yīng)理論,充分利用動(dòng)物藥、植物藥之所長(zhǎng),發(fā)揮引經(jīng)功效,以助藥到病所。4、重視煎服方法,根據(jù)不同的病情,不同的藥物,選擇不同的煎服方法,有利于發(fā)揮藥效,有助于疾病恢復(fù)。5、內(nèi)外合治,分為內(nèi)服外敷和針?biāo)幉⑴e兩部分,吳氏擅長(zhǎng)內(nèi)服中藥治療疾病,并常根據(jù)病情需要,確定具體治療方案,或配合藥物外敷、或施針灸治療等。采用多種療法相結(jié)合的治療方式,優(yōu)勢(shì)明顯,既能取長(zhǎng)補(bǔ)短,又可通過(guò)多種途徑達(dá)到最終的治療目的。上述方法,對(duì)前人有繼承、有發(fā)展。通過(guò)對(duì)上述內(nèi)容的整理,能夠?qū)蔷贤ㄓ盟幪厣c策略有更深的了解,有利于指導(dǎo)我們?cè)谂R床實(shí)踐中恰當(dāng)?shù)�、合理的用�?提高我們運(yùn)用溫病學(xué)理論指導(dǎo)臨床實(shí)踐水平。
[Abstract]:Wu Ju Tong (1758-1836) was an outstanding febrile scientist born in Huaiyin, Jiangsu Province. Because the father can not afford to buy books, concentrate on medicine. Wu had no family, no teacher, a hard study, diligent in clinical evidence, dare to innovate, and combined with his many years of clinical experience, the establishment of the three Jiao syndrome differentiation theory system, a book into "febrile disease discrimination." In his later years, he was the author of Wu Ju Tong's Medical case and the Book of Medicine and Diseases, which recorded a wealth of clinical practice and experience. A book on the differentiation of febrile diseases served as a theoretical guide for clinical practice, while Wu Ju Tong's medical case proved the theory by practice and complemented each other. In addition to expounding the related exogenous diseases, it also expounds the methods of syndrome and treatment of many internal injuries and miscellaneous diseases, and the ways of drug use. This article mainly focuses on Wu Jutong's three books, summarizes the characteristics of Wu's medication under the guidance of the Tri-Jiao syndrome differentiation theory, and sums up his experience in the treatment of exogenous diseases and internal injuries and miscellaneous diseases. To provide a reliable theoretical basis for basic research and clinical practice. The main contents of the research include five aspects: 1, the dosage is flexible, in the treatment of coke such as feather, in the treatment of Jiao Ruheng, under the treatment of Jiao Ruyong under the guidance of the great law, medicine is light, heavy, a few jin, flexible. There are other special cases, which require special treatment. The dosage forms are changeable, because pills, powders, plasters, and saltans have their own merits. Wu's application of medicine due to human illness is focused on obtaining curative effect .3. using skillfully the medicine, the medicine has always had the theory of returning to the classics. Wu gave play to this foundation. He attached particular importance to the corresponding theory of nature and man, fully utilized the advantages of animal medicine and plant medicine, brought into full play the effect of channelling classics, and paid attention to the method of decoction, according to different conditions, different drugs, in order to help the medicine to go to the sick place. The choice of different methods of decoction is beneficial to the exertion of drug effect, to the recovery of disease .5. it can be divided into two parts: internal and external application and acupuncture and medicine. Wu is good at taking traditional Chinese medicine to treat diseases, and often determines the specific treatment plan according to the need of illness. Or cooperate with medicine external application, or apply acupuncture and moxibustion treatment and so on. The combination of multiple therapies has obvious advantages, which can not only complement each other's weaknesses, but also achieve the ultimate therapeutic goal through various ways. The above method has inherited and developed to the predecessors. Through the arrangement of the above contents, we can have a deeper understanding of the characteristics and strategies of Wu Ju Tong's medication, which is helpful to guide us to use appropriate and reasonable drugs in clinical practice, and to improve our clinical practice level by using the theory of febrile disease.
