【摘要】：目的:系統(tǒng)評(píng)價(jià)電針治療腦卒中后認(rèn)知障礙(post-stroke cognitive impairment,PSCI)的療效與安全性。方法:計(jì)算機(jī)檢索中國(guó)知網(wǎng)數(shù)據(jù)庫(kù)(CNKI)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)(CBM)、萬(wàn)方數(shù)據(jù)庫(kù)(WanFang)、維普全文期刊數(shù)據(jù)庫(kù)(VIP)、PubMed及The Cochrane library數(shù)據(jù)庫(kù)中有關(guān)電針治療PSCI的所有隨機(jī)對(duì)照試驗(yàn)(randomized control trials,RCTs),檢索時(shí)間為建庫(kù)至2016年10月。由2名研究者獨(dú)立地進(jìn)行文獻(xiàn)篩選與資料提取,采用Cochrane系統(tǒng)評(píng)價(jià)手冊(cè)方法對(duì)納入研究進(jìn)行質(zhì)量評(píng)價(jià),采用RevMan 5.3軟件進(jìn)行Meta分析。結(jié)果:總共納入14項(xiàng)RCTs,合計(jì)896例PSCI患者。Meta分析顯示,電針組在改善PSCI患者簡(jiǎn)易精神狀態(tài)檢查量表(MMSE)積分[MD=1.78,95%CI(0.24,3.32),P=0.02]、蒙特利爾認(rèn)知評(píng)定量表(Mo CA)積分[MD=1.92,95%CI(0.96,2.88),P0.000 1]、P 300潛伏期[MD=-11.01,95%CI(-18.91,-3.11),P=0.000 6]、P 300波幅[MD=1.56,95%CI(1.14,1.98),P0.00001]、Fugl-Meyer運(yùn)動(dòng)功能量表(FMA)積分[MD=10.74,95%CI(2.67,18.82),P=0.009]、臨床療效[RR=1.37,95%CI(0.98,1.91),P=0.06]方面優(yōu)于對(duì)照組;在日常生活能力的改善方面,電針組療效與對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義[MD=6.38,95%CI(-2.41,15.18),P=0.15]。結(jié)論:本次系統(tǒng)評(píng)價(jià)證實(shí)電針治療PSCI有效且安全性較可靠,電針可改善PSCI患者認(rèn)知功能、運(yùn)動(dòng)功能,但由于本次納入文獻(xiàn)質(zhì)量較低,仍需更多設(shè)計(jì)合理的多中心RCT研究加以驗(yàn)證。
[Abstract]:Objective: to evaluate the efficacy and safety of electroacupuncture (EA) in the treatment of post-stroke cognitive impairment (PSCI) after stroke. Methods: a computerized search for CNKI, CBM, Wan Fang, Weip full-text journal database, and all randomized controlled trials on electroacupuncture for PSCI were conducted in the database of China Web site, China Biomedical Literature Database, Wanfang Journal Database, Weip Full-Text Journal Database, and all randomized controlled trials on electroacupuncture treatment of PSCI. Trialsl RCTs, the retrieval time is until October 2016. Two researchers independently carried out literature screening and data extraction, using the Cochrane system evaluation manual method to evaluate the quality of the inclusion study, and using RevMan 5.3 software to carry out Meta analysis.
