【摘要】：目的：以老年骨折術(shù)后發(fā)熱患者為研究對(duì)象,以老年骨折術(shù)后特點(diǎn)以及中醫(yī)理論為本,通過(guò)運(yùn)用中醫(yī)辨證思維分析病情,以滋陰清熱法為綱,應(yīng)用清骨散加減治療退熱,觀察治療后發(fā)熱的時(shí)間、血象與時(shí)間等指標(biāo)與對(duì)照組的不同,觀察滋陰清熱法治療老年骨折術(shù)后發(fā)熱的有效性,從而促進(jìn)患者術(shù)后康復(fù),為老年人術(shù)后骨折愈合提供更好的環(huán)境與條件。方法：本研究選取老年骨折術(shù)后發(fā)熱患者60例,按照單盲隨機(jī)的原則將其平均分配到治療組和對(duì)照組,治療組術(shù)后給予清骨散加減治療；對(duì)照組給予物理降溫。兩組每天定時(shí)測(cè)量腋溫,治療后24小時(shí)開(kāi)始觀察連續(xù)7天的體溫,觀察治療后發(fā)熱的時(shí)間、血象、住院費(fèi)用與時(shí)間等指標(biāo),記錄并整理相關(guān)數(shù)據(jù),運(yùn)用統(tǒng)計(jì)學(xué)分析并處理,最終得出結(jié)果并加以討論。成果：1、兩組方案均可治療老年骨折術(shù)后發(fā)熱,但治療組運(yùn)用清骨散加減治療退熱效果更快速,患者發(fā)熱持續(xù)時(shí)間更短。2、兩組均能改善老年骨折術(shù)后發(fā)熱患者的中醫(yī)臨床癥狀,總有效率對(duì)照組為80%,治療組為90%,經(jīng)統(tǒng)計(jì)學(xué)處理可以認(rèn)為在改善中醫(yī)臨床癥狀療效方面治療組優(yōu)于對(duì)照組。3、兩組治療均可不同程度降低患者SAS積分,說(shuō)明在改善患者焦慮方面均取得良好的效果。結(jié)論：以滋陰清熱法為原則,運(yùn)用清骨散加減能有效的治療老年骨折術(shù)后發(fā)熱,從而改善患者的臨床癥狀及焦慮狀況,為臨床上老年骨折術(shù)后的骨折愈合與康復(fù)提供了一條科學(xué)有效的方法選擇。
[Abstract]:Objective: to study the patients with fever after operation of senile fracture, take the characteristics of senile fracture and the theory of TCM as the basis, analyze the condition by using TCM dialectical thinking, and take the method of nourishing yin and clearing away heat as the outline. Using Qinggu Powder to treat fever, observe the time, blood and time of fever after treatment, and observe the effectiveness of nourishing yin and clearing heat method in treating the fever after operation of senile fracture. In order to promote postoperative rehabilitation of patients and provide a better environment and conditions for postoperative fracture healing of the elderly. Methods: 60 patients with fever after operation were selected in this study. According to the principle of single blind randomization, it was equally distributed to the treatment group and the control group, and the treatment group was treated with Qinggu Powder after operation. The control group was given physical cooling. The axillary temperature was measured regularly every day in the two groups. The body temperature was observed 24 hours after treatment for 7 days. The time of fever, blood picture, hospitalization cost and time after treatment were observed. Records and arranges the related data, uses the statistical analysis and the processing, finally obtains the result and discusses. But the treatment group treated with Qinggu powder plus or minus the effect of antipyretic faster, the duration of fever in patients with shorter. 2, both groups can improve the clinical symptoms of the elderly patients with fever after fracture, the total effective rate of 80% in the control group. The treatment group is 90. The treatment group is superior to the control group in improving the curative effect of TCM clinical symptoms. The two groups of treatment can reduce the SAS score of patients in varying degrees. Conclusion: taking the method of nourishing yin and clearing away heat as the principle, the addition and subtraction of Qinggu San can effectively treat the postoperative fever of senile fracture. It can improve the clinical symptoms and anxiety of the patients and provide a scientific and effective method for fracture healing and rehabilitation.
