【摘要】：目的：評價柴苓杉貝湯聯(lián)合RCHOP方案治療氣郁痰結(jié)型NHL(非霍奇金淋巴瘤)的臨床療效及安全性。方法：用隨機(jī)數(shù)字法把符合入組標(biāo)準(zhǔn)的72例患者分為中藥加化療組(A組)和化療組(B組),各36例。兩組均接受了2周期的RCHOP方案的化療,其中A組于RCHOP方案化療開始同時口服中藥柴苓杉貝湯,150m1/次,日兩次(早晚)。觀察治療后骨髓抑制情況；外周血象(WBC, NEUT, PLT, Hb)變化情況；中醫(yī)臨床癥狀及生存質(zhì)量的變化情況；肝腎功、心電異常發(fā)生情況。結(jié)果：1療效評價：骨髓抑制發(fā)生率A組為68.57%,B組為94.29%,A組較B組低,差異有統(tǒng)計學(xué)意義(P0.05)。A組與B組骨髓抑制分度比較,差異有統(tǒng)計學(xué)意義(P0.05)。與B組相比,A組治療后血象降低幅度小,差異有統(tǒng)計學(xué)意義(P0.05)。中醫(yī)癥狀改善有效率A組為65.71%,B組為8.57%,A組高于B組,差異有統(tǒng)計學(xué)意義(P0.05)。A組與B組中醫(yī)癥狀改善分度比較,差異有統(tǒng)計學(xué)意義(P0.05)。與B組相比,A組治療后中醫(yī)癥狀總積分和單個中醫(yī)癥狀積分下降幅度均較大,差異有統(tǒng)計學(xué)意義(P0.05)。與B組相比,A組治療后KPS評分提高幅度較大,差異有統(tǒng)計學(xué)意義(P0.05)。2肝腎功和心電圖異常發(fā)生評價：腎功正常；肝功異常情況對比(P0.05),差異無統(tǒng)計學(xué)意義,心電異常情況對比(P0.05),差異無統(tǒng)計學(xué)意義。結(jié)論：1.柴苓杉貝湯可以降低化療骨髓抑制發(fā)生率和改善骨髓抑制程度。2.柴苓杉貝湯可以改善血象降低情況。3.柴苓杉貝湯可以改善中醫(yī)臨床癥狀和改善患者生存質(zhì)量。
[Abstract]:Objective: to evaluate the clinical efficacy and safety of Chailing Shanbei decoction combined with RCHOP regimen in the treatment of NHL (non-Hodgkin's lymphoma) with qi depression and phlegm. Methods: 72 patients were randomly divided into two groups: group A (n = 36) and group B (n = 36), who were treated with Chinese medicine plus chemotherapy (group A) and chemotherapy group (group B, n = 36). Two cycles of RCHOP chemotherapy were given to both groups, in which group A took Chailing Shanbei decoction orally at the beginning of RCHOP regimen, 150m1/ twice a day (sooner or later). The changes of myelosuppression, peripheral blood (WBC, NEUT, PLT, Hb), clinical symptoms and quality of life of traditional Chinese medicine, liver and kidney function and abnormal ECG were observed after treatment. Results: 1 Evaluation of curative effect: the incidence of myelosuppression was 68.57% in group A and 94.29% in group B, which was significantly lower in group A than in group B (P0.05:). A group and B group). The difference was statistically significant (P0.05). Compared with group B, the hemogram of group A decreased slightly after treatment, the difference was statistically significant (P0.05). The effective rate of TCM symptom improvement in group A was 65.71%, group B was 8.57%, group A was higher than group B, the difference was statistically significant (P0.05). Compared with group B, the total score of TCM symptom and the score of single symptom decreased significantly in group A after treatment, the difference was statistically significant (P0.05). (2) Evaluation of abnormal liver and kidney function and ECG: normal renal function; Liver function abnormality situation contrast (P0.05), the difference is not statistically significant, the ECG abnormal situation contrast (P0.05), the difference is not statistically significant. Conclusion: 1. Chailing Shanbei decoction can reduce the incidence of chemotherapy myelosuppression and improve the degree of myelosuppression. 2. Chailing shanbei decoction can improve the decrease of hemogram. 3. Chailing Shanbei decoction can improve the clinical symptoms of traditional Chinese medicine and improve the quality of life of patients.
