【摘要】：目的:本研究旨在觀察益腎健脾通絡(luò)法治療脾腎氣虛型慢性腎小球腎炎蛋白尿的臨床療效。材料與方法:使用電子病歷系統(tǒng),收集2014年1月份—2016年1月份,就診于遼寧中醫(yī)藥大學(xué)附屬醫(yī)院病房和門(mén)診,并被確診為慢性腎小球腎炎的患者,依據(jù)本實(shí)驗(yàn)研究所制定的診斷標(biāo)準(zhǔn)、納入標(biāo)準(zhǔn)以及排除標(biāo)準(zhǔn),收錄僅使用基礎(chǔ)治療配以中成藥黃葵膠囊的患者,共33例,作為對(duì)照組;收錄在對(duì)照組治療基礎(chǔ)上服用由參芪地黃湯加味組成的中藥復(fù)方益腎健脾通絡(luò)方的患者,共35例,作為治療組。療程為兩周,觀察患者治療前后的癥狀、體征,統(tǒng)計(jì)分析兩組患者治療前后的中醫(yī)癥候、24小時(shí)尿蛋白定量,及血尿便常規(guī)、肝功能、腎功能、心電圖等安全性指標(biāo),從而評(píng)價(jià)益腎健脾通絡(luò)法治療脾腎氣虛型慢性腎炎蛋白尿的臨床療效及安全性。結(jié)果:1.西醫(yī)臨床療效:治療組臨床控制9例,顯效13例,有效9例,無(wú)效4例,總有效率為88.57%;對(duì)照組臨床控制3例,顯效9例,有效8例,無(wú)效13例,總有效率為60.61%,治療組西醫(yī)臨床療效明顯優(yōu)于對(duì)照組(P0.05)。2.24小時(shí)尿蛋白定量:治療組患者的24h UTP較治療前有明顯改善(P0.05),且療效相比對(duì)照組更加顯著(P0.05)。3.中醫(yī)癥侯療效:治療組臨床痊愈7例,顯效9例,有效13例,無(wú)效6例,總有效率為82.86%,對(duì)照組臨床痊愈3例,顯效6例,有效12例,無(wú)效12例,總有效率為63.64%,治療組的中醫(yī)癥候療效明顯高于對(duì)照組(P0.05)。4.益腎健脾通絡(luò)法治療慢性腎炎蛋白尿脾腎氣虛證的安全性良好。結(jié)論:益腎健脾通絡(luò)法治療中醫(yī)證屬脾腎氣虛型慢性腎小球腎炎蛋白尿的效果顯著,使該病患者臨床癥狀及體征明顯好轉(zhuǎn),療效肯定,安全性良好,值得推廣。
[Abstract]:Objective: to observe the clinical effect of tonifying kidney and invigorating spleen and dredging collaterals on chronic glomerulonephritis with deficiency of spleen and kidney qi. Materials and methods: patients with chronic glomerulonephritis were collected from January 2014 to January 2016 in the wards and outpatient clinics of affiliated Hospital of Liaoning University of traditional Chinese Medicine, using the electronic medical record system. According to the diagnostic criteria, the inclusion criteria and the exclusion criteria, 33 patients with basic treatment and Chinese patent medicine Huangkui capsule were included as the control group. A total of 35 patients who were treated with Shenqi Dihuang decoction and Yishen Jianpi Tongluo prescription on the basis of the control group were included as the treatment group. The course of treatment was two weeks. The symptoms and signs of the patients before and after treatment were observed, and the symptoms of TCM before and after treatment were statistically analyzed. The quantitative analysis of urine protein in 24 hours and the safety indexes of hematuria routine, liver function, renal function, electrocardiogram, etc. Objective: to evaluate the clinical efficacy and safety of tonifying kidney and invigorating spleen and dredging collaterals in the treatment of chronic nephritis with deficiency of spleen and kidney qi. The result is 1: 1. The clinical efficacy of western medicine: in the treatment group, 9 cases were clinically controlled, 13 cases were effective, 9 cases were effective, 4 cases were ineffective, and the total effective rate was 88.57 cases, while in the control group, there were 3 cases of clinical control, 9 cases of remarkable effect, 8 cases of effective and 13 cases of failure. The total effective rate was 60.61.The clinical curative effect of western medicine in the treatment group was obviously better than that in the control group (P0.05n.2.24h): the 24h UTP of the patients in the treatment group was significantly improved than that before treatment, and the curative effect was more significant than that of the control group. In the treatment group, 7 cases were cured, 9 cases were effective, 13 cases were effective, 6 cases were ineffective, the total effective rate was 82.86 cases. In the control group, 3 cases were clinically cured, 6 cases were markedly effective, 12 cases were ineffective. The total effective rate was 63.64, and the curative effect of TCM symptom in the treatment group was significantly higher than that in the control group (P 0.05. 4). The method of tonifying kidney and invigorating spleen and dredging collaterals is safe in the treatment of chronic nephritis with deficiency of spleen and kidney qi. Conclusion: the treatment of chronic glomerulonephritis with deficiency of spleen and kidney qi and deficiency of spleen and kidney by the method of tonifying the kidney and invigorating the spleen and dredging collaterals has a remarkable effect on proteinuria. The clinical symptoms and signs of the patients with chronic glomerulonephritis can be improved obviously.
