【摘要】：甲狀腺乳頭狀癌頸淋巴結(jié)轉(zhuǎn)移較常見,規(guī)范的頸淋巴結(jié)清掃術(shù)有利于降低局部復(fù)發(fā)率及再手術(shù)率并改善病人預(yù)后。然而由于頸部解剖結(jié)構(gòu)復(fù)雜,如果術(shù)前評估不仔細(xì),清掃范圍不規(guī)范,手術(shù)操作不熟練,某些部位的轉(zhuǎn)移淋巴結(jié)容易在手術(shù)中被遺漏而導(dǎo)致殘留。其中,位置較低的中央?yún)^(qū)淋巴結(jié)、右側(cè)喉返神經(jīng)深面淋巴結(jié)、Ⅱb區(qū)淋巴結(jié)、位置深在的Ⅳ區(qū)淋巴結(jié)、Ⅴ區(qū)淋巴結(jié)、頸叢神經(jīng)根間淋巴結(jié)、胸鎖乳突肌-胸骨舌骨肌間淋巴結(jié)和咽旁淋巴結(jié)是行頸淋巴結(jié)清掃術(shù)時容易被遺漏的轉(zhuǎn)移部位,而頸部增強(qiáng)CT掃描有利于術(shù)前轉(zhuǎn)移淋巴結(jié)的評估。熟悉容易遺漏的轉(zhuǎn)移淋巴結(jié)部位、準(zhǔn)確的術(shù)前評估、規(guī)范的清掃范圍和嫻熟的手術(shù)技巧是減少頸淋巴結(jié)清掃術(shù)轉(zhuǎn)移淋巴結(jié)殘留的關(guān)鍵。
