【摘要】：目的:觀察水針刀聯(lián)合雪蓮注射液治療棘上韌帶炎的臨床療效,增加對(duì)于脊柱棘上韌帶炎治療的可行方法,為臨床規(guī)范化治療提供科學(xué)依據(jù)。方法:選擇2015年11月-2017年1月期間就診于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院骨科門診,符合棘上韌帶炎的診斷標(biāo)準(zhǔn)、納入標(biāo)準(zhǔn)的患者64例。將樣本隨機(jī)分配為2組,治療組和對(duì)照組各32例患者。失訪病例按無(wú)效計(jì)算。治療組采用水針刀聯(lián)合雪蓮注射液治療,對(duì)照組采用傳統(tǒng)針刀治療,兩組均治療兩個(gè)療程,并且隨訪兩個(gè)月。觀察兩組患者治療前后疼痛程度、臨床癥狀、活動(dòng)功能等的變化情況,對(duì)治療后兩組的綜合療效、腰背部疼痛緩解程度及功能改善進(jìn)行對(duì)比。結(jié)果:水針刀聯(lián)合雪蓮注射液治療組明顯優(yōu)于單純應(yīng)用針刀治療組,對(duì)照組總有效率低于治療組,并且通過(guò)治療后J0A腰背痛量化評(píng)分及Roland-Morris功能障礙評(píng)分經(jīng)統(tǒng)計(jì)學(xué)分析表明治療組在治療前后疼痛及功能的改善上均優(yōu)于對(duì)照組,并具有統(tǒng)計(jì)學(xué)意義。兩組患者治療后2個(gè)月復(fù)發(fā)情況進(jìn)行隨訪時(shí),兩組具有顯著差異,表明水針刀聯(lián)合雪蓮注射液治療棘上韌帶炎在遠(yuǎn)期療效上具有優(yōu)勢(shì)。結(jié)論:臨床研究結(jié)果表明:水針刀聯(lián)合雪蓮注射液治療棘上韌帶炎療效確切,同時(shí)創(chuàng)傷小,復(fù)發(fā)率低,療效持續(xù)時(shí)間長(zhǎng),安全性高,副作用少,易于被患者接受,治療費(fèi)用少,適用于通過(guò)手法治療、物理治療效果不佳,拒絕或無(wú)法使用激素封閉治療,以及尋求中醫(yī)中藥治療的患者,是臨床上一種值得推廣的治療方法,為擴(kuò)展腰背棘上韌帶炎治療的臨床方法提供了科學(xué)合理的依據(jù)和資料。
[Abstract]:Objective: to observe the clinical effect of water needle knife combined with Xuelian injection in the treatment of supraspinal ligamenttis, to increase the feasible method for treatment of spinal supraspinal ligament, and to provide scientific basis for clinical standardized treatment.Methods: from November 2015 to January 2017, 64 patients with supraspinal ligamenttis were included in the orthopaedic clinic of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine.Samples were randomly divided into two groups, treatment group and control group of 32 patients.The missing visit cases are calculated as invalid.The treatment group was treated with water needle knife combined with Xuelian injection, the control group was treated with traditional needle knife, the two groups were treated with two courses of treatment and followed up for two months.The changes of pain degree, clinical symptom and activity function were observed before and after treatment. The comprehensive curative effect, relief degree and function improvement of lumbar and back pain were compared between the two groups after treatment.Results: the treatment group with water needle knife combined with Xuelian injection was superior to the simple acupuncture treatment group, and the total effective rate of the control group was lower than that of the treatment group.After treatment, the quantitative score of J0A low back pain and the score of Roland-Morris dysfunction showed that the treatment group was superior to the control group in pain and function improvement before and after treatment, and had statistical significance.There was a significant difference between the two groups when the recurrence was followed up 2 months after treatment, which indicated that the combination of water needle knife and Xuelian injection had an advantage in the treatment of supraspinal ligament inflammation.Conclusion: the results of clinical study show that the treatment of supraspinal ligament inflammation with water needle knife combined with Xuelian injection is effective, with small trauma, low recurrence rate, long curative effect, high safety, less side effects, easy to be accepted by patients, and less cost of treatment.It is suitable for patients who have poor effect of physical therapy through manual therapy, who refuse or can not use hormone blocking therapy, and who seek traditional Chinese medicine treatment. It is a treatment method worth popularizing in clinic.It provides scientific and reasonable basis and data for the treatment of supraspinal ligamenttis.
