【摘要】：目的:將《排便習(xí)慣改變脾氣虛證癥狀分級(jí)量化表》用作參照,對(duì)益氣升降湯方治療大腸癌術(shù)后患者化療期間出現(xiàn)排便習(xí)慣改變的效應(yīng)作出評(píng)價(jià),同時(shí)觀察、監(jiān)測(cè)并記錄患者治療的細(xì)胞免疫功能、KPS評(píng)分、腫瘤標(biāo)記物水平、腫瘤復(fù)發(fā)轉(zhuǎn)移情況及化療時(shí)發(fā)生的毒副反應(yīng),并進(jìn)行前后對(duì)比和分析,由此為益氣升降湯治療大腸癌術(shù)后排便異常在臨床上推行及廣泛應(yīng)用,為研發(fā)新的抗癌中藥制劑提供理論依據(jù)與新的契機(jī)。方法:針對(duì)收集的已確診大腸癌術(shù)后患者(60例)展開隨機(jī)對(duì)照臨床試驗(yàn)研究,按亂數(shù)表法將其分為治療組和對(duì)照組(各30例)。其中接受益氣升降湯方聯(lián)合FOLFOX4方案化療者為治療組,單純接受FOLFOX4方案化療者為對(duì)照組,14天為1周期,連續(xù)觀察4周期。把《排便習(xí)慣改變脾氣虛證癥狀分級(jí)量化表》作為益氣升降湯所產(chǎn)生治療效應(yīng)的主要評(píng)定標(biāo)準(zhǔn),并觀察比較常用的療效性及安全性檢測(cè)指標(biāo)在治療前后的變化。結(jié)果:使用益氣升降湯中藥聯(lián)合FOLFOX方案化療的患者在大便次數(shù)、頻繁便意感/肛門下墜/不盡/脹感方面的前后評(píng)分差異有高度統(tǒng)計(jì)學(xué)意義(P0.01),在糞便性狀,排便時(shí)間、排便困難/過度用力排便、腸鳴亢進(jìn)的方面的前后評(píng)分差異有統(tǒng)計(jì)學(xué)意義(P0.05);該組經(jīng)治療后在糞便性狀、排便困難/過度用力排便、頻繁便意感/肛門下墜/不盡/脹感、體倦乏力、惡心嘔吐方面與單用FOLFOX方案化療的評(píng)分差異有統(tǒng)計(jì)學(xué)意義(P0.05);此外結(jié)果提示,經(jīng)秩和檢驗(yàn),該組治療方法對(duì)增加患者細(xì)胞免疫功能及體力狀況,降低腫瘤指標(biāo)及化療毒副作用發(fā)生率方面的優(yōu)勢(shì)強(qiáng)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:益氣升降湯方在化療期間治療大腸癌術(shù)后患者的排便習(xí)慣改變,能夠減輕臨床癥狀,調(diào)節(jié)細(xì)胞免疫功能,改善體力狀況,在一定程度上能夠降低腫瘤標(biāo)記物值,同時(shí)化療毒副作用輕,值得臨床運(yùn)用。
[Abstract]:Objective:To evaluate the effect of Yiqi Shengjiang Decoction on the change of defecation habits in patients with colorectal cancer after chemotherapy, and to monitor and record the cellular immune function, KPS score, tumor marker level, tumor recurrence and metastasis. The toxicity and side effects of chemotherapy were compared and analyzed before and after treatment, which provided a theoretical basis and a new opportunity for the clinical application of Yiqi Shengjiang Decoction in the treatment of abnormal bowel movements after colorectal cancer surgery, and for the development of New Anticancer Traditional Chinese medicine preparations. The treatment group was treated with Yiqi Shengjiang Decoction combined with FOLFOX4 regimen chemotherapy, and the control group was treated with FOLFOX4 regimen chemotherapy alone. The patients received FOLFOX4 regimen chemotherapy for a period of 14 days for four consecutive cycles of observation. In order to evaluate the therapeutic effect of Yiqi Shengjiang Decoction and observe the changes of the commonly used efficacy and safety indexes before and after treatment.Results:There were significant differences in the scores of fecal frequency, frequent fecal feeling/anal falling/unexhausted/distension between the patients treated with Yiqi Shengjiang Decoction and FOLFOX regimen chemotherapy. Highly statistically significant (P 0.01), there were significant differences in stool traits, defecation time, dyspepsia / overexertion defecation, hypersonic bowel sounds (P 0.05); the group after treatment in the stool traits, dyspepsia / overexertion defecation, frequent fecal sense / anal drop / endless / distension, fatigue, nausea and vomiting There was significant difference between FOLFOX regimen and FOLFOX regimen alone (P 0.05). In addition, the results showed that the treatment group had more advantages than the control group in increasing cellular immune function and physical status, reducing tumor indicators and the incidence of side effects of chemotherapy, and the difference was statistically significant (P 0.05). Jiangtang Decoction can alleviate clinical symptoms, regulate cellular immune function, improve physical condition, to a certain extent, reduce the value of tumor markers, while the side effects of chemotherapy are mild, which is worthy of clinical application.
