【摘要】：目的:觀察補(bǔ)腎寧心湯治療心腎不交型卵巢早衰的臨床療效,分析其對(duì)患者卵泡刺激素(FSH)、黃體生成素(LH)、雌二醇(E2)、子宮內(nèi)膜厚度及子宮體積的影響,探討其對(duì)性腺軸的影響及影響機(jī)制。方法:將64例符合病例選擇標(biāo)準(zhǔn)的心腎不交型卵巢早衰患者隨機(jī)分為試驗(yàn)組(口服補(bǔ)腎寧心湯)和對(duì)照組(口服坤泰膠囊)各32例。觀察兩組患者治療前后中醫(yī)證候的改變情況;治療前后兩組患者FSH、LH、E2、子宮內(nèi)膜厚度及子宮體積的變化情況。結(jié)果:1.疾病療效方面,試驗(yàn)組、對(duì)照組總有效率分別為81.25%、43.75%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2.中醫(yī)證候療效方面,試驗(yàn)組、對(duì)照組總有效率分別為87.5%、56.25%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);中醫(yī)證候積分和方面,兩組患者治療后積分和較治療前均降低,治療后試驗(yàn)組積分和低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.單項(xiàng)癥狀療效方面,試驗(yàn)組各項(xiàng)中醫(yī)癥狀改善率均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。4.月經(jīng)復(fù)潮方面,試驗(yàn)組、對(duì)照組復(fù)潮率分別為87.5%、59.38%,兩組復(fù)潮率比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。5.輔助檢查方面,試驗(yàn)組治療后FSH、LH降低,E2升高,與治療前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療后組間比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組治療后子宮內(nèi)膜增厚,子宮體積增大,與治療前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療后組間比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:口服補(bǔ)腎寧心湯治療心腎不交型卵巢早衰療效確切,能使患者復(fù)潮,中醫(yī)證候療效明顯,能夠改善卵巢功能。試驗(yàn)組與對(duì)照組比較,綜合療效、中醫(yī)證候療效及卵巢、子宮功能恢復(fù)都有明顯優(yōu)勢(shì)。
[Abstract]:Objective: to observe the clinical effect of Bushen Ningxin decoction (BNXD) in treating premature ovarian failure with heart and kidney disjoint type, and to analyze its effect on follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E _ 2), endometrial thickness and uterine volume. To investigate the effect and mechanism of gonadal axis. Methods: 64 cases of premature ovarian failure with heart and kidney disjoint type were randomly divided into two groups: experimental group (32 cases) and control group (32 cases). The changes of TCM syndromes before and after treatment and the changes of FSHLHN E2 endometrial thickness and uterine volume before and after treatment were observed. The result is 1: 1. The total effective rate of the treatment group and the control group were 81.25 and 43.75, respectively. The difference between the two groups was statistically significant (P 0.05). In terms of the curative effect of TCM syndromes, the total effective rates of the experimental group and the control group were 87.5% and 56.25, respectively, and the difference between the two groups was statistically significant (P 0.05), and the scores and aspects of TCM syndromes in the two groups were decreased after treatment and compared with those before treatment. After treatment, the score of the experimental group was lower than that of the control group, and the difference was statistically significant (P 0.05). The improvement rate of each TCM symptom in the test group was higher than that in the control group, and the difference was statistically significant. In terms of menstrual resuscitation, the rates of resuscitation in the experimental group and the control group were 87.5% and 59.38, respectively, and the difference between the two groups was statistically significant (P 0.05). As for adjuvant examination, the level of FSH LH decreased after treatment in the trial group, which was significantly higher than that before treatment (P 0.05), and there was significant difference between the two groups after treatment (P 0.05), and the endometrium thickened and the uterus volume increased after treatment in the trial group. There was significant difference between the two groups before and after treatment (P 0.05). Conclusion: Bushen Ningxin decoction is effective in treating premature ovarian failure of heart and kidney disjoint type. Compared with the control group, the experimental group has obvious advantages in comprehensive curative effect, TCM syndrome curative effect, ovary and uterine function recovery.
