【摘要】：目的探討旋復(fù)花代赭石湯聯(lián)合甲氧氯普胺和氟哌啶醇對(duì)腫瘤惡病質(zhì)惡心嘔吐的效果。方法采用前瞻性隨機(jī)對(duì)照試驗(yàn),將286例腫瘤惡病質(zhì)惡心嘔吐患者隨機(jī)分為甲旋二聯(lián)組(n=73)、甲氟二聯(lián)組(n=75)、甲氟旋三聯(lián)組(n=68)和對(duì)照組(甲氧氯普胺組,n=70)。采用WHO抗癌藥副作用惡心嘔吐分級(jí)標(biāo)準(zhǔn)、惡心嘔吐及干嘔指數(shù)評(píng)價(jià)量表(INVR)和多維度疲乏癥狀量表-簡(jiǎn)表(MFSI-SF)分別對(duì)患者用藥前、2周、8周進(jìn)行跟蹤測(cè)評(píng)。結(jié)果三聯(lián)組在用藥2、8周時(shí)止吐作用有效率分別為76.5%和94.1%,顯著高于對(duì)照組和二聯(lián)組(P0.05);三聯(lián)組在用藥2周時(shí)INVR評(píng)分顯著低于對(duì)照組和甲旋二聯(lián)組(P0.05),但與甲氟二聯(lián)組無(wú)統(tǒng)計(jì)學(xué)差異;三聯(lián)組在用藥8周時(shí)INVR和MFSI-SF評(píng)分均顯著低于對(duì)照組和二聯(lián)組(P0.05)。結(jié)論旋復(fù)花代赭石湯聯(lián)合西藥三聯(lián)用藥方案對(duì)腫瘤惡病質(zhì)惡心嘔吐患者,顯示了長(zhǎng)期穩(wěn)定的止吐效果,且能有效改善患者疲乏,值得臨床推廣。
[Abstract]:Objective to investigate the effect of Xinfen Huadai Zhe Shi decoction combined with metoclopramide and haloperidol on the nausea and vomiting of tumor cachexia. Methods in a prospective randomized controlled trial, 286 patients with nausea and vomiting of cachexia were randomly divided into three groups: medroxine group (n = 73), meflurane group (n = 75), trimeflurane group (n = 68) and control group (n = 70). WHO, (INVR) and MFSI-SF were used to measure the adverse effects, nausea and vomiting. The patients were followed up for 2 weeks and 8 weeks before treatment. Results the effective rate of antiemetic effect in the triple group was 76.5% and 94.1% respectively, which was significantly higher than that in the control group and the combined group (P0.05), and the INVR score in the triple group was significantly lower than that in the control group and the methyl-circumflex group at 2 weeks (P0.05), but there was no significant difference between the triple group and the meflorine group. The scores of INVR and MFSI-SF in the triple group were significantly lower than those in the control group and the second group at 8 weeks (P0.05). Conclusion Xinfu Huadai Zhe Shi decoction combined with western medicine has a long-term and stable antiemetic effect on patients with cachexia, and it can effectively improve the fatigue of patients. It is worth popularizing in clinic.
