【摘要】：《傷寒論》由張仲景所著,是現(xiàn)存最早的理法方藥俱全的系統(tǒng)論述外感熱病的臨床醫(yī)書,其,是最早提出辨證論治的醫(yī)學(xué)著作。張仲景在繼承《黃帝內(nèi)經(jīng)》對外感熱病的認(rèn)識的基礎(chǔ)上,結(jié)合臨床實踐,極大地豐富了原有內(nèi)容,創(chuàng)立了其獨有的六經(jīng)辨證體系,六經(jīng)辨證歸納了疾病的寒熱趨向、邪正盛衰,基本上概括了臟腑與十二經(jīng)的病變,提示了外感病變化發(fā)展的規(guī)律,完善了外感熱病的辨證論治體系,在中國醫(yī)學(xué)史上具有無可比擬的重要地位,并啟發(fā)了后世溫病學(xué)三焦辨證的創(chuàng)立。目的:本課題旨在通過收集并閱讀李培生教授發(fā)表的學(xué)術(shù)論文及著作,總結(jié)其學(xué)術(shù)思想,并通過收集整理《傷寒論》中與“發(fā)熱”有關(guān)的條文及李培生教授醫(yī)學(xué)著作中對于相關(guān)條文的解析,分析李培生教授對于相關(guān)條文的理解和對于外感熱病的認(rèn)識,總結(jié)歸納其對于治療外感發(fā)熱疾病的臨床經(jīng)驗與學(xué)術(shù)思想。方法:本研究采用文獻分析、歸納的研究方法,通過閱讀文獻總結(jié)李培生教授研究《傷寒論》的學(xué)術(shù)思想,并分析《傷寒論》中包含發(fā)熱相關(guān)癥狀的條文與李培生教授對相關(guān)條文的分析,從辨證要點、病因病機、治法、方藥、類似證等方面進行總結(jié)歸納,結(jié)合其他醫(yī)家的理解,總結(jié)李培生教授對于治療外感熱病的獨特思想。結(jié)論:通過研究,本文得出如下結(jié)論:太陽病為表證,以發(fā)熱惡寒并見為特點,是外感病的初期階段,本階段正氣奮起御外,當(dāng)用汗法助正氣祛邪外出,病輕者用桂枝湯類,病重者用麻黃湯類,若有表里同病者,則隨其癥治之;本病誤下邪熱內(nèi)陷可導(dǎo)致痞證、臟結(jié)等,用陷胸湯泄熱行水,用瀉心湯除痞。陽明病為里實熱證,發(fā)熱以發(fā)熱不惡寒、汗出、熱勢高為特點,其病煎灼津液,慎不可妄用汗下;熱在氣分、彌漫內(nèi)外用白虎湯清熱,有津傷氣耗用白虎加人參湯;若邪熱與燥矢互結(jié)腑氣不通大便不下,用承氣湯類通下腑實;中焦熱與濕互結(jié)致膽汁疏泄失常而外溢,陽黃兼表用麻黃連翹赤豆湯、陽黃熱重濕輕用梔子柏皮湯、陽黃兼里證腹?jié)M用茵陳蒿湯。少陽病發(fā)熱以寒熱往來為特點,兼見口苦、咽干,目眩,是邪居于半表半里,治之以小柴胡湯和解少陽;少陽病發(fā)熱分為小柴胡湯證及少陽病變證,隨證治之。太陰病多惡寒,若有脾陽虛兼表可發(fā)熱,見惡寒重發(fā)熱輕,不可峻發(fā)汗,當(dāng)用桂枝湯緩發(fā)其汗;若脾陽虛下利較甚用表里雙解之法而以溫里為主,治之以桂枝加人參湯。少陰病發(fā)熱多為陰虛發(fā)熱,多見發(fā)熱與心煩、口渴咽干并見,用清熱育陰的黃連阿膠湯、豬芩湯等;陰虛兼太陽表實證見發(fā)熱惡寒無汗、足冷、脈反沉,用麻黃附子細(xì)辛湯或麻黃附子甘草湯。厥陰病發(fā)熱可分為寒熱錯雜與厥陰熱利,若有寒熱錯雜證,治療當(dāng)寒溫并施,方用烏梅丸清上溫下、安蛔止利;上熱下寒吐逆自利或咽喉不利、唾膿血,治以清上溫下補虛;有利下粘膩膿血,腹痛,里急后重,肛門灼熱,口渴,脈數(shù)有力者,以苦寒的白頭翁湯治之。
