【摘要】：目的:本研究旨在探討淋巴結(jié)跳躍轉(zhuǎn)移在食管鱗癌中的發(fā)生率,確定影響食管鱗癌淋巴結(jié)跳躍轉(zhuǎn)移的危險(xiǎn)因素及評(píng)估淋巴結(jié)跳躍轉(zhuǎn)移在胸段食管鱗狀細(xì)胞癌患者中的預(yù)后價(jià)值。方法:收集了2011年1月1日到2013年12月31日之間在山東省腫瘤醫(yī)院接受食管癌完全切除術(shù)的113名淋巴結(jié)陽性的患者。結(jié)果:淋巴結(jié)跳躍轉(zhuǎn)移在全組的發(fā)生率為43.4%(49/113)。淋巴結(jié)跳躍轉(zhuǎn)移組的中位生存時(shí)間和5年生存率分別為18.0個(gè)月和3.0%,非淋巴結(jié)跳躍轉(zhuǎn)移組的中位生存時(shí)間和5年生存率分別為31.0個(gè)月和30.0%(P0.0001)。性別(P=0.027)、年齡(P=0.008)和腫瘤分化程度(P=0.026)與淋巴結(jié)跳躍轉(zhuǎn)移的發(fā)生有關(guān)。腫瘤位置(P=0.014),T分期(P=0.000)、N分期(P=0.000)和跳躍轉(zhuǎn)移(P=0.001)是食管鱗癌獨(dú)立的預(yù)后影響因素。結(jié)論:跳躍是食管鱗癌常見的淋巴結(jié)轉(zhuǎn)移方式之一,發(fā)生淋巴結(jié)跳躍轉(zhuǎn)移者其預(yù)后相對(duì)較差。
[Abstract]:Objective: to investigate the incidence of leaping lymph node metastasis in esophageal squamous cell carcinoma, to determine the risk factors of lymph node jumping metastasis and to evaluate the prognostic value of jump lymph node metastasis in thoracic esophageal squamous cell carcinoma. Methods: 113 patients underwent complete resection of esophageal carcinoma from January 1, 2011 to December 31, 2013 were collected. Results: the incidence of lymph node jump metastasis was 43.4% (49 / 113). The median survival time and 5-year survival rate were 18.0 months and 3.0 months in the lymph node jumping metastasis group, and 31.0 months and 30.0% in the non-lymph node jumping metastasis group (P 0.0001). Sex (P0. 027), age (P0. 008) and tumor differentiation (P0. 026) were associated with lymph node jumping metastasis. Tumor location (P0. 014) T stage (P0. 000) and skip metastasis (P0. 001) were independent prognostic factors of esophageal squamous cell carcinoma. Conclusion: jumping is one of the most common lymph node metastases in esophageal squamous cell carcinoma.
