【摘要】：目的:在老師的指導下,收集60例腰間盤突出癥患者病例。通過組間比較,擬觀察出天池傷科手法與牽引基礎(chǔ)治療法治療腰間盤突出的臨床療效分析。在本課題結(jié)束時,總結(jié)完成關(guān)于天池傷科手法治療腰間盤突出的臨床療效觀察的碩士研究生畢業(yè)論文。通過統(tǒng)計學比較,科學分析天池傷科手法的臨床療效判定,對以后臨床治療方案多樣性選擇提供一定依據(jù)。方法:本課題采用隨機分組對照法,對60例腰間盤突出癥患者進行納入篩選后,隨機分為治療組和對照組,通過組間比較,明確臨床療效評價。在下腰痛、腿痛和麻刺感、步態(tài)、直腿抬高實驗、感覺障礙、肌力下降等級、日�；顒佑绊懙确矫孀鞒鲈u分,比較兩組治療1個月后的療效。結(jié)果:治療組與對照組基線資料比較后表明,治療組與對照組在性別、年齡、病程上不具有統(tǒng)計學意義,表明兩組在基線資料上具有可比性。通過統(tǒng)計學計算,對照組治療前后癥狀與體征總評分差異具有統(tǒng)計學意義(P0.05),對照組治療后癥狀與體征總評分顯著高于治療前;治療組治療前后癥狀與體征總評分差異同樣具有統(tǒng)計學意義(P0.05),治療組治療后癥狀與體征總評分同樣顯著高于治療前。治療組痊愈14例(46.7%),好轉(zhuǎn)15例(50%),未愈1例(3.3%),總有效率為96.7%。對照組痊愈8例(26.7%),好轉(zhuǎn)14例(46.7%),未愈8例(26.7%),總有效率為73.4%。對兩組療效進行統(tǒng)計分析顯示,兩組療效差異具有統(tǒng)計學意義(P0.05),治療組療效顯著優(yōu)于對照組。結(jié)論:天池傷科手法在治療腰間盤突出癥上療效確切具有一定的科學依據(jù),在腰間盤突出癥的治法上值得選擇和進一步推廣。
[Abstract]:Objective: to collect 60 cases of lumbar disc herniation under the guidance of teachers. The clinical effect of manipulation and traction basic therapy on lumbar disc herniation in Tianchi Trauma Department was analyzed by comparison between the two groups. At the end of this thesis, the thesis is about the observation of clinical curative effect of manual treatment of lumbar disc protrusion in Tianchi Trauma Department. Through statistical comparison, the scientific analysis of the clinical efficacy of Tianchi Trauma technique, to provide a basis for the selection of clinical treatment options in the future. Methods: 60 patients with lumbar disc herniation were randomly divided into treatment group and control group. The scores of low back pain, leg pain and tingling, gait, straight leg elevation test, sensory disturbance, muscle strength decline grade, daily activity effect were compared between the two groups after one month treatment. Results: the comparison of baseline data between the treatment group and the control group showed that there was no significant difference in sex, age and course of disease between the treatment group and the control group, indicating that the baseline data of the two groups were comparable. Through statistical calculation, the total scores of symptoms and signs in the control group were significantly different before and after treatment (P0.05), and the total score of symptoms and signs in the control group was significantly higher than that before treatment. The total scores of symptoms and signs before and after treatment in the treatment group were also significantly different (P0.05), and the total scores of symptoms and signs in the treatment group were also significantly higher than those before treatment. In the treatment group, 14 cases (46.7%) were cured, 15 cases (50%) improved, 1 case (3.3%) was not recovered, the total effective rate was 96.775%. In the control group, 8 cases were cured (26.7%), 14 cases were improved (46.7%), 8 cases were not recovered (26.7%), the total effective rate was 73.4%. Statistical analysis of the two groups showed that there was significant difference between the two groups (P0.05), the therapeutic effect of the treatment group was significantly better than that of the control group. Conclusion: the therapeutic effect of Tianchi Trauma in treating lumbar disc herniation has certain scientific basis, and it is worth choosing and popularizing in the treatment of lumbar disc herniation.
