【摘要】:目的:研究咖啡因?qū)ρ装Y痛針刺鎮(zhèn)痛效應(yīng)及下丘腦疼痛相關(guān)分子表達(dá)的影響。方法:以C57BL/6J小鼠為研究對(duì)象,篩選基礎(chǔ)熱痛閾值在10 s~20 s的小鼠,隨機(jī)分為空白組、模型組、模型+電針組(簡(jiǎn)稱(chēng)模針組)、模型+電針+咖啡因組(簡(jiǎn)稱(chēng)模咖針組);使用完全弗氏佐劑足底注射制備炎癥痛模型;模咖針組攝入70 mg/Kg/day咖啡因,空白組、模型組、模針組攝入飲用水;模針組、?п樈M進(jìn)行電針干預(yù),2/100Hz疏密波,0.2mA,15分鐘,1次/日,連續(xù)干預(yù)11天;采用輻射熱測(cè)痛法測(cè)定各組痛閾,治療結(jié)束后處死動(dòng)物取下丘腦,用PCR Array高通量檢測(cè)技術(shù)檢測(cè)下丘腦疼痛相關(guān)分子的表達(dá)。結(jié)果:(1)咖啡因?qū)ρ装Y痛針刺鎮(zhèn)痛效應(yīng)的影響結(jié)果:組間比較:造模后3天,與空白組比較,模型組、模針組以及?п樈M小鼠痛閾明顯下降,差異有統(tǒng)計(jì)學(xué)意義(P0.000),而模型組、模針組以及?п樈M三組間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),具有可比性。在造模后7天,模針組與模型組痛閾比較差異有統(tǒng)計(jì)學(xué)意義(P0.000);模咖針組與模型組痛閾比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);?п樈M與模針組比較差異有統(tǒng)計(jì)學(xué)意義(P0.000)。在造模后13與14天,模針組與模型組痛閾比較差異有統(tǒng)計(jì)學(xué)意義(PO.000),?п樈M與模型組痛閾比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),模咖針組與模針組比較差異有統(tǒng)計(jì)學(xué)意義(P0.001)。組內(nèi)比較:模型組在造模后7天的痛閡與造模后3天比較有所改善(P0.001);在造模13與14天的痛閾與造模后3天比較差異有統(tǒng)計(jì)學(xué)意義(P0.000);在造模13與14天的痛閾與造模后7天比較差異有統(tǒng)計(jì)學(xué)意義(PO.000);而第13和14天的痛閾差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。模針組在造模后7天的痛閾與造模后3天比較差異有統(tǒng)計(jì)學(xué)意義(P0.000);模針組在造模13與14天的痛閾與造模后7天比較異有統(tǒng)計(jì)學(xué)意義(P0.000);模針組在造模13與14天的痛閾比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。?п樈M在造模后7天的痛閾與造模后3天比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);模咖針組在造模13與14天的痛閾與造模后7天比較差異有統(tǒng)計(jì)學(xué)意義(P0.001);模咖針組在第13天與第14天痛閾比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。(2)下丘腦內(nèi)疼痛相關(guān)分子mRNA的表達(dá):與空白相比較,模型組有18個(gè)基因明顯,其中表達(dá)上調(diào)有13個(gè),下調(diào)有5個(gè);與模型組比較,模針組基因有7個(gè)基因變化明顯,其中表達(dá)下降有1個(gè),表達(dá)上調(diào)有6個(gè);與模型組比較,模咖針組有25基因變化明顯:表達(dá)上調(diào)有24個(gè),表達(dá)下調(diào)有1個(gè);與模針組比較,模咖針組有25基因變化明顯,其中表達(dá)上調(diào)有24個(gè),表達(dá)下調(diào)有1個(gè)。其中,與咖啡因影響電針鎮(zhèn)痛相關(guān)的基因是Ngf。與空白組相比,模型組中基因Ngf的表達(dá)上調(diào),上調(diào)倍數(shù)為1.9053;與模型組相比,模針組中Ngf基因的表達(dá)下調(diào),下調(diào)倍數(shù)為0.4475;與模型組相比,?п樈M中基因Ngf變化不明顯,其值為0.7253。結(jié)論:(1)電針對(duì)炎癥痛有確切的鎮(zhèn)痛效應(yīng);(2)電針期間攝入咖啡因(70 mg/Kg/day),可抑制電針的鎮(zhèn)痛效應(yīng);(3)咖啡因攝入減弱電針鎮(zhèn)痛效應(yīng)可能與下丘腦Ngf基因的表達(dá)有關(guān)。
