【摘要】：目的:咳嗽變異性哮喘(Cough variant asthma,CVA),是哮喘的一種特殊類型,而且是一種呼吸系統(tǒng)疾病的常見(jiàn)病�？人允荂VA的主要或唯一的臨床表現(xiàn),很多人由于咳嗽持續(xù)發(fā)生或者反復(fù)發(fā)作,導(dǎo)致咳嗽遷延不愈,最終引發(fā)咳嗽變異性哮喘。近年來(lái)咳嗽變異性哮喘的發(fā)病率持續(xù)增高的趨勢(shì)。目前現(xiàn)代醫(yī)學(xué)主要以抗炎藥、支氣管擴(kuò)張藥、抗過(guò)敏藥物來(lái)應(yīng)用對(duì)癥治療,但其療效尚不理想,并且對(duì)人體的副作用較多,停藥后容易復(fù)發(fā)。西醫(yī)對(duì)于本病的治療具有一定的局限性。而中醫(yī)藥的治療根據(jù)審證論治,辨清證候,審明病因病機(jī),采取針對(duì)性的治則從根本上治愈并改善患者的整體癥狀,提高生活質(zhì)量。因此運(yùn)用中醫(yī)藥治療該病日益受到關(guān)注。導(dǎo)師楊道文教授在其二十多年的臨床工作中,對(duì)于咳嗽變異性哮喘的認(rèn)識(shí)及治療積累了豐富的經(jīng)驗(yàn)。"益氣疏風(fēng)方"為楊道文教授根據(jù)多年的臨床經(jīng)驗(yàn)所創(chuàng),方藥基本組成為黃芪、白術(shù)、蜂房、蟬蛻、僵蠶、炙麻黃、苦杏仁,本著益氣扶正以治本,疏風(fēng)祛邪以治標(biāo),標(biāo)本兼治的治療原則。本研究旨在觀察運(yùn)用益氣疏風(fēng)方加減治療辨證為氣虛風(fēng)動(dòng)證的咳嗽變異性哮喘患者,探討益氣疏風(fēng)方治療咳嗽變異性哮喘的臨床療效,以期為治療本病提供一條新的診治思路。方法:本論文第一部分通過(guò)文獻(xiàn)檢索、整理歸納的方式,總結(jié)古代醫(yī)家以及現(xiàn)代醫(yī)家對(duì)咳嗽變異性哮喘的相關(guān)研究,主要包括本病的中醫(yī)病名,病因病機(jī),不同的診治方法和臨床經(jīng)驗(yàn)四大部分。第二部分是采用自身對(duì)照的方法,對(duì)20例辨證為氣虛風(fēng)動(dòng)證的咳嗽變異性哮喘患者運(yùn)用"益氣疏風(fēng)方"進(jìn)行治療,重點(diǎn)觀察患者的咳嗽、咽癢、惡風(fēng)、自汗、神疲乏力等癥狀與體征,分別記錄于治療前、治療后評(píng)分,應(yīng)用統(tǒng)計(jì)軟件SPSS20.0對(duì)評(píng)分進(jìn)行分析,觀察并評(píng)估"益氣疏風(fēng)方"加減對(duì)咳嗽變異性哮喘的臨床療效。結(jié)果:運(yùn)用益氣疏風(fēng)方治療氣虛風(fēng)動(dòng)證的咳嗽變異性哮喘患者,經(jīng)治療前與治療后癥狀評(píng)分比較,有顯著統(tǒng)計(jì)學(xué)意義(P0.01),可以明顯改善患者的臨床癥狀。其中臨床治愈1例,顯效4例,有效13例,無(wú)效2例,治療總有效率為90%。結(jié)論:益氣疏風(fēng)方加減對(duì)改善咳嗽變異性哮喘氣虛風(fēng)動(dòng)證患者的臨床癥狀及體征療效確切,取得了顯著臨床效果,具有重要的臨床指導(dǎo)意義與使用價(jià)值。由此證實(shí)益氣疏風(fēng)法的有效性及適應(yīng)范圍,并為臨床提供了一條新的診治思路。
[Abstract]:Objective: cough variant asthma (Cough variant, asthma, CVA) is a special type of asthma, and is a common disease of respiratory system disease. The clinical manifestations of cough is mainly or only CVA, a lot of people because of persistent cough or recurrent cough, leading to delayed healing, eventually cause cough variant asthma in recent years. Cough variant asthma incidence trend. Modern medicine mainly anti-inflammatory drugs and bronchodilators, anti allergic drug application to symptomatic treatment, but the effect is not ideal, and more side effects on the human body, easy to relapse after drug withdrawal. Western medicine has limitations the treatment for the disease. The treatment of Chinese medicine according to the trial on the treatment of differentiating syndromes, etiology and pathogenesis of the Ming, take targeted treatment fundamentally cure and improve overall symptoms, improve the quality of life and therefore. The use of Chinese medicine treatment of the disease has been paid more and more attention. Professor Yang Daowen in the more than 20 years of clinical work, for the understanding and treatment of cough variant asthma has accumulated rich experience. The purpose of this study is to observe the use of Yiqi Shufeng Decoction for treating the syndrome of qi deficiency and wind srirring cough variant asthma patients, to investigate the effects of Yiqi Shufeng Decoction in the treatment of the clinical curative effect of cough variant asthma, in order to provide a new treatment strategy for the treatment of this disease.
