【摘要】：目的:通過觀察益氣通陽活血方治療氣虛寒凝血瘀型糖尿病下肢血管病變(lower extremity arterial disease in diabetes patients,LEADDP)的臨床療效,探討益氣通陽、活血化瘀法的作用機(jī)制,拓展治療糖尿病下肢血管病變的思路。方法:研究選取LEADDP患者60例,隨機(jī)分為兩組,觀察組和對照組各30例。在兩組均給予糖尿病基礎(chǔ)治療、阿司匹林和辛伐他汀口服的前提下,觀察組加用益氣通陽活血方加減,療程均為8周,后進(jìn)行療效性評定。觀察治療前后證候積分、總膽固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、纖維蛋白原(FIB)和踝肱指數(shù)(ABI)的變化,對相關(guān)數(shù)據(jù)采用統(tǒng)計軟件進(jìn)行統(tǒng)計分析,評價其綜合療效。結(jié)果:1中醫(yī)證候療效比較:經(jīng)過8周治療,兩組患者證候積分明顯改善,觀察組總有效率為86.67%,對照組總有效率為70%。兩組患者治療后中醫(yī)證候組間比較P0.05,有統(tǒng)計學(xué)差異。治療后兩組患者的單個癥狀療效比較,肢體麻木、疼痛等癥狀的改善觀察組均優(yōu)于對照組,且有統(tǒng)計學(xué)意義。2實驗室指標(biāo)比較:兩組患者治療前后總膽固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白、纖維蛋白原和踝肱指數(shù)均明顯改善,與本組治療前比較,P0.05,且觀察組與對照組比較,P0.05,觀察組明顯優(yōu)于對照組。3安全性分析:治療期間,兩組患者均未出現(xiàn)不良反應(yīng)和毒副作用,安全性較好。結(jié)論:益氣通陽活血方可顯著降低糖尿病下肢血管病變患者的證候積分,能有效改善患者總膽固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白、纖維蛋白原和踝肱指數(shù)的水平,療效確切,且用藥期間無毒副作用。證明益氣通陽活血方對于臨床治療氣虛寒凝血瘀型糖尿病下肢血管病變具有較好的療效。同時,也說明從氣虛寒凝血瘀立論是治療糖尿病下肢血管病變的重要研究思路。
[Abstract]:Objective: to observe the clinical effect of Yiqi Tongyang Huoxue recipe in treating lower extremity extremity arterial disease in diabetes patients with diabetes mellitus with Qi deficiency, cold coagulation and blood stasis, and to explore the mechanism of invigorating qi and removing yang and activating blood circulation and removing blood stasis. To expand the treatment of diabetic lower extremity vascular disease. Methods: 60 patients with LEADDP were randomly divided into two groups: the observation group (n = 30) and the control group (n = 30). On the premise of taking aspirin and simvastatin orally, the two groups were given basic treatment of diabetes mellitus. The observation group was treated with Yiqi Tongyang Huoxue recipe for 8 weeks, and the curative effect was evaluated. The changes of syndromes score, TC, TG, LDL-CU, HDL-CU, FIBF and ABI were observed before and after treatment. Statistical software was used to analyze the related data and evaluate the comprehensive curative effect. Results: after 8 weeks of treatment, the syndromes integral of the two groups were obviously improved. The total effective rate of the observation group was 86.67 and the total effective rate of the control group was 70. Two groups of patients after treatment compared with traditional Chinese medicine syndrome group P0.05, there was statistical difference. Two groups of patients after treatment of single symptoms, limb numbness, pain and other symptoms of the observation group were better than the control group, and there was statistical significance .2 laboratory index comparison: the two groups of patients before and after treatment total cholesterol, triglyceride, Low density lipoprotein, high density lipoprotein, fibrinogen and ankle brachial index were all significantly improved, compared with those before treatment (P 0.05), and the observation group compared with control group (P 0.05). The observation group was superior to the control group in safety analysis. There were no adverse reactions and side effects in both groups, and the safety was good. Conclusion: Yiqi Tongyang Huoxue decoction can significantly reduce the syndrome score of patients with diabetic lower extremity vascular disease, and can effectively improve the levels of total cholesterol, triglyceride, low density lipoprotein, high density lipoprotein, fibrinogen and ankle brachial index. The curative effect is definite and there is no side effect during medication. It is proved that Yiqi Tongyang Huoxue prescription has good curative effect on lower extremity vascular disease of diabetes mellitus with deficiency of qi and cold coagulation and blood stasis. At the same time, it also shows that the theory of qi deficiency, cold coagulation and blood stasis is an important research idea in the treatment of diabetic lower extremity vascular disease.
