本文選題：腧穴配伍 + 糖尿病胃輕癱　； 參考：《湖南中醫(yī)藥大學(xué)》2016年碩士論文

【摘要】：目的:通過研究不同按部選穴針刺治療糖尿病胃輕癱大鼠的療效差異,探討按部選穴是影響腧穴配伍效應(yīng)的主要因素之一。方法:將60只雄性SPF級(jí)SD大鼠,適應(yīng)性喂養(yǎng)1周后,隨機(jī)分為空白對(duì)照組、模型組、足三里+中脘組、足三里+內(nèi)關(guān)組、足三里+非經(jīng)非穴組,每組12只。除外空白組,其余48只大鼠運(yùn)用鏈脲佐菌素-普通喂養(yǎng)法建立糖尿病胃輕癱大鼠模型,于第10周末開始針刺治療,治療4周后處死,取血清、下丘腦、胃底組織。運(yùn)用ELISA檢測(cè)血清中Ghrelin含量;應(yīng)用免疫組化檢測(cè)胃底組織及下丘腦組織中Ghrelin蛋白含量;應(yīng)用Real-time PCR檢測(cè)胃底組織、下丘腦Ghrelin、GHSR mRNA的表達(dá)。結(jié)果:1.與空白組相比,模型組的食量、下丘腦及胃底ghrelin、GHSR mRNA表達(dá)明顯升高,小腸推進(jìn)率、血清中Ghrelin含量、下丘腦及胃底Ghrelin灰度值明顯降低(P0.05),表明造模成功;2.與模型組相比,足三里+中脘組、足三里+內(nèi)關(guān)組、足三里+非經(jīng)非穴組下丘腦及胃底Ghrelin、GHSR mRNA表達(dá)明顯降低,小腸推進(jìn)率、血清中Ghrelin含量、下丘腦及胃底Ghrelin灰度值表達(dá)明顯升高(P0.05),表明三個(gè)治療組均能有效治療DGP。3.與足三里+中脘組相比,足三里+內(nèi)關(guān)組、足三里+非經(jīng)非穴組飲食量增多,足三里+內(nèi)關(guān)組胃底組織及下丘腦組織的Ghrelin、GHSR mRNA表達(dá)均升高(P0.05),足三里+內(nèi)關(guān)組血清Ghrelin含量明顯降低(P0.05),表明足三里+中脘組在調(diào)節(jié)DGP大鼠Ghrelin方面優(yōu)于足三里+內(nèi)關(guān)組。結(jié)論:針刺改善糖尿病胃輕癱胃腸動(dòng)力與大鼠血清Ghrelin含量、胃底組織及下丘腦中Ghrelin、GHSR表達(dá)有關(guān),表明配伍局部腧穴優(yōu)于配伍遠(yuǎn)端腧穴,按部選穴是影響腧穴配伍效應(yīng)的主要因素之一。
[Abstract]:Objective: to study the difference of therapeutic effect of different acupoints selection in treating diabetic rats with gastroparesis, and to explore that the selection of acupoints is one of the main factors affecting the compatibility of acupoints. Methods: sixty male SPF SD rats were randomly divided into control group, model group, Zusanli Zhongwan group, Zusanli Neiguan group and Zusanli non-meridian group (12 rats in each group) after one week of adaptive feeding. Except the blank group, the other 48 rats were treated with streptozotocin-common feeding method to establish diabetic gastroparesis model. Acupuncture treatment began at the end of the 10th week. After 4 weeks of treatment, the rats were killed and serum, hypothalamus and gastric fundus were collected. The content of Ghrelin in serum was detected by ELISA, the content of Ghrelin protein in gastric fundus and hypothalamus was detected by immunohistochemistry, and the expression of GHSR mRNA in hypothalamus was detected by Real-time PCR. The result is 1: 1. Compared with the blank group, the food intake, the expression of GHSR mRNA in hypothalamus and stomach floor of the model group were significantly increased, the small intestinal propulsive rate, the content of Ghrelin in serum and the gray value of Ghrelin in hypothalamus and stomach floor were significantly decreased in the model group, indicating that the model was successful. Compared with the model group, the expression of GHSR mRNA in the hypothalamus and stomach floor of Zusanli Zhongwan group, Zusanli Neiguan group and Zusanli Feijing group was significantly decreased, the small intestinal propulsion rate and the content of Ghrelin in serum were significantly decreased. The expression of Ghrelin in hypothalamus and gastric fundus was significantly increased (P 0.05), which indicated that all the three treatment groups could effectively treat DGP.3. Compared with Zusanli Zhongwan group, Zusanli Neiguan group, Zusanli Feijing group increased diet, The expression of GHSR mRNA in gastric fundus and hypothalamus in Zusanli Neiguan group was significantly higher than that in Zusanli Neiguan group. The serum Ghrelin content in Zusanli Neiguan group was significantly lower than that in Zusanli Neiguan group, indicating that Zusanli Zhongwan group was superior to Zusanli Neiguan group in regulating Ghrelin in DGP rats. Conclusion: the improvement of gastrointestinal motility of diabetic gastroparesis by acupuncture is related to the content of Ghrelin in serum, the expression of ghrelin GHSR in gastric floor tissue and hypothalamus, which indicates that the compatibility of local acupoints is better than that of distal acupoints. Selection of acupoints by the ministry is one of the main factors affecting the compatibility of acupoints.
