【摘要】：目的:觀察獨活寄生湯配合正清風(fēng)痛寧離子導(dǎo)入法治療寒濕腰痛的臨床療效以及證候改善情況。方法:選取符合腰椎間盤突出、腰肌勞損診斷標(biāo)準(zhǔn)及中醫(yī)辨證屬“寒濕凝滯”病機(jī)的60例腰痛患者隨機(jī)分為治療組和對照組,各30例。治療組采用口服獨活寄生湯加正清風(fēng)痛寧注射液中醫(yī)定向透藥療法,對照組采用口服獨活寄生湯加生理鹽水離子導(dǎo)入。觀察兩組治療前后簡式疼痛問卷(SF-MPQ)評分、中醫(yī)療效及兩組主要癥狀、體征變化。擬用統(tǒng)計軟件SPSS22.0對所有實驗數(shù)據(jù)進(jìn)行統(tǒng)計學(xué)分析。結(jié)果:兩組治療前后簡式疼痛問卷表(SF-MPQ)評分表明,治療組在改善疼痛程度方面明顯優(yōu)于對照組,兩組治療前后分差有明顯差異(P0.05),療效觀察指標(biāo)由功能活動,直腿抬高,行走功能,下肢肌力,椎旁壓痛5部分組成,兩組治療前后該指標(biāo)均明顯下降,說明兩組藥物均能有效改善相應(yīng)癥狀。組間對比,兩組治療前后分差并無明顯差異,說明治療組未能明顯優(yōu)于對照組。中醫(yī)證候量表評分的比較中,兩組治療前后組內(nèi)對比P0.05,說明兩組藥物均能改善癥狀。兩組治療前P0.05,無明顯差異,具有可比性。兩組治療后統(tǒng)計量,P0.05,有明顯差異,治療組優(yōu)于對照組。兩組治療前后差值P0.05,有明顯差異,治療組優(yōu)于對照組。中醫(yī)證候療效方面,兩組總有效率兩組未見明顯差異(P0.05)。說明兩組療效相當(dāng)。兩組患者治療期間均未見明顯毒副作用和過敏現(xiàn)象。結(jié)論:采用正清風(fēng)痛寧中醫(yī)定向透藥療法療效較好且無明顯毒副作用,且對寒濕腰痛的伴隨癥狀有明顯的改善。中醫(yī)定向透藥療法在寒濕腰痛的治療中具有較好的臨床療效以及應(yīng)用價值,值得進(jìn)一步推廣應(yīng)用。
[Abstract]:Objective: to observe the clinical effect and syndromes improvement of Duhuzhongtang combined with Zhengqing Fengtongning ionization in the treatment of cold dampness and low back pain. Methods: 60 patients with low back pain were randomly divided into treatment group (n = 30) and control group (n = 30) according to the diagnostic criteria of lumbar disc herniation, lumbar muscle strain and TCM syndrome differentiation. The treatment group was treated with traditional Chinese medicine directional permeation therapy of Zhengqing Fengtongning injection, while the control group was treated with saline ion. The scores of SF-MPQs, the curative effect of TCM and the changes of main symptoms and signs of the two groups were observed before and after treatment. The statistical software SPSS22.0 is used to analyze all the experimental data. Results: the SF-MPQs of the two groups before and after treatment showed that the treatment group was superior to the control group in improving the pain degree, the difference between the two groups was significant (P 0.05). Walking function, lower extremity muscle strength and paravertebral tenderness were composed of 5 parts. The index decreased significantly before and after treatment in both groups, indicating that both drugs could effectively improve the corresponding symptoms. There was no significant difference between the two groups before and after treatment, indicating that the treatment group was not significantly better than the control group. In the comparison of TCM syndrome scale scores, the comparison between the two groups before and after treatment P 0.05, indicating that the two groups of drugs can improve symptoms. There was no significant difference (P 0.05) between the two groups before treatment. There was significant difference between the two groups after treatment, and the treatment group was superior to the control group. The difference between the two groups before and after treatment was significant (P 0.05), and the treatment group was superior to the control group. There was no significant difference in the total effective rate between the two groups in the treatment of TCM syndromes (P 0.05). The results showed that the two groups had the same curative effect. There were no obvious side effects and allergies during treatment in both groups. Conclusion: Zhengqing Fengtongning has good curative effect and has no obvious side effects, and it can obviously improve the accompanying symptoms of cold and dampness low back pain. The directional permeation therapy of traditional Chinese medicine has good clinical effect and application value in the treatment of cold dampness and low back pain, which is worth further popularizing.
