【摘要】：目的:研究三甲復脈湯加減對姿勢步態(tài)異常(PIGD)型帕金森病(PD)患者抑郁和睡眠障礙的影響,并從神經炎癥、氧化應激探討其作用機理。方法:采用前瞻性、隨機、安慰劑對照試驗設計,將100例入組病例按1:1比例隨機分組,兩組均予左旋多巴+舍曲林,治療組予三甲復脈湯加減,對照組予中藥安慰劑。療程均為12周。在0、4、8、12周對HAMD、PDSS、UPDRSⅢ進行評定,并檢測外周血IL-17、尿酸水平。結果:兩組基線資料具有可比性。隨著治療時間延長,兩組HAMD、UPDRSⅢ評分依次遞減,并在各時間點優(yōu)于治療前(P0.05),且治療12周后治療組均低于對照組(P0.05)。治療后8、12周,兩組PDSS評分均優(yōu)于治療前(P0.05),且治療組的評分改善大于對照組(P0.05)。兩組外周血IL-17、尿酸水平在各時間點均優(yōu)于治療前(P0.05),且在治療12周后治療組均優(yōu)于對照組(P0.05)。結論:三甲復脈湯加減可以顯著改善PIGD型PD抑郁和睡眠障礙,并能增強左旋多巴對運動癥狀的療效,其對外周血IL-17和尿酸表達的調節(jié)可能是該方作用機理之一。
[Abstract]:Aim: to study the effects of Sanjia Fumai decoction (SFMT) on depression and sleep disorder in patients with PIGD type Parkinson's disease (PD), and to explore its mechanism from neuroinflammation and oxidative stress. Methods: a prospective, randomized, placebo controlled trial was used. 100 patients in the control group were randomly divided into two groups at 1:1. Both groups were treated with levodopa sertraline, the treatment group was treated with Sanjia Fumai decoction, and the control group was treated with placebo. The course of treatment was 12 weeks. The levels of IL-17 and uric acid in peripheral blood were measured at 12 weeks after 12 weeks. Results: the baseline data of the two groups were comparable.
