異黏蛋白(MTDH)在轉(zhuǎn)移性乳腺癌中的表達及臨床意義

【摘要】:目的:通過分析異黏蛋白在乳腺癌原發(fā)灶、轉(zhuǎn)移灶中表達的情況,及其與乳腺癌生物學(xué)特征、生存時間的關(guān)系,推測異黏蛋白與乳腺癌的預(yù)后的關(guān)系。方法:1、選取2008年-2014年間于我院治療的可獲得病理切片的轉(zhuǎn)移性乳腺癌患者共74例。在74例轉(zhuǎn)移性乳腺癌患者中,有26例既有原發(fā)灶,又有轉(zhuǎn)移灶。其余的48例中24例只有原發(fā)灶,24例只有轉(zhuǎn)移灶。最終獲得轉(zhuǎn)移性乳腺癌原發(fā)灶組織50例、轉(zhuǎn)移灶組織50例和未發(fā)生轉(zhuǎn)移患者的原發(fā)灶組織50例,對其行MTDH免疫組化,對MTDH的表達情況進行統(tǒng)計學(xué)分析。2、隨機選出50名未發(fā)生轉(zhuǎn)移的乳腺癌患者,對其原發(fā)灶行免疫組化,與上述原發(fā)灶MTDH的表達情況進行統(tǒng)計學(xué)分析。3、統(tǒng)計患者相關(guān)資料,如年齡、月經(jīng)情況、激素水平、分子分型、腫塊大小、淋巴結(jié)轉(zhuǎn)移數(shù)目、無病生存期、總生存期,分析MTDH的表達與上述資料是否相關(guān)。推測MTDH在乳腺癌預(yù)后判斷中的作用和意義。結(jié)果:在50例原發(fā)灶及轉(zhuǎn)移灶中,MTDH在原發(fā)灶高表達的為37例,低表達為13例,在轉(zhuǎn)移灶中高表達的為46例,低表達為4例(P=0.031)。在26名患者原發(fā)灶及轉(zhuǎn)移灶中MTDH均高表達的為18例(69.3%),原發(fā)灶低表達轉(zhuǎn)移灶高表達為5例(19.2%),原發(fā)灶高表達轉(zhuǎn)移灶低表達為3例(11.5%),均低表達的為0例(0.0%)。在50例轉(zhuǎn)移性乳腺癌和非轉(zhuǎn)移性乳腺癌中,MTDH高表達的分別為24例、37例,低表達的分別為26例、13例(P0.05)。MTDH的表達情況與PR表達(P=0.029)有關(guān)。MTDH的表達影響預(yù)后,對于總生存期無影響。結(jié)論:1、在乳腺癌原發(fā)灶中,MTDH高表達,預(yù)示容易發(fā)生轉(zhuǎn)移;2、PR表達呈陰性的乳腺癌患者,MTDH更傾向于高表達;3、MTDH高表達與轉(zhuǎn)移部位無關(guān);4、在乳腺癌轉(zhuǎn)移灶中,MTDH高表達,它可能成為未來治療的新靶點;5、MTDH是乳腺癌DFS的獨立預(yù)后因素,但對于總生存期無影響。
[Abstract]:Objective: to analyze the expression of heteromucin in primary and metastatic lesions of breast cancer, and its relationship with biological characteristics and survival time of breast cancer, and to speculate the relationship between heteromycin and prognosis of breast cancer. Methods: 1. 74 patients with metastatic breast cancer treated in our hospital from 2008 to 2014 were selected. Of 74 patients with metastatic breast cancer, 26 had both primary and metastatic lesions. Of the remaining 48 cases, only primary lesions were found in 24 cases and metastatic lesions in 24 cases. Finally, 50 cases of primary tissues of metastatic breast cancer, 50 cases of metastatic tissues and 50 cases of non-metastatic breast cancer tissues were obtained. MTDH immunohistochemical staining was performed for them. The expression of MTDH was analyzed statistically. 2. 50 patients with breast cancer without metastasis were randomly selected, and their primary lesions were treated with immunohistochemistry. The expression of MTDH in the above primary tumor was analyzed statistically. 3. The relevant data of patients, such as age, were analyzed. Menstruation, hormone level, molecular classification, tumor size, lymph node metastasis, disease-free survival, total survival. The role and significance of MTDH in prognosis of breast cancer were speculated. Results: in 50 cases of primary tumor and metastasis, 37 cases were high expression of MTDH, 13 cases were low expression, 46 cases were high expression and 4 cases were low expression in primary tumor (P0. 031). There were 18 cases (69.3%) with high expression of MTDH, 5 cases (19.2%) with high expression of MTDH, 3 cases (11.5%) with low expression of MTDH and 0 cases (0.00%) with low expression of MTDH. In 50 cases of metastatic breast cancer and non-metastatic breast cancer, the high expression of MTDH was found in 24 cases, 37 cases and the low expression in 26 cases, respectively. The expression of). MTDH was related to the expression of PR (P0. 029) in 13 cases (P0.05). The expression of MTDH affected the prognosis, but had no effect on the overall survival time. Conclusion: 1, in primary breast cancer, the high expression of MTDH indicates that it is easy to metastasize; 2in breast cancer patients with negative PR expression, MTDH is more likely to be overexpression; 3MTDH high expression is not related to the location of metastasis; 4, in breast cancer metastasis, MTDH is highly expressed. MTDH is an independent prognostic factor for breast cancer DFS, but has no effect on overall survival.
