【摘要】：目的:本研究擬通過對曹志群教授在臨床工作中用于治療消化性潰瘍的相關(guān)方劑、中藥進(jìn)行整理歸類,分析其內(nèi)在用藥規(guī)律,包括常用藥物、藥對、組方規(guī)律及用藥指征,為臨床辨證論治提供參考。方法:整理2010-2015年曹志群教授門診病歷資料,從中選擇符合本次研究納入標(biāo)準(zhǔn)病例127例,處方179首,應(yīng)用“中醫(yī)傳承輔助系統(tǒng)”,采用該軟件集成的規(guī)則分析、改進(jìn)互信息法、復(fù)雜系統(tǒng)熵聚類及無監(jiān)督的熵層次聚類等數(shù)據(jù)挖掘方法[1],對藥物之間的規(guī)律進(jìn)行相關(guān)性分析。結(jié)果:通過對179首處方進(jìn)行分析,藥物四氣五味頻次統(tǒng)計,以溫性、寒性藥和甘味、辛味、苦味最多見。使用頻次大于36次的中藥有25味,包括砂仁、黃芪、丹參、白術(shù)、清半夏、黨參、醋元胡、蒲公英、炒枳殼、白芷、白芍、烏藥、百合等。最常用的藥物種類為補虛藥,其次是理氣藥、清熱藥和活血化瘀藥�；陉P(guān)聯(lián)規(guī)則分析,頻次超過45次的藥物組合共130組�；趶�(fù)雜系統(tǒng)熵聚類方法,得出核心組合30個,基于無監(jiān)督的熵層次聚類方法,演化出新處方15個。結(jié)論:通過對曹志群教授治療消化性潰瘍的處方進(jìn)行分析,總結(jié)出以補益脾胃為主,輔以辛開苦降,調(diào)理脾胃氣機,化濕消痰,并重視活血化瘀藥的應(yīng)用,配伍清熱藥以消胃中郁熱的用藥經(jīng)驗,臨床效果顯著,對臨床治療和指導(dǎo)用藥具有相當(dāng)大的推廣價值。
[Abstract]:Objective: the purpose of this study was to sort out and classify the relevant prescriptions and traditional Chinese medicines used by Professor Cao Zhiqun in the treatment of peptic ulcer, and to analyze the internal drug use rules, including common drugs, drug pairs, prescription rules and indications of drug use. To provide reference for clinical treatment based on syndrome differentiation. Methods: from 2010 to 2015, Professor Cao Zhiqun's outpatient medical records were collected, 127 cases and 179 prescriptions were selected to meet the criteria of this study. The software was used to analyze the rules of integration and to improve the mutual information method. Data mining methods such as entropy clustering and unsupervised entropy hierarchical clustering in complex systems [1] were used to analyze the correlation between drugs. Results: according to the analysis of 179 prescriptions, the frequency of four factors and five flavors of drugs were analyzed. The most common drugs were warm, cold, sweet, pungent and bitter. More than 36 times of use of traditional Chinese medicine have 25 flavors, including Amomum, Astragalus, Salvia miltiorrhiza, Atractylodes macrocephala, Pinellia ternata, Codonopsis pilosula, vinegar Yuan Hu, dandelion, fried Fructus Aurantii, Angelica dahurica, Radix Paeoniae Alba, Radix The most commonly used drugs are tonifying and deficiency drugs, followed by Qi-regulating drugs, heat-clearing drugs and blood-activating drugs. Based on association rule analysis, there were 130 drug combinations with more than 45 times frequency. Based on the entropy clustering method of complex systems, 30 core combinations and 15 new prescriptions were obtained based on unsupervised entropy hierarchical clustering method. Conclusion: by analyzing the prescriptions of Professor Cao Zhiqun in the treatment of peptic ulcer, it is concluded that the main prescription is tonifying the spleen and stomach, supplementing the Xin Kaiyu, regulating the Qi of the spleen and stomach, removing dampness and eliminating phlegm, and paying attention to the application of promoting blood circulation and removing blood stasis. The clinical effect of the combination of antipyretic drugs for removing heat in the stomach is remarkable, and it is of great value in popularizing the clinical treatment and guiding the drug use.
