【摘要】：目的初步探索上海及周邊地區(qū)高血壓病中醫(yī)證型與心血管危險因素的相關(guān)性。方法按納入標準選擇原發(fā)性高血壓病患者213例,充分收集中醫(yī)四診信息,對其進行辨證分型,并進行統(tǒng)計分析。結(jié)果 (1)高血壓各證型分布比例從高到低依次為肝腎陰虛證,肝陽上亢證,陽氣虛衰證,氣陰兩虛證,痰濕壅盛證。(2)高血壓不同中醫(yī)證型與心血管危險因素的比較。①高血壓不同中醫(yī)證型組年齡從高到低依次為陽氣虛衰證,肝腎陰虛證,痰濕壅盛證,肝陽上亢證,氣陰兩虛證,高血壓不同中醫(yī)證型組間在病程、性別中未見明顯差異;②高血壓不同中醫(yī)證型組血同型半胱氨酸均值均高于指南界定范圍(≥10 mmol/mL),各組間比較差異無統(tǒng)計學意義;③高血壓不同中醫(yī)證型組空腹血糖水平從高到低依次為氣陰兩虛證,陽氣虛衰證,肝腎陰虛證,肝陽上亢證,痰濕壅盛證;高血壓不同中醫(yī)證型組糖化血紅蛋白水平從高到低依次為氣陰兩虛證,陽氣虛衰證,肝腎陰虛證,痰濕壅盛證,肝陽上亢證;各證型組餐后2 h血糖差異無統(tǒng)計學意義;④高血壓不同中醫(yī)證型組三酰甘油水平從高到低依次為痰濕壅盛證,氣陰兩虛證,陽氣虛衰證,肝腎陰虛證,肝陽上亢證;高血壓不同中醫(yī)證型組高密度脂蛋白水平從高到低依次為肝腎陰虛證,痰濕壅盛證,肝陽上亢證,陽氣虛衰證,氣陰兩虛證,各證型組其余血脂指標差異無統(tǒng)計學意義。結(jié)論肝腎陰虛證在高血壓患者中所占比重最高;陽氣虛衰證組患者年齡最大;各證型高血壓患者血同型半胱氨酸水平普遍較高;氣陰兩虛證組患者血糖水平最高;痰濕壅盛證患者甘油三酯最高。
